How Do I Request A Transcript?

1. We MUST have your (blue) signed Transcript Release Form in order to send any of your records. (Do not write college names on form.)

2. Make sure checklist is COMPLETE.3. Sign the form on the door of the Guidance

Office BEFORE you submit checklist.

Students are excused with the understanding that their parents are responsible for them during the day they are absent from school. However, students are representing our school and must obey all school rules and regulations since the excuse is for educational purposes.

Permission is not granted until this form is signed by the counselor then by the principal.

One copy of this form will be given to the main office and serve as an excuse; the other copy will be place on file in the guidance office.

The use of this slip is limited to two (2) days per school year. Only seniors are permitted two (2) excused days for college visits.

The student must bring to the counselor a letter of verification from the place of the visit.
