Background of the study

Introduction

A computerized medical record management systemcomprises an information filter for assuring that recorddate units offered to the system for storage are completeand not redundant.

Ocampo's Pediatric and Maternity Clinic wasstarted in year 1996 and owned by Dr. RizalitoOcampo. It is located at San Bartolome NovalichesQuezon City. As up to date, the clinic has an average of25 patients per day.



Statement of the Problem

As time goes by, the advancement of the

technology in which it simplifies almost in every aspect

of a person in their daily lives. Working with computers

enables the business to work efficiency and less time

consuming. In the modern times, there are

establishments that use Automated Systems in their

businesses.

But there are some establishments that still using

manual procedures like Ocampo s Pediatric and

Maternity Clinic used Patients Record Charts to

collect their

patient’s

information, which is a paper-

based manual system.

Statement of Objectives

General Objective

The main objective is to provide easy, fast and reliablerecord keeping, updating and retrieving of patient record.

Specific Objectives

To develop a computerized medical record managementsystem software for Ocampo`s Pediatric and MaternityClinic.

To provide software that can store and retrieve thedata of the patients.

To ease and speed up the recovery of patients records.

To provide information of the patients that will last for a

long term.



Scope and Delimitations of the Study

Scope

The proposed system will help the

company to record and monitor theinformation of every transaction, to lessentheir paperwork and to reduce the inevitablemistakes due to the manual process. The

admin can also manage the user account andpasswords. The user can view the neededinformation only and can`t edit theinformation.

Delimitations

The software application that will be

developed by the researchers is not onlinebased application. In this software the doctorcan manipulate all the data of the patientswhile the secretary can add the information of

the patients only. The software applicationwill run only in windows 7 or more latestversions of windows.



Significance of the Study

The main benefactor of thisresearch is the Ocampo’s Pediatric and

Maternity Clinic; that it will ease theprocess of their transactions and at thesame time lessen the time it consumes.

The researchers will also benefit, for thevaluable experiences and knowledge it gives.

Brief Introduction

This chapter composes of different topics thatdiscuss the whole system and its functionalities. Thetopics of this chapter are system flowchart thatexplains how the data flows in the system and how

decisions are made to control the event. Data FlowDiagrams where is it explains how data is processedand transferred in a system. And also include theDatabase Attributes Illustration, EntityRelationship Diagram which represent therelationship of entities to each other, DatabaseDictionary which describes the contents, format, andstructure of a database and the relationship betweenits elements, Program Screenshots, and Program
