This leaflet is about becoming aMember of Cheshire and WirralPartnership NHS Foundation Trust (CWP).CWP is a Foundation Trust that provides health services for local people in partnership with other organisations.

Our services include:• Mental health services• Learning disability services• Physical health services

Who can be a member?Anyone aged 11 years old and over who has an interest in the work of CWPcan become a member of the trust.

Our aim is to get as many people as possible who use CWP services, their carers and families, to join us as a member.

This will help make sure a variety of voices are heard and involved in our services.

How involved can I get?We appreciate that all of our members are different and will want to have different levels of involvement with our Trust.

How involved you become is completely up to you.
