� PREOPERATIVE PLANNING

To obtain the best results, preoperative planning using the

given templates is highly recommended. The templates

show the profile of the cups and of the cranial modules at

different eccentricities enlarged by 15%.

The templates also show the centre of rotation of the femoral

head both when using a spacer with protrusion (+ 5 mm) as

well as in all the other cases (+ 0 mm) (Figs. 1-2).

� ACETABULAR REAMING

Using the desired surgical technique expose the acetabulum

so as to have an adequate view for correct reaming.

Where necessary, remove the existing implant.

Then remove any osteophytes and expose the cotyloid rim

to get the best view of the anatomy of the bone to check for

any cavitary and/or segmentary defects.

It is important to have a clear view of the lower margin of the

acetabulum to be able to place the caudal hook correctly.

Start reaming the acetabulum using a reamer (Fig. 3) with a

smaller diameter than the one identified during templating,

mounted onto a reamer handle (Fig. 4).

Engaging the spherical reamer with the reamer handle

(Figs. 5-6):

1. Insert the reamer to correspond with the pins on the

locking lever.

2. Push the locking guide of the reamer handle downwards

and turn the reamer in an counter-clockwise direction by

a 1/4 of a turn.

3. Release the guide.

Push the locking guide again and turn the reamer clockwise

to remove it from its handle.

Insert the reamer into the acetabulum keeping it at an

abduction axis of about 45° and at an anteversion axis of

about 15° (Fig. 7).

Note. This angulation refers to the ideal angle of the cup.

There may however be some alterations in the acetabular

anatomy making it necessary to ream the acetabular cavity

at a different angle and therefore deviate from said values.

In any case we recommend aiming for this ideal angle when

reaming.

Gradually proceed with larger diameters with the reamer

until the subchondral bone is reached.

It is possible to prepare the housing for the cranial module

if applicable. This can be done by reaming the housing with

a reamer with a diameter corresponding to the diameter of

the definitive cup.

� ACETABULAR CUP TRIAL (WITH CRANIAL MODULES) AND INTRODUCTION

Having completed the preparatory step for the acetabular

housing, proceed with using the trial cup.

In the case of a Delta One TT cup without using a cranial

module, proceed with a check of the overall dimensions

by using a trial cup included in the acetabular reamer

instrumentation, (codes 9057.20.522 - 544), which has a size

corresponding to the last reamer used, which is screwed

onto the multi-purpose handle (Fig. 8).

The large openings in the trial cup allow you to check that

there is uniform contact with the acetabular bottom (Fig. 9).

For the Delta Revision cup, the overall dimensions can be

checked by using the trial cups (codes 9055.33.050 - 066)

screwed onto the multi-purpose handle (Fig. 10).

With these cups the position of the final cup fins can be

checked. If it is planned to use the cranial module (for both

Delta One and Delta Revision cups) screw the trial cranial

module (Fig. 11) corresponding to the size of the trial cup

(and to the planned eccentricity; 12 mm or 18 mm), onto the

trial cup.

Note. The Delta One cup sizes 50, 54, 58 and 62 ONLY can

be used for coupling with the cranial modules.

Tightening is done by locking a specific pin (Fig. 12) into one

of the three housings in the test cotyle (marked with circles

in (Fig. 13) and passing two of the screws (there are six in the

instrumentation) through the two holes in the cup nearest to

the polar opening (Fig. 14).

These screws will be housed in two of the fi ve threaded

openings in the cranial module.

The cranial module can be locked at three angles; at an angle

of 0°, - 30° and +30° with respect to the symmetrical axis of

the cups.

Position 0° is achieved by locking the pin in the central

housing (in the trial cup) (Fig. 15, a).

Positions +/- 30° are achieved by locking the pin in the right

or left hand housings (Fig 15, b-c).

The trial and fi nal cranial modules are tightened out of the

operating fi eld;

Afterwards, evaluate the dimensions of the trial and repeat the

operation of reaming or selecting the eccentricity of the cranial

module, where necessary.

You then implant a cup with the same rated dimension as the

last reamer used.

If for example a reamer with a diameter of 54 mm (effective

diameter) was used, then a cup with a nominal diameter of

54 mm will be implanted. This diameter is written on the pack

and marked on the prosthesis.

Take the cup (Delta One or Delta Revision), with the diameter

corresponding to the last reamer used, out of its sterile pack.

Screw the cranial module (if required) into the chosen

position. The final cranial module is fixed in the Delta One TT

cup in the same way as the test module. A third screw is used

for the Delta Revision TT cup which is placed in the cranial

zone of the module and passed through the opening in one

of the cup winglets (depending on the chosen direction)

(Fig. 16). It is better to partially screw in all the screws

first before tightening them, in order to avoid the risk of

misalignment between cup and module that could make

it difficult to insert the last screw. If you are using a Delta

Revision TT cup, bend the winglets with the appropriate tool

(after fixing the cup to the multi-purpose handle) to adapt

them to the anatomy of the acetabular (Fig. 17).

Care should be taken not to bend the winglets backwards

and forwards too many times and so break them. Adapt

the caudal hook in the same way (Fig. 18). If you are using

a cranial module this has to be fixed on the cup before

bending the winglets.

Impact the cotyle using the multi-purpose handle or the wrench

for Delta Cups (code 9055.51.015) with the adapters for Delta

Cups (codes 9055.51.110 - 130).

Keeping a firm hold on the positioner, tap it axially with a

hammer so as to firmly insert the cup, making sure that the

caudal hook is correctly housed in the obturator foramen

(Fig. 19).

Check that the cup is suffi ciently buried in the acetabular

housing and check the primary stability of the implant using

the stem of the positioner and operating gentle leverage stress

at the various planes.

Then unscrew the positioner and check the contact between

cup and acetabular bottom through the polar opening.

Then go on to the next stage: inserting the bone screws

through the cup openings.

� INSERTING BONE SCREWS

We recommend using the bone screws supplied with the

Delta system only (Fig. 20).

There could be problems with engaging the articular liner in

the cup correctly if other screws are used.

All the openings in the winglets can be used as long as there

is sufficient bone stock to anchor the screws.

Note. Using a cranial module in the Delta Revision cup pre-

cludes using two of the six holes in the cup and one of the

proximal holes in the winglets.

Two helix drills are available, one short and one long (Fig. 21),

used to drill holes 15 and 30 mm deep respectively.

Insert the helix drill with flexible mandrel into the drill guide

(Fig. 22).

Place the point guide into the preselected opening, keeping

to the direction of the opening and start drilling the osseous

tissue (Fig. 23).

Use the screw pliers (Fig. 24) to grip the screw and hold it

in position.

Drive in the screw with the screwdriver (Fig. 25).

As soon as the screw takes a hold of the bone, remove the

pliers and complete screwing. After inserting the screws

inside the cup, fix the cranial fins in the same way.

We kindly ask surgeons to pay particular attention

when implanting DELTA-REVISION, DELTA-REVISION

TT and DELTA-ONE TT cups with DELTA spacers

(codes 5885.15.5XX-5886.15.5XX).

The intra-operative internal protrusion of bone screws

must be absolutely avoided, in order to guarantee a

complete conical coupling between the spacer and

the acetabular cup.

Method for determining the correct bone screws

insertion:

1. Use a finger to assess that the screw/s head

is/are seated flush against the shell to prevent

improper seating of the liner and/or spacer. If the

bone screw/s is/are protruding, please tighten

again until seated correctly. If this maneuver is

not effective the screw/s must be removed and

re-positioned along a more appropriate axis.

2. Insert one of the Large liner trials into the cup

(from 9055.50.020 to 9055.50.035) and check for

its stability. If the trial liner is unstable, tighten the

bone screws and check again trial liner stability

until the liner becomes steady inside the cup.

If this maneuver is not effective the screw/s must

be removed and re-positioned along a more

appropriate axis.

An effective way to fix the cranial winglets is to insert the

screws nearest to the cup first and then insert the others.

� INSERTING THE TRIAL SPACER

At this point, if you are going to use spacers, you can insert

the trial spacers (Fig. 26).

Spacers are used to re-establish the centre of rotation and

the coverage and anteversion angles. Trial spacers (except

the angled 10° not protruded, code 9058.86.500) are fixed

to the cup in the desired position using the trial polar screws

included in the instrument set.

Method for ensuring correct trial and definitive spacer

insertion: insert the spacer with the polar safety

screw all at once, by holding the screw with the fixed

screwdriver (9095.10.222). Slightly tighten the screw

to ensure the correct conical alignment. Then loose

the polar screw and check for the spacer stability with

fingers. After verifying that the spacer is stable inside

the shell, firmly tighten the polar screw and perform

the trial reduction (Figs. 27-28).

NOTES FOR USING THE SPACERSDelta Revision and Delta ONE cups, sizes 50 to 66:

• The neutral spacer without angles and protrusions (code

5885.15.510) does not need a test component as one of the

large sized trial liners, with a head of an appropriate diameter

(included in the instrument set), can be used.

The final component of the neutral spacer without angles

and protrusions has one single opening that serves to house

the polar pin of the liners.

The final component does not need a tightening screw.

• The +5 mm neutral protruded spacer (code 5885.15.520)

has one single polar opening, which is used both for the

tightening screw and for the liner pin (in both test and final

versions)

• The spacer without protrusions angled at 10° (code

5886.15.500 - on request only) has a polar pin to replace the

tightening screw (in both test and final versions). The existing

opening is used to house the polar pin of the liners.

• The protruded spacer angled at 10° +5 mm (code

5886.15.505) has one single polar opening (in both trial and

final versions) and the tightening screw is screwed into this

opening (in both test and final versions).

• Spacers angled at 20° (both neutral and protruded) (codes

5886.15.510 - 520) have two openings and the one used to

house the trial and final screws is shown in the figure i.e.

the one located on the opposite side to the reference notch

and it is not threaded (Fig. 29 the opening which houses the

tightening screw is circled in red).

The trial and final spacers not angled (code 5885.15.510)

or angled at 10° without protrusion (code 5886.15.500 - on

request only) are not fixed with a polar screw and must

therefore be put into place by means of the fingers and

ensuring the firm locking of the Morse taper.

Insert the spacer with the polar screw all at once. Slightly

tight the screw to ensure correct morse taper alignment.

Position the spacer to replicate the correct positioning/

orientation as with the trial. Unscrew the polar screw and

check for spacer’s stability. Tighten the polar screw and

perform trial reduction.

Delta one cups, sizes 44, 46, 48:

The small sized neutral spacer without angles does not need

a trial component as one of the small sized trial liners, with a

head of an appropriate diameter (included in the instrument

set), can be used.

With the trial spacers the trial screws is used.

Final spacers only have one opening to house the final screw

(as the X small ceramic liner does not have a polar peg).

To change the liner position loose the screw and wth a

gentle percussion disassemble it from the cup.

Reorientate the spacer and then tight the screw.

� TRIAL REDUCTION

Acetabular time normally precedes femoral time, so we

recommend avoiding any contact between the acetabular

cup and the femoral stem, once the acetabular component

has been implanted.

To this end we suggest inserting the cup’s own trial liner into

it (or into the trial spacer if there is one) ensuring that it has a

size corresponding to the cup (Fig. 30).

Proceed with the trial reduction after placing the appropriate

head for testing into position (Fig. 31).

If you should find that the cup is in the wrong position with

respect to the femoral component (risk of impingement or

dislocation) you can use or correct the angle of the spacers

to change the anteversion and coverage angles.

Mark the osseous tissue with the electric scalpel in

correspondence with the direction identified as the one along

which the dislocation could occur. This will serve afterwards

as a guide for the protruded liner or angled spacer.

� INSERTING THE FINAL SPACER

It is COMPULSORY to use one of the spacers if you intend

to use a ceramic liner.

Spacers are optional (if you want to change the centre of

rotation and the coverage and anteversion angles) for

polyethylene or metal liners.

Thoroughly clean the inner housing of the cup before

inserting the spacer and make sure that the surrounding soft

tissues do not prevent the spacer from entering. The same

procedure as the one given for trial spacers (see paragraph

5) can be used for insertion.

Replicate the same positioning as with the trial.

� INSERTING POLYETHYLENE AND METAL LINERS

A Morse cone is used to fix all the liners of the Delta system

inside the spacers.

Thoroughly clean the inner housing of the cup (or the

spacer) before inserting the final liner and make sure that

the surrounding soft tissues do not prevent the liner from

entering.

Polyethylene and metal liners (Fig. 32) are fitted by gripping

the border between thumb and forefinger, with the forefinger

resting in the cavity.

In the specific case of a neutral polyethylene liner, push

towards the rim of the acetabular cup. It is a good idea to

accompany the pushing action by a slight rotation of the

liner.

Release the liner, pushing it gently into the cup with your

forefinger.

Once again make sure that the liner has been lodged

correctly inside the cup by feeling the perimeter.

Special care must be taken to ensure that the protruded

liners (lipped) are coupled correctly.

Do not use the protruding end (shoulder) to hold and

push the liner into the cup; this could cause misalignment

between cup and liner. Insert the liner by gripping the neutral

end between thumb and forefinger. Keeping your forefinger

in the cavity, push the liner down towards the bottom of the

cup. Use the liner impactor screwed to the beater and gently

tap the inside of the liner at a perpendicular angle to the

equatorial plane in order to complete coupling (Fig. 33).

Check that the neutral border of the liner does not protrude

out from the rim of the cup by feeling for the perimeter.

If the liner becomes jammed sideways it will have to be

removed by striking the metal rim of the cup with a flat-

bladed osteotome. The vibrations will shake the liner out of

its housing.

The polyethylene liner is removed by screwing a bone screw

to the bottom.

When you have finished, definitively reduce the articulation

prior to thoroughly cleaning the articular surfaces.

Note. All the Delta polyethylene, metal and ceramic liners

are fitted with a specific pin for polar opening occlusion.

� INSERTING THE CERAMIC LINER

IT IS NOT POSSIBLE TO COUPLE CUP WITH

CERAMIC LINER DIRECTLY.

Ceramic liners cannot be inserted into the cup directly and

one of the SPACERS has to be used.

Thoroughly clean the inner housing of the spacer before

inserting the cup and make sure that the surrounding soft

tissues do not prevent the liner from entering.

How to impact the ceramic liner:

Fix the Ceramic liner positioner (code 9058.85.090) to the

MEDIUM-LARGE liner positioner (code 9058.85.220). Screw

the liner positioner to the multipurpose handle (Fig. 35).

Assemble the ceramic liner to the ceramic liner positioner

(Fig. 36).

Impact the liner in the cup (Fig. 37).

Remove the handle by pulling it and making the ceramic liner

positioner disengaging from the ceramic liner.

Make sure that the liner has been lodged correctly inside the

cup by feeling the perimeter. The border of the liner must not

protrude out from the rim of the cup. The liner could break if

it is in the wrong position.

If the liner becomes jammed sideways it will have to be

removed by striking the metal rim of the cup with a flat-

bladed osteotome. The vibrations will shake the liner out of

its housing.

WARNING! Never re-engage the removed ceramic

liner, or a new ceramic liner in a housing that has

already been occupied by a ceramic liner beforehand.

Complete the implant by using a polyethylene liner

only.

The trial liner is placed inside the cup to use the 42 mm

double mobility.

The 40 mm liner is used into one of the spacers.

Insert the diameter 28 trial femoral head and the mobile trial

liner for double mobility onto the cone of the stem (Fig. 38)

and proceed with the trial reduction, to determine the length

of the head.

Note. With the double mobility can only be used the S,M,L

lenghts.

Insert the final liner into the cup for the final implant.

Place the head and the mobile liner in the Head-Liner Clamp

and tighten the screw until the two components are properly

joined together (Fig. 39).

Insert the components onto the cone of the stem and

proceed with final reduction of the implant (Fig. 40).
