The SKY is the limit as DDC embarks on it’s 6th annual dance recital! Join us as we weave this year’s
dreams in the SKY and celebrate all the beautiful things that fly above us.
RECITAL LOCATION: Stillwell Theater at KSU

MANDATORY REHEARSAL DATES: MAY 3 & 9, 2020
*your dancer will be scheduled to participate on only one rehearsal date and one recital date unless enrolled in multiple classes with DDC. Rehearsal and recital schedule will be posted by mid March.
* THE REHEARSAL IS MANDATORY for the benefit of your dancer’s group as a whole. If your dancer cannot attend the stage rehearsal, unfortunately your dancer will not be permitted to participate in the show.

____$130 Late Recital: I have missed the recital deadline. I understand costumes are now on a first come first serve basis. I understand DDC must receive both form and payment immediately and if costume is unavailable that I will be refunded. I understand I must contact DDC immediately for payment arrangement.

My child has my permission to participate in DDC’s 2020 recital. By signing this form, I am agreeing to pay my recital fee in full prior to January 15 and that if my recital fee is not received by DDC prior to the deadline, I may be in jeopardy of receiving my costume late. I understand that my child is required to participate in one MANDATORY dress rehearsal on either Sun. May 3 or Sat. May 9 at the scheduled time TBA in March 2020. I understand that by my child’s participation in the show, my child may be photographed or filmed for promotional purposes. I understand that there is no refund for my recital fee and I have circled to my best knowledge, my dancer’s estimated size in May and that any alterations that might be needed will be my responsibility.

To register for recital, please fill out the form below legibly and send to DDC office. Please do not leave this form at your child’s school. DDC must receive both your recital form and recital payment by the deadline in order to receive your dancer’s costume in time for rehearsal. Please note that all
payments made to DDC will be applied toward tuition FIRST! Your account must be paid in full in order for your dancer to participate in this year’s show! All past due accounts must be paid in full before rehearsal in
order to receive your dancer’s costume and be eligible to participate in recital.
