Mechanism of actionThe counter - irritant / Gate Control TheoryTraditionalthinkingisthatthepain-relievingmechanismofactionofmenthol,aningredientinBiofreezePainReliever,resultsfromacounter-irritanteffect.Simplyput,acounter-irritanthasaneffectofoverridingnoxiouspainsignalstravelingtothebrainthroughaprocessmodulatedbetweenpain-transmittingandnon-paintransmittingneurons.Thisprocess,knownas“gatecontrol”or“gating,”wasfirstdescribedbyMelzackandWall.2Gatecontroltheoryisbasedontheunderstandingthatpainistransmittedbytwokindsofafferentnervefibers.OneisthelargermyelinatedA-deltafiber,whichcarriesquick,intense-painmessages.Theotheristhesmaller,unmyelinated“C”fiber,whichtransmitsthrobbing,chronicpain.Athirdtypeofnervefiber,calledA-beta,is“nonnociceptive,”meaningitdoesnottransmitpainstimuli.ThegatecontroltheoryassertsthatsignalstransmittedbytheA-deltaandCpainfiberscanbethwartedbytheactivation/stimulationofthenonnociceptiveA-betafibersandthusinhibitanindividual’sperceptionofpain.Morerecently,researchershavediscoveredthatmentholfurtherstimulatestheTRPM8coldreceptors.Eventhoughithaslongbeenacceptedthatcoolingandcoldcaninduceanalgesia,themechanismhasbeenpoorlyunderstood.Now,asscientistshavecontinuedtoisolatethesespecificheat-sensitivetransientreceptorpotential(TRP)channelswithinoursensoryneuronswearenowabletohaveamuchbetterunderstandingofthebody’scutaneoustemperaturedetection.WiththeisolationandfurtherstudyofthesespecificTRPM8receptorsthereisstrongerevidencesupportingthebenefitsachievedwiththecoolingeffectofmentholinfunctioningasatopicalanalgesicwhileactivatingthesereceptorsandtheresultingconsequentpainrelief.
