Caughey and her team recently did,

turning a “Lunch and Learn” into a fun

“Launch and Learn” event!

Sponsored by Windsong Sailing Academy of Lake Lanier, Capt. Dave Crumbley facilitated an unconventional event that was like no other in-office or classroom training. Over the last several years, Capt. Dave has been using the lake and his 28 foot sailboat to provide corporate adventures. After talking with several dentists, Capt. Dave took his corporate venue and modified the event for dental professionals.

Not knowing what to expect, Dr. Caughey and her team showed up at Holiday Marina all spiffed up in their matching nautical shirts showing off their team spirit and enthusiasm before they stepped foot on the dock! The weather couldn’t have been more perfect…clear blue skies, 66 degrees, and 10-12 mile per hour winds created ideal conditions for this obscure adventure.

After a brief training on how to properly and safely get on the boat, maintain safety on the water, and other basic preliminaries, they were soon on the water. The first order of the day was to learn how to sail the boat under the direction of Capt. Dave. After each team member had an opportunity to rotate through each position and show competency following orders, the next ses-sion began.

Equipped with instructions, a map of the lake, and binoculars, they sailed in search of a mysterious treasure. Yes, it was a trea-sure hunt that would test many skills from reading navigation signs, completing team challenges, and learning about team com-munication, all while sailing the boat.

Planning their strategy as Capt. Dave motored the boat through the marina, Dr. Caughey and her team met their first chal-lenge when a sudden wind blew their map into the water. As their first team challenge, they made an attempt to retrieve the map, but alas, it soon sank beyond their reach to the bottom of the marina waters.

As everyone knows, a great plan always has a backup plan, and being the great captain that he is, Capt. Dave soon had a backup map in their hands, and on they went.

Wrist bands of various colors identified their unique “behavior style” giving them more information about themselves and the people that they work with each day. Refer-

ence to the different styles was made as the team took on the many challenges pre-sented to them…all while they continued to sail the boat seeking the lost treasure.

Reluctantly, each “mate” took their turn at the helm bellowing orders like, “Prepare to tack, mateys! Arrr!”, as they aimed to change the direction of the boat. Sudden gusts of wind, plentiful that day, drew excitement from the crew in anticipation of hitting maximum speed.

This active event continued to stimulate them physically, mentally, and emotionally – all while challenging their abilities to ac-complish new tasks. It didn’t take long for everyone to acquire their sea-legs and soon they began to look for “dark waters”, a sign of approaching winds, so they could cruise full sails at optimum speed.

Spared of walking the plank, this lively crew had an experience that will not only be an extraordinary memory for the rest of their lives, but also a demonstration of their team dynamics in an unfamiliar and fun environment.

This active event continued to stimulate them physically, mentally, and emotionally – all while challenging their abilities to accomplish new tasks.

DENTAL BIOFILMS: CONQUER THE ENEMY WITHIN

The wheels may be about to fall off the free ride that harmful bacteria have enjoyed in the human mouth. Oral health science now understands more than ever about how microbes communi-cate, coordinate, breath and eat. We also understand more about “who’s who” within biofilm colonies, and the protective power of the biofilm matrix itself. Gleaned from the latest biofilm research, this month’s Sunstar E-Briefs will help you understand why dental biofilms behave as they do, and how to protect your patients against harmful microbial activity. It is timely, relevant information designed to help improve the way you practice.

Think harmful bacterial are rogue bad guys? Think again.

Science reveals 99.9% of all bacteria live in biofilm communities. Within the oral cavity, biofilms prefer to settle in the gingival sul-cus because it offers a hard, non-shedding surface and rich supply of nutrients that includes gingival crevicular fluid and human food. Small grooves in areas between approximal tooth surfaces protect microbes against salivation, tongue movement and the effects of chewing. In addition, the moist, consistent temperature of the oral cavity creates favorable conditions for microbial communities to flourish.

The biofilm record of survival reaches back to the beginning of life on Earth. Certain bacteria have withstood eons of environmental challenge thanks to a layer of slime (exopolymeric matrix) that protects the microorganisms inside. Within that matrix, bacteria are able to adjust their metabolism and thrive—protected from viruses, amoebae and therapeutic antibiotics. Biofilm communities periodically release swarms of planktonic bacteria in search of other locations to attach and establish new biofilm colonies.

GRAND CANYON-SIZE PROBLEM

Sharing a microbe’s view of the oral cavity helps clinicians under-stand the vast coloniza-tion opportunities for biofilms. A bacterium is typically 1.0μ in length. The sulcus has an average depth of 0.3μ and width of 0.15mm, which, from a microbe’s viewpoint, is a pocket roughly twice as deep as the Grand Canyon.

As one example of colonization opportunities, overhanging crowns create an environment thought to encourage the development of microbial communities associated with the causes of gingivitis, periodontitis and caries. Even underneath tiny overhanging crowns that measure less than a half-millimeter, the environment is favor-able for a strong gathering of mixed-species anaerobic biofilms. As they begin to thrive, these microbial communities can lower the oxygen tension at the gingival boundary and provide a continuous source of pathogenic bacteria to populate the area. Overhanging crown margins are areas that dental hygienists should consider of particular interest when treating patients.

BALANCE OF POWER IN HEALTHY GINGIVA

Bacteria in a healthy gingival sulcus comprise small numbers of aerobic and anaerobic organisms. Populations of oxygen-depen-

dent aerobic species, such as Streptococcus, live in upper biofilm layers, while lower layers sans oxygen are home to anaerobic species Fusobacteria, Capnocytophaga, or the facultative anaerobe Acti-nomyces. When a patient’s hygiene practice falters, it can shift the microbial balance of the sulcus and allow bacteria to accumulate. Higher levels of bacteria trigger increased levels of excreted bacte-rial metabolic by-products, which are harmful to tissue and lead to inflammation.

Bacteria in the human sulcus have varying appetites for oxygen. Micro-aerophilic organisms prefer environments where oxygen concentration is low, while anaerobic bacteria exist in areas lacking oxygen altogether. Some anaerobic strains are particularly danger-ous because of their tendency to invade host cells; Fusobacterium nucleatum, for example, has been identified as anaerobic bacteria that may signal transition from health to disease. The disease pro-cess is supported by the spread of the “red complex” community comprising Treponema denticola, Porphyromonas gingivalis and Tannerella forsythus.

THE ORIGINAL CELLULAR NETWORK

Organized, community responses to stress have been a key factor in biofilms’ ongoing survival. Bacteria within biofilms communicate

by using chemical or even electrical signals—transmitted along so-called “nanowires”—that facilitate coordinated responses. And because of their stable spatial relationships, bacteria can exchange genetic material and cooperate in metabolic enterprises efficiently. Dental biofilm colonies are most harmful when allowed to remain intact, and should be disrupted by all possible means. Mechanical disruption remains the gold standard for biofilm control, and this method can include use of a soft pick, floss or interproximal brush (see sidebar).

The formation of bacterial biofilms occurs in all natural ecosys-tems, and as bacteria form organized communities on surfaces and at interfaces, the process is limited only by the availability of nutri-ents. If the plaque on the tooth surface and in the gingival crevice is not removed regularly by mechanical intervention, the special-ized biofilms that cause caries and periodontitis develop rapidly. Regular cleaning of tooth surfaces, above and below the gum line, is the key to effective biofilm management.

The foundation of good oral health will always be regular, thor-ough interdental plaque removal.

LIGHTEN THE LOAD

Countering interdental biofilm with mechanical removal is the definitive way to reduce a patient’s microbial load. Here are top tips to achieve optimal mechanical disruption:

Dental Floss• All types disrupt biofilm equally well. Floss holders, threaders, soft picks and power flossing instruments can be effective—especially for individuals with physical limitations.

Good Technique• Gently seesaw floss between contacts.• Curve floss around tooth in a C-shape.• Floss mesial and distal surfaces.• Slide floss up and down with pressure away from gingiva.• Use three to five strokes per surface.

Patient Compliance• Studies show interdental brushes are preferred to floss.• Clinicians can boost compliance by considering which method of mechanical disruption a patient prefers.

Floss Alternatives• Interdental brushes achieve compliance typically superior to floss, and can be used to apply therapeutic agents.• Interdental tips.• Disposable interdental cleaners are recommended for patients with busy lifestyles.• Water irrigation units clean gingiva and teeth, and can also deliver antimicrobials.

Mismanagement is often the reason for new business failure. Therefore, a wise business owner surrounds himself with the talented and experienced people it takes to make a business survive, grow, and prosper.

To run a business properly, you need to have expertise in a number of areas—fi nancial planning, accounting, law, taxation, insurance, and investment management, to name a few. Because it is often impossible for a professional in one of these areas to be an expert in all of the other areas as well, it is often desirable for business owners to work with a team of advisers.

Naturally, however, you’ll need a quarterback to direct and coordinate the advisory team. Typically, this person is your fi nancial planner. What is the fi rst thing a new president of the United States does after being sworn in to offi ce? He builds his cabinet, selecting the most qualifi ed persons to advise him on matters in which he himself is not qualifi ed. The secretary of defense counsels the president on military matters, the secretary of state counsels him on worldwide political matters, and the secretary of the interior counsels him on the environment. Thus, the president generally makes no decision until he is fully apprised by the experts. That’s what you need to do. You need to build your

own cabinet or team of professional advisers who can help develop and implement your business plans most eff ectively.

Tip: Banks are more likely to lend money to a business that is managed by people with extensive experience in the type of business you own. An advisory team that provides management assistance on a regular basis may help you get money more easily.

Members of Your Team

Your team may be made up of people from both inside and outside your company. The insiders may be part of your salaried management staff , while the number of outside advisers is largely a function of cost. When selecting advisers, you should be aware of their various professional designations, such as, certifi ed public accountant and certifi ed fi nancial planner, as well as the scope of their expertise.

Like any sports team, you need your basic players out on the fi eld at all times. These are the people who know what needs to be done and do their jobs well. For example, budgets need to be prepared by those who know how

Over the last several years, investors have grown accustomed to historically low interest rates. Ever since the Federal Reserve Board’s target fed funds rate—the rate at which banks lend to one another—hit a high above 19% in mid-1981, the long-term direction of rates has been downward. In the last decade, the Fed’s data* shows the target rate has never been much higher than 6%. And since December 2008, the Fed has kept it at a previously unheard-of level between 0.25% and zero to try to ensure that credit would be available to promote economic recovery.

Because bond prices typically rise when interest rates fall, that decline in yields has produced a bull market in bonds over the last decade. But what happens when the trend reverses? Even if they continue to remain relatively stable for a while, ultra-low interest rates have nowhere to go but up. When the economic recovery begins to show signs of strength, at some point the Federal Reserve Board will begin to raise the target rate again. When that happens, bond prices

also will begin to reverse their long-term direction. Here are some factors to consider in anticipation of future rising interest rates.

Bond Maturities: When Short is Sweet

When interest rates rise, long-term bonds generally feel the impact the most. In an extended period of rising interest rates, bond buyers become reluctant to tie up their money for longer periods, because they foresee higher yields in the future. The later the bond’s maturity date, the greater the risk that its yield will eventually be superseded by that of newer bonds. As demand drops and yields increase to attract purchasers, prices fall.

There are various ways to manage that impact. If you own individual bonds, you always have the option of holding them to maturity. In that case, you should suff er no loss of principal, unless the borrower defaults.

Special Players—Outside Advisers

Although your business may need a variety of advisers for diff erent purposes, some typical advisers include the following:

� Temporary, or for a Particular Purpose: Certain players (like the football kicker) are only needed at particular key moments. You may need a player to do only specifi c things (e.g., a C.P.A. to do the taxes each year). Perhaps you need to assemble a special team to handle a particular game (e.g., you are expanding or selling your business). Architects, attorneys, C.P.A.s, and brokers are some types of independent contractors that you can hire when your need occurs.

� Associates and Colleagues: Pick the brains of your associates and colleagues. As a business owner, you have probably joined groups of others who have similar experiences (e.g., Rotary, Small Business Administration). Take advantage of these opportunities to get advice from these “unoffi cial” team players.

� Accredited Accountant: An accredited accountant has passed an examination prepared by the Accreditation Council of Accountancy and Taxation. This is a national accounting accreditation board affi liated with the National Society of Public Accountants and the College for Financial Planning. Typically, accredited accountants specialize in small-business accounting.

� Attorney: An attorney has obtained a Juris Doctor (J.D.) degree from an accredited law school, and has received a license to practice law from a state bar organization. Attorneys are required to take a broad range of courses in

law school, typically, encompassing such areas as corporations, contracts, commercial law, and wills and trusts. Attorneys are particularly necessary when fi rst establishing your business and when designing contracts and other legally enforceable documents.

� CERTIFIED FINANCIAL PLANNER™ professional: A CERTIFIED FINANCIAL PLANNER (CFP®) professional has passed various courses in areas of investments and estate planning. The candidate must pass a comprehensive examination on these topics. Additionally, they are required to have three years of work experience and to participate in continuing education courses.

� Certifi ed Public Accountant: A Certifi ed Public Accountant (C.P.A.) has passed the American Institute of Certifi ed Public Accountants (AICPA) national examination, which tests an individual’s ability in accounting, auditing, business law, and taxation. A C.P.A. is licensed in the state where they practice.

� Chartered Financial Consultant®: A Chartered Financial Consultant® (ChFC®) has passed a fi nancial services curriculum, with emphasis on life insurance. A ChFC® must have three years of work experience and participate in continuing education courses.

� Chartered Life Underwriter®: A Chartered Life Underwriter® (CLU®) is specifi cally trained in life insurance. They must have three years of work experience and participate in continuing education courses.

� Personal Financial Specialist: A personal fi nancial specialist (PFS) is a C.P.A., who concentrates on fi nancial planning issues, and has met certain educational requirements, including continuing education courses. A test must be passed to obtain this designation.

� Stockbroker: A stockbroker is trained in

the basics of investment analysis, and the mechanics of trading. Stockbrokers must pass the Series 7 licensing exam (six hours, approximately 250 questions) administered under the Financial Industry Regulatory Authority, Inc. (FINRA), which allows them to advise you, solicit your business, and execute transactions on your behalf. Most states also require the Series 63 exam.

� Investment Adviser: An investment adviser is anyone who is paid to off er fi nancial advice.

How do You Build a Good Team?

Choosing your professional advisers can be one of your most critical business decisions. How much advice you need, want, and can aff ord are some of the issues you should consider when forming a team. Only you can decide the best mix of advisers for your team. You may want to follow the below steps.

Defi ne Needs and Roles

Break down your particular needs into component parts. Do you need a highly specialized sales force? How about a marketing department? Do you need legal advice? How about a fi nancial and tax expert? Determining what you need will make it easier to conduct your search.

You need to defi ne each adviser’s role and communicate this information to all parties involved. Specify the conditions when one professional should defer to the judgment of another. You may want to appoint your quarterback—one adviser who coordinates the overall eff orts of all parties to ensure that the “big picture” is kept in mind. Typically, the coordinator will be a fi nancial planner with a strong tax background, or perhaps an accountant with a fi nancial planning credential. This person is likely to be the one you’ll be dealing with frequently.

Find the players

Now that you understand the roles of the potential players on your team, it’s time to fi nd the players. Credentials, experience, reputation, expertise, and cost are the most important factors in selecting an adviser. When composing a team, however, there are some additional considerations. A qualifi ed player is one who is ready and able to join the team. Ability can only be determined by asking probing questions.

Check Their Credentials

•�Education: Be sure the candidate has the proper degree.

•�Training: Be sure the candidate has an appropriate number of years of recent experience and the appropriate professional designations.

A qualifi ed charitable distribution isn’t reported as taxable income or claimed as a charitable deduction. Similarly, the distribution won’t increase your adjusted gross income (AGI) for other tax purposes.

Here’s what you can accomplish tax-wise by making this move:

� Deductions for charitable donations may

also be restricted by other rules based on 30% and 50% of AGI. The IRA payout is exempt from these ceilings.

� If a taxpayer has carryover charitable deductions from the prior year, he or she may receive the full benefi t of the deductions this year. Otherwise, the regular charitable limits are applied before the carryover is allowed.

� The charitable distribution is never realized as taxable income. This can lower your AGI for the deduction fl oors

Charitable-minded retirees have another chance to do a good tax deed with benefi ts. The 2010 Tax Relief Act retroactively reinstates the tax break for “charitable rollovers” to Jan. 1, 2010 and extends it through 2011. � Willingness to Work with Other Financial Specialists: To get diff erent specialists to function as one unit, you need team players. Although your attorney is the expert on legal matters, they should be willing to discuss the legal ramifi cations of a decision with other professionals. This remains true even if another adviser prefers a diff erent approach. Be up-front about the team approach; make sure that each adviser is aware of the names, business addresses, and telephone numbers of the other advisers. In fact, one way to build a successful team is to let one adviser refer you to other professionals with whom they have a professional relationship.

� Ability to Communicate Ideas to You Cearly and Simply: It is vital for you to understand your advisers. Make sure that they can explain complex topics in simple language and are willing to provide such explanations until you feel comfortable with a particular decision.

Set any ground rules before you hire an adviser, and make sure that they know just what you expect.

� Personal Accessibility: If your accountant is too busy to take your phone calls or pay attention to the other advisers, they are probably not the best choice for your team. In terms of personality, you’ll want to deal with people with whom you feel comfortable.

� Your Own “Gut Feelings”: Your own instincts play a part in your decision. Do you like the candidate? Will the candidate be a good fi t? These questions can really only be answered by you.

Now that the team is assembled, the game plan should be fully understood by each player. Make sure your goals and any ground rules are clear. You should have occasional group meetings to review progress toward your fi nancial goals and to discuss major events that have impacted your overall plan.

What’s the Downside?

Commissions paid to business brokers, merger and acquisition specialists, investment bankers, and other experts can be expensive. However, the old adage remains true: You get what you pay for.

Retaining One Adviser

Cash fl ow is often a problem for start-up businesses. For these businesses, the only aff ordable option may be to hire one experienced business fi nancial planner to provide a wide range of advice, and plan to employ an attorney and an accountant only when they are absolutely necessary.

For growing businesses and liquid start-ups, the wisest course is to work with a team of advisers, with each adviser serving as an expert in his or her own area. In this way, you can be sure of receiving the most up-to-date strategies and the most legally accurate methods. Furthermore, by employing the team approach, fi nancial decisions should be made with the “big picture” or overview in mind. In other words, advice will be coordinated among the professionals to ensure that only the best business decisions are made.

� This tax provision might also benefi t taxpayers, who claim the standard deduction. It is the equivalent of taking a tax deduction as an itemizer.

Note that the tax break for charitable distributions applies to Roth IRAs as well as traditional IRAs. Roth IRA distributions to individuals who are over age 59½, are usually tax-free. However, a portion of a distribution may be taxable for a Roth in existence less than fi ve years. Therefore, it is usually better to use a traditional IRA rather than a Roth for this purpose. The tax break for charitable rollovers is scheduled to expire after 2011.

interest payments. That can make bonds less attractive to buyers. However, not all investments are hurt by higher prices. For example, commodities, such as oil and wheat, typically do well in infl ationary periods. In fact, increases in commodity prices are often what trigger a bout of infl ation. If you’re primarily interested in the overall value of your portfolio rather than a regular income stream, your fi nancial professional can help you explore whether you should consider diversifying into asset classes that tend to benefi t from infl ation and that might help counteract the potential impact of falling bond prices.

Though bonds are aff ected most directly, equities aren’t necessarily immune to rate increases. Though many companies borrowed money in recent years to take advantage of low rates and postpone the need to issue bonds for some time, those that haven’t may see their borrowing costs increase, which could aff ect their bottom lines. Even those that squirreled away cash could be hit, when they return to the bond markets eventually.

Also, if interest rates rise to a level that’s competitive with the return on stocks, that could reduce investor demand for equities.

Higher Rates Aren’t All Bad News

For those who’ve been diligent about saving, or who have kept a substantial portion of their investments in cash, higher rates could be a boon. Savings accounts, CDs, and money market funds are all likely to do better at providing income than they have in recent years.

The downside, of course, is that if higher rates are accompanied by infl ation, such cash alternatives might not keep pace with rising prices. And bear in mind, a money market fund is not insured or guaranteed by the Federal Deposit Insurance Corporation or any other government agency. Although the fund seeks to preserve the value of your investment at $1 per share, it is possible to lose money investing in a money market fund.

If a scholarship is used to pay for college tuition, fees, books, or required equipment, it is not taxable; however, if the scholarship is used to cover room and board, travel costs, or optional equipment, then it is taxable. The scholarship is also taxable if it is awarded as

payment for teaching, research or some other required service.

With most scholarships, recipients can decide how to apply the funds. While your fi rst instinct may be to apply it to tuition, fees, or books, thus making it tax free, this choice may impact your ability to claim the Lifetime Learning or the American Opportunity (formerly Hope) tax credits. These credits are based on the amount of tuition and fees paid. Therefore, if tuition and fees are paid in-part or in-full by a scholarship, you personally will have paid less when calculating your credit. Tax credits generally reduce your taxes dollar for dollar, so this could be a substantial savings.

This scenario has the most impact on your ability to claim the Lifetime Learning credit, calculated as 20% of the fi rst $10,000 of tuition and fees up to $2,000. For example, if you receive a large scholarship and apply it to these expenses, you may be left with less than $10,000 in eligible tuition and fees to count toward the credit.

The American Opportunity credit is calculated diff erently—100% of the fi rst $2,000 of tuition and fees, plus 25% of the next $2,000 of such expenses, up to $2,500. You can only take

one of these credits in a given year for the same student. Therefore, if you have multiple students in college, you may want to weigh your options with a tax adviser. Additionally, both credits have diff erent income phase-out levels. It is advisable to discuss your specifi c situation with a tax expert.

If the scholarship has no restrictions on how it can be applied, and you fall within the income limits to take the credits, consider calculating tax projections to determine your best option. First, try applying the scholarship to tuition, and then reduce the amount of eligible tuition that can be used to calculate the tax credits. Second, try applying the scholarship to room and board, paying income tax on the scholarship, but allowing all tuition to be counted when calculating the credits.

Calculating credits and determining how best to apply a scholarship can be diffi cult, even if you have a relatively simple tax return. However, the savings from taking advantage of federal tax credits—up to $2,500 in some cases—may make it worth your time to talk to your tax expert.
