B-2 What Are the Promising Targets for Future Therapeutic Approaches?
Stuart, et al. Int J Gynecol Cancer 2011;21:750–5
• The most promising targets in clinical trials are angiogenesis and homologous recombination deficiency.
• To select patients for trials investigating these targets, predictive biomarkers are required. Understanding mechanisms of resistance is a priority.
• Other promising targets currently being studied based on ovarian cancer biology include:
– PI3-Kinase and Ras/Raf pathways
– Folate receptor
Angiogénesis: Un paso muy inicial
•Necesidad de aporte sanguíneo para crecer (1-2 mm).
•Asociada a ascitis, carcinomatosis y peor pronóstico
• Hay un nivel 1 de evidencia?
• Es esa evidencia clínicamente significativa?
• Tenemos todas las preguntas resueltas?

• If PPS is longer than 12 months, there is a less than 30% chance that a trial will report a significant OS, even after reporting a PFS improvement at a high level of significance (p<0.001)
• The influence of PPS means that a lack of statistical significance in OS does not imply lack of improvement in OS

PFI is defined from the last day of platinum until PD
The following subgroups should be considered:
Progression while receiving last platinum based therapy or within 4 weeks of last pt dose
Progression-free interval since last line of platinum of < 6 months
Progression-free interval since last line of platinum of 6-12 months
Progression-free interval since last line of platinum of > 12months
ILP> 12 meses

Receptores de Folato
Tumour cells depend on folates: DNA synthesis and repair
Antifolates: Aminopterin, Metotrexate, Pemetrexed
Folates transport mechanism: RFC: Major transport system. Low affinity
PCFT: Transport under acidic conditions
FR (α,β,): not expressed in normal cells. High affinity Found in: kidney, liver, lung, thymus, placenta, choroid plexus
Tumour cells overexpressing FR (FRα): Epithelial: 80%
Related with stage and agressively growing subtypes
CEO: RFα como diana
MoAB High specificity. Not related with free folate
Low immunogenicity
Ex: Farletuzumab
Conjugates High afinity to receptor Folate-Active drug: Vintafolide
Folate-Imaging compounds: Etarfolatide
T –cells: adoptive immunotherapy Target cells recognized by T-cells (CAR-T cells)
Nanotecnology Liposomes, lipid nanoparticles, polymeric nanoparticles…

Conclusiones
• La supervivencia en Cáncer de Ovario es limitada.
• Bevacizumab en primera línea retrasa la recidiva en 6 meses.
• En recurrencia las opciones de curación son casi nulas.
• Nuevos fármacos (Trabectedina, Bevacizumab) obtienen beneficios significativos.
• Con excepción de angiogénesis y BRCA las alteraciones moleculares en cáncer de ovario, son poco frecuentes.
• La ausencia de biomarcadores predictivos dificulta la elección del tratamiento.
