“After reablement, I achieved my personal outcomes and was able to be independent at home and manage on my own again” – Service User Quote

- 211 (35%) service users finished reablement needing no further support.

- Cumulative saving of 7.1 hrs. per week per service user

“It was nice to get some of my skills back and be less dependent on Care at Home for my daily support” Service User Quote

- 304 (50%) service users progressed to Care At Home services after reablement with reduced support requirement

- Cumulative saving of 2 hrs. per week per service user

Since October 2010 the introduction of the Reablement Service has enabled our service users to lead a fuller and more independent life at home.

Current hours delivered per week at Dec 2011 are 9,822 hrsPre reablement original projections for Dec 2011 are 13,000 hrs

Perth & Kinross Council Reablement Service

- 85 SCO’s were trained to deliver reablement- “Once I completed my reablement training and began working with service users I found my job very rewarding” – staff member quote

- Every month 3 social care officers leave our service and it is their service users we transition to external providers.

Key Features of Rapid Response

Single Point of Contact through the Community Care Access Team (0845 301 11 20) during normal working hours, or outwith these hours via the OOHs Social Work Team or NHS Tayside OOHs Hub at Wallace town

Dedicated District Nurse will sit with the Community Care Access Team to respond to the health issues of people in crisis

The Rapid Response Service uses a multi-disciplinary model to ensure individuals referred at times of crisis access the kind of support they need, when they need it.

The Rapid Response Service is available across the whole of Perth and Kinross including additional Rapid Response SCO capacity
