Kaiser Permanente uses the same quality, member experience, or cost-related measures to select practitioners in Marketplace Silver-tier plans as it does for all other Kaiser Foundation Health Plan (KFHP) products and lines of business. Members enrolled in KFHP Marketplace plans have access to all professional, institutional and ancillary health care providers who participate in KFHP plans’ contracted provider network, in accordance with the terms of the members’ KFHP plan of coverage. All Kaiser Permanente Medical Group physicians and network physicians are subject to the same quality review processes and certifications.

A narrow network is defined as a tailored, tiered or high-performance network that has 80 percent or less overlap with a health plan’s broadest network offered to a member in any type of plan offered by that health plan. Kaiser Permanente does not have a narrow network with respect to its Marketplace plans. Members enrolled in Kaiser Foundation Health Plan (KFHP) Marketplace plans have access to the same provider networks as members enrolled in all other KFHP plans.

Step 1. Choose Your Doctor Good health care begins with you building a relationship with your personal physician. Your doctor is your health care advocate, your direct link to all Kaiser Permanente facilities, and your source for referrals to specialists.

Select your personal physician from any of our available providers. You can change your personal physician at any time and for any reason.

To choose a provider:

1. Select a medical location where you plan to receive services. Most members select a medical facility that is convenient to home or work.

2. Choose a doctor from one of three primary care options:

• Family Medicine cares for members of all ages and specializes in caring for entire families.

• Internal Medicine specializes in medical and preventive care for adults.

• Pediatrics focuses on the specialized needs of children from birth to the age of 21.

3. To choose and change your doctor, simply call your facility or go online at kp.org/chooseyourdoctor.

Each personal physician listing in this directory includes a website address that will link you to more information about our doctors. Step 2. Making appointments Two easy ways to schedule your appointments:

• Visit kp.org/appointments on your computer or use the Kaiser Permanente mobile app, and tap Appointments on the app dashboard. You can schedule office or phone visits with your primary care physician and same-day adult care.

• Call your doctor’s office.

Preparing for your appointment • Check in with your Kaiser Permanente member ID card and a valid photo ID.

• If you have additional health insurance in addition to Kaiser Permanente, please bring the insurance card with you.

• Bring the names of any medications you are taking, including non-prescription medicines, so we can make sure that your prescriptions are uninterrupted and your doctor has all the information needed to care for you.

• Tell the doctor about any treatments you are currently receiving, and feel free to ask questions.

To cancel an appointment, visit kp.org/ appointments or call the 24-hour appointment cancellation line of your Kaiser Permanente medical location. If you’re bringing in a child that is not your own, please get an authorization form from Member Services or have a notarized Health Care Power of Attorney form.

Step 3. Paying for services When it’s time for your appointment, be prepared to pay for your services. Your portion may include a copayment, coinsurance, or deductible.

Charges for office visits, labs, X-rays, other tests, procedures, or prescription medication copayments, are due on the same day that you receive services.

You will be informed in advance if any deposits or prepayments are required prior to certain high-cost services or items related to scheduled procedures.

Pay with cash, personal check, debit or credit card (Visa®, MasterCard®, Discover®, and American Express®).

You may be billed supplemental charges for services performed after you’ve paid and left the clinic. For example, your doctor may need to send tissue samples or specimens for further testing.

• You can track costs for services you’ve received at kp.org/outofpocket.†

• Get a personalized cost estimate at kp.org/ costestimates. Before you come in for care, use this tool to find out what you can expect to pay out of pocket for common exams, tests, and services.*
