What Can You Expect From CAH PIR?

Central Adelaide and Hills Partners In Recovery (CAH PIR) aims to assist services and supports to work in a more collaborative, coordinated and integrated way for people with severe and persistent mental illness and complex needs, and their carers and families.

CAH PIR works in partnership with the community, primary health care providers and partner organisations to provide high quality, person-centred, equitable and accessible healthcare and support to our local community.

Support Facilitators will work with clients to identify how services could be better coordinated, and strengthen the relationships between their services.

Referral to Central Adelaide and Hills Partners In Recovery can be made by calling 1800 894 448 and you will be guided through the referral process.

Alternatively, you can request a copy of the Central Adelaide and Hill Medicare Local Mental Health referral form by emailing [email protected]

You can also ask your support worker to assist you to put a referral into CAH PIR.

You can also ‘walk in’ to one of the agencies listed in this brochure and ask for a referral to be sent to CAH PIR.

The completed referral form can be sent via fax, email or post. Once received, a CAH PIR staff member will contact you to discuss your referral.

Partners In Recovery eligibility criteria includes:

• You have complex needs and require support from multiple services

• You have, or may have, a mental illness that is severe and persistent

• You have had recent contact with services and they are concerned about your mental health and/or other issues

• Existing services and coordination are not meeting your needs

• You consent to being involved in CAH PIR

• We will involve you and your carer (if you wish) in planning and decision making around your support

• We maintain hope that you can receive the appropriate supports and services you require

• We are recovery focussed

• Support Facilitators to navigate the system with you and on your behalf
