Conclusion : Très bonne valeur prédictive négative de la TEP pour nodule > 1 cm.
Si nodule > 1 cm non-fixant = Surveillance
(à considérer en fonction de la probabilité pré-test)
• 40 études retenues ; Un total de 1474 lésions (nodules ou masses) ; Prévalence médiane de malignité 72% ; Construction de courbes ROC
Limites de la méta-analyse: Peu de nodules < 1 cm ; Pas d’info sur nodules semi-solides
Limites de la TEP
• Risque de faux négatifs dans :
- Carcinomes « bronchiolo-alvéolaires »terme obsolète faisant référence à différentes entités : adénocarcinome in-situ ;
adénocarcinome micro-invasif ; adénocarcinome invasif lépidique-prédominant; adénocarcinome invasif mucineux
- Tumeurs carcinoïdes typiques
- Certaines lésions kystiques à paroi fine
- La sensibilité est limitée pour les nodules entre 8 et 10mm (Seule l’analyse
visuelle a de la valeur)
• La TEP est inadéquate pour les nodules < 8 mm (mais ceci pourrait changer avec les nouvelles machines TEP).

nécessite la vigilance.
Elle n’est utile que pour des lésions >1,5 cm (mesure faussée pour les petites lésions par l’effet de

Exemple d’un nodule de 9mm que l’on aurait pu
qualifier de négatif sur la base du SUV (1,5)

Importance de l’imagerie dans la prise en charge
du Cancer Bronchique Non à Petites Cellules
• Recherche de métastases à distance pour éviter une chirurgie lourde et inutile
• Recherche avant thoracotomie d’une atteinte ganglionnaire médiastinale N2 ou N3 qui impose une chimiothérapie néoadjuvante ou une (radio)-chimiothérapie exclusive
• Préciser le statut T
• Préciser les champs en cas de radiothérapie
Statut T : Il prend en compte : la taille de la tumeur, l’invasion locale, la
proximité à la carène, une atélectasie, un nodule dans le même poumon

La TDM de l’examen TEP-TDM ne remplace pas
une TDM de qualité diagnostique
La TEP est effectuée en respiration libre. La partie TDM doit être
effectuée dans les mêmes conditions. Une TDM acquise en inspiration profonde
provoque des artefacts sur les images TEP (artefacts de correction d’atténuation)
et sur les images de fusion (mauvais recalage)

Un examen TEP négatif permet de s’affranchir de la médiastinoscopie.
A nuancer : en cas de tumeur centrale, envahissement hilaire, ganglions ≥ 16 mm en petit axe en TDM, et de faible métabolisme de la tumeur primitive.

Conclusion : devant un Foyer TEP (adénopathie N2 ou N3), une confirmation est nécessaire (par médiastinoscopie, biopsie guidée par échographie endo-bronchique, ou trans-oesophagienne).

N.B. un nodule controlatéral malin peut être une métastase du primitif connu
ou un 2ème primitif (importance pronostique et pour la prise en charge)

Rôle important de la TEP/TDM corps entier
au 18FDG
• Découvre une atteinte métastatique non vue à l’imagerie conventionnelle chez environ 15% des patients.
• Caractérise une hypertrophie surrénalienne vue en TDM.
• Plus sensible que la scintigraphie osseuse pour les lésions osseuses et ostéo-médullaires
• Limite: Elle est peu sensible pour les métastases cérébrales. « Nécessité de compléter le bilan par une IRM ou une TDM injectée »

The use of fused PET/CT images for patient selection and radical radiotherapy
target volume definition in patients with non-small cell lung cancer: Results of a
prospective study with mature survival data.

• MATERIALS AND METHODS: Patients still eligible for radical RT after conventional staging underwent RT-planning PET/CT and, if disease was still treatable to 60Gy, they entered our planning study, where visually-contoured tumour volumes derived with and without PET information were compared.
• RESULTS:
• Of 76 eligible patients, only 50 received radical chemoRT after PET/CT while 26 (34%) received palliative therapies because PET/CT detected advanced disease.
Without PET, FDG-avid tumour would reside outside the planning target volume (PTV) in 36% of radical cases.
