2.12 PACKAGE SERVICES

2.12.1 CUSTOM BIZSAVER

A. DESCRIPTION AND AVAILABILITY

1. Custom BizSaver is a package of Guidebook services available to business customers with 1 to 10individual Business Flat Rate access lines (access lines) as described in Part 4, Section 2 of thisGuidebook.

2. Custom BizSaver is available to business customers who are new customers or who meet thecriteria for one of the following categories of eligible customers:

a. Retention: Applies to all existing customers.

b. Winback: Applies to all customers who have discontinued their business service with the Companyfor the purpose of establishing service with a Competitive Local Exchange Carrier (CLEC) operatingwithin the Company's territory and who are returning their service to the Company.

c. Win: Applies to customer who established their service with a CLEC operating within theCompany's territory and who now wish to establish their service with the Company. Win does notinclude customers who established service with a CLEC that is an affiliate of the Company.

2.12.1 CUSTOM BIZSAVER (Cont’d)

B. REGULATIONS

1. Custom BizSaver is available to all eligible Retention, Winback and Win Customers, describedin A. preceding, and new customers.

2. Custom BizSaver is available to business customers who subscribe to 1 to 10 individualBusiness Flat Rate Service access lines, as described in Part 4, Section 2 of this Guidebook.

3. All access lines on the billing telephone number must subscribe to Custom BizSaver.

4. Custom BizSaver is not available with other products or promotions providing non-recurringcharge (NRC) waivers or discounts or access line, usage, or features, except as provided for inthis Guidebook.

5. Customers must verbally agree to a 1-year term commitment for Custom BizSaver, or whereapplicable, must agree on-line when using an authorized Company website, where available.

In lieu of a one year verbal agreement, customers may elect to sign a written two or three yearterm agreement and receive pricing denoted in C.2, following.

6. Upon expiration of the one, two or three year term, the customer will be billed at non-discounted rates for the individual package components unless the customer expressly electsanother service or plan or has previously opted for a renewal option as specified in 7. following.The renewal option in 6. following, is not available to customers opting for the written two orthree year term agreement.

7. At the time of the initial subscription, the customer may elect to renew Custom BizSaver for upto two (2) additional 1-year terms under the same terms as the initial subscription. If thecustomer elects the renewal option, a renewal letter will be sent prior to the expiration of each1-year term. The customer must contact the Company to cancel the renewal; otherwise, thenew 1-year term will take effect upon expiration of the previous term.

Upon completion of the final renewal term, the customer will be billed at non-discounted ratesfor the individual package components unless the customer expressly elects another service orplan.

8. If the customer discontinues their Custom BizSaver package before the expiration of their one, twoor three year term, an Early Termination Charge will be assessed equal to 50% of the monthlyrecurring charges for each month remaining in the term agreement.

The customer may reduce or increase the number of lines in the Custom BizSaver package withoutincurring an Early Termination Charge provided the Custom BizSaver package is not completelydiscontinued. A new one, two or three year term will begin when the number of lines changes underthe billing telephone number of the Custom BizSaver package to which the customer subscribers.Written two and three year term agreements will require the customer to sign a new two-year orthree-year written agreement.

9. Custom BizSaver Early Termination Fee Waiver

Eligible customers will received a one-time waiver or refund of early termination fees associated withCustom BizSaver. Eligible customers include business customers who terminated their CustomBizSaver service for the purpose of establishing service with a Competitive Local Exchange Carrier(CLEC) and who have now returned to the Company and subscribe to Custom BizSaver with a termequal to or greater than the term of the previously terminated Custom BizSaver agreement. The BillName on the billing telephone number must be the same as on the prior Company account withCustom BizSaver.

10. Early Termination Fees are not applicable if during the Custom BizSaver term period the customerconverts to another Company access and local usage plan with a term equal to or greater than theremaining Custom BizSaver term plan.

11. Win and Winback customers who commit to a one, two, or three year term agreement will have themonthly recurring charges for Custom BizSaver waived on all eligible lines for the first month ofbilling. This waiver of charges is not applicable in the first month of any subsequent renewal termor for first month of the second or third year in a two or three year term agreement.

12. Hunting is an optional feature for the Custom BizSaver Multiple Line option. If Hunting is selectedafter the initial installation of Custom BizSaver, non-recurring charges will apply per Part 3, Section1 of this Guidebook.

13. Custom BizSaver is not available with other products/services or promotions that provide non-recurring charge/service charge waivers or discounts on the access line, usage, or features, exceptas provided for in this Guidebook.

14. Two and Three Year Term Agreements

a. Custom BizSaver is available for a two or three year term provided the customer agrees andsigns a written term agreement.

b. Rates for Two Year and Three Year Terms are specified in C.2, following.

c. Except for B.6, preceding, all Regulations contained herein apply to the two and three year termagreements.

A. DESCRIPTION AND AVAILABILITY

1. uSelectsm 3 offers residence customers a combination of residence Custom Calling 2000 andCustom Calling Services at a package rate.

2. uSelectSM 3 consists of the following services:

● Caller ID, and

● The customer must also select two features from the following list of component services:

● Call Screen● Call Return● Priority Ringing● Repeat Dialing● Three-way Calling1

● Speed Calling 8● Call Forwarding1

● Call Waiting1

● Distinctive Ring1

● Intercom Plus1

● Privacy Manager

3. uSelectSM 3 is available to any residence customer where Caller ID and at least two componentservices are available. The component services may be purchased individually at their standardtariff rates.

4. uSelectSM 3 may be included in other packages or bundles that are marketed under other names.

a. Limitations:

1. uSelectSM 3 may be ordered either on the main/primary access line when the customerpurchases only one access line, or on any additional access line, provided that thecustomer also subscribes to 2-line uSelectSM 3 on the primary line.

2. 2-Line uSelectSM 3 is available on a residential customer’s main/primary line when thecustomer subscribes to an additional line billed to the same account. Both lines must be ofthe same class of service. 2-Line uSelectSM 3 may only be billed once per account and thefeatures must be provisioned on the main/primary line; the features cannot be splitbetween multiple lines. If the customer discontinues the qualifying additional line andretains the main/primary line features, 2-Line uSelectSM 3 will revert to uSelectSM 3 at ratesset forth herein.

3. Residence customers currently subscribing to uSelectSM 3 on their main/primary line andone or more additional lines billed to the same account may request billing at the 2-LineuSelectSM 3 price on the main/primary access line.

4. The uSelectSM 3 and 2-Line uSelectSM 3 component services may only be purchased asa monthly subscription. Usage Sensitive Call Management Services and charges arenot included in the monthly package prices.

5. The component services of uSelectSM 3 and 2-Line uSelectSM 3 are provided on a perline basis. All services must be purchased on the same line in order for the customerto be eligible for the package price.

6. Customers subscribing to uSelectSM 3 or 2-Line uSelectSM 3 will benefit from thepackage price until they disconnect Caller ID or one or more of the package componentservices.

7. Subscribers will continue to benefit from the uSelectSM 3 and 2-Line uSelectSM 3 prices ifthey disconnect one or both selectable component services and concurrently replacethose disconnected services with other services from the selection list. Any applicablenonrecurring charges may apply to feature changes.

8. If the customer subscribes to more than two but less than five component services onthe selection list, the two most expensive services will be included in the package.Services purchased in excess of those purchased as components of the packages willbe charged at the Guidebook rates and may include any applicable multi-featurediscounts.

b. Exceptions:

1. Discounted monthly rates for any other combinations of the services provided in thepackages on the same access line, as specified elsewhere in this Guidebook, do not applyto the component services selected by the customer within the packages.

2. Nonrecurring charges do not apply to the installation of uSelectSM 3 and 2-Line uSelectSM 3,but such charges may apply to the installation of Caller ID and selectable componentservices.

2.12.4. uSelectSM Standard

A. DESCRIPTION AND AVAILABILITY

1. uSelectSM Standard offers residence customers a combination of residence CustomCalling 2000 and Custom Calling Services at a package rate.

2. uSelectSM Standard customers must select four (4) features from the following list ofcomponent services:

● Call Forwarding1

● Call Waiting1

● Distinctive Ring1

3. uSelectSM Standard is only available to a residence customer when it has beendetermined that Caller ID cannot be provisioned at the customer’s location and at leastfour (4) component services are available. Component services may be purchasedindividually at their respective Guidebook rates.

2.12.9. MILITARY RECONNECTION PACKAGE

A. DESCRIPTION

This is a consumer package service that is available to military reservists who seek reconnection of theirresidential telephone service after returning from overseas military duty. The Military Reconnection Packageprovides the reservists who are returning from overseas duty with a waiver of their installation charges (e.g.,service order charge and central office line connection charge) when they order a new residential telephoneline and at least one custom calling feature at the same time.

2. REGULATIONS

a. Customer must concurrently purchase one residential flat rate basic access line plus a minimum ofone custom calling feature.

b. Utility will waive the installation charges associated with the new residential line and feature.

c. Eligible customers are military personnel involved in the following conflicts:- Operation Iraqi Freedom (Iraq)- Operation Enduring Freedom (Afghanistan)- Other overseas military operations- Move to a new location within the Company’s service area.

d. The customer must self-identify as a military personnel, and provide basic information including thename and contact number of their commanding officer, and the date of their qualifying military orders.

e. Customers must purchase at least one call management feature (Custom Calling Services or CustomCalling 2000 Services) to be eligible.

f. This package will be available from December 21, 2005 to November 30, 2006.

A. DESCRIPTION

This consumer package service is available to residential customers who have their local telephoneservice with another carrier and who now bring their local telephone service to the Utility. TheAccess Line & Feature Bill Credit package provides a $10 per month bill credit for each of four(4)months to customers who bring their local telephone service from a competitor and purchase aminimum of one (1) custom calling feature (from a designated list) at the same time.

B. REGULATIONS

a. Customer must concurrently purchase one residential access line plus a minimum of one customcalling feature (from a designated list).

b. The approved list of custom calling features are as follows: Caller ID, Call Waiting, Call Return,3-Way Calling, Speed Call 8, Call Forwarding, Repeat Dialing, Call Screen, and Priority Ring.

c. The customer must subscribe to a minimum of one feature from the approved list, but maysubscribe to more than one, including any of the approved features within packages.

d. If the customer discontinues service or cancels their feature subscription for any reason prior toreceiving all four months of bill credits, the customer will forfeit any monthly bill credits notreceived. The bill credit benefit will not exceed $40.00 per customer.

e. This offer is not available to customers who have local service with an affiliate of the Utility.

f. This package will be available from January 23, 2006 to January 22, 2007.

This is a consumer package service that is available to eligible Residential customers who agree to remain acustomer of the Company when they move 1) within the AT&T Nevada service area or 2) from an AT&TCalifornia service area to an AT&T Nevada service area. The Customer must also have or add Caller IDService according to the regulations below. The Customer will receive a one-time credit equal in value to thenonrecurring Service Ordering Charge and Central Office Line Connection Charge per line. The credit willappear on the customer’s monthly bill following acceptance of this offer.

2. REGULATIONS

a. The components of this package are provided in accordance with the Regulations in the AT&T NevadaTariff P.U.C.N. for exchange access services.

b. The customer must keep their existing residential access line, retain their existing Caller ID service, ororder new Caller ID service.

c. The customer must have or order Caller ID service on at least one line in order to receive the ServiceOrdering Charge credit. The customer will receive the Central Office Line Connection Charge credit oneach line that has Caller ID Service on it.

d. The maximum number of lines that qualify for the offer is three.

1. Business Local Calling Assurance is available to business customers with 1 or 2 business lines whoagree to a 12 month term limit and commit to the Access Line Service, Unlimited Local UsageService, Caller ID, and Call Forwarding Service at the package rate.

2. Business Local Calling Assurance is not available on FX Service, Remote Switching Service, WATSaccess lines, PBX Trunks, Centrex or Semi-Public Coin Services.

B. REGULATIONS

1. A customer may subscribe to one agreement for Business Local Calling Assurance at a givenlocation. A customer may have up to 2 lines maximum per location subscribed to a Business LocalCalling Assurance agreement.

2. Customers will receive a waiver of normally applicable Service Ordering and Line Connectionnonrecurring charges (NRCs) associated with the establishment of local exchange access lines thatare ordered at the time of initial subscription to Business Local Calling Assurance. Standard NRCswill apply to services added after the initial order.

3. The 12-month term is only available as a 12 month oral re-subscribe option. Under the 12-month re-subscribe option, the plan will renew for 12 month intervals. A maximum of two 12-month terms areavailable after the initial 12-month term. The customer will receive written notification of theirselection outlining the details of the agreement, plus subsequent notifications regarding their re-subscribe options prior to the expiration of each 12-month term. Customers are under no obligation tore-subscribe after any 12 month term period.

Easy Rate is an optional business package that includes a network access line, customer selected optionalfeatures, optional Hunting and unlimited local calling. Customers must subscribe to and maintain aminimum of 40 Easy Rate Lines.

B. Terms and Conditions

Easy Rate is available to business customers who agree to a month to month subscription of a minimum of40 business lines per account from those states where AT&T operates as an Incumbent Local ExchangeCarrier, where Easy Rate is available and who agree to a month to month arrangement.

All lines on the account must be subscribed to Easy Rate. If the customer requires other types or classes ofservice they must be established on a separate account. Customers must request Easy Rate on their linesto be eligible for this offer.

Easy rate is available only on Business Access lines (1MB) only. Easy Rate is not available on ForeignExchange Service, Remote Call Forwarding Service, WATS/800 Service, PBX Trunks, DID, Centrex, ISDNService or Semi-Public Coin Service.

Easy Rate includes optional Custom Calling Features and Hunting listed below and free, unlimited localcalling.

If the Easy Rate account falls below a total of 40 eligible lines per account for a given month, rates for theremaining access lines, features and hunting will revert to their applicable month to month tariff rates.

2.12 PACKAGE SERVICES

2.12.16 SELECT FEATURE PACKAGE OFFER

A. Description

The Select Feature Package Offer gives customers who agree to subscriber new to the SelectFeature Package (or any larger package marketed under another name , that has the samecomponents as the Select Feature package) or who elect to keep the Select Feature Package a $4.00bill credit each month for 12 months.

B. Regulations

The Select Feature Package Offer is available to existing customers who call to disconnect one ormore access lines.

This offer can be used in addition to the single line retention offer ($6.00 for 12 months) but can not becombined with the multi-line retention offer ($12 for 12 months).

Only one credit application is allowed for the life of the offer for qualifying access lines.

The customer must keep the Select Feature package (or other eligible feature package) to receive thebill credit.

The components of this package are provided in accordance with the regulations in the AT&T NevadaTariff PUCN for exchange access services.

2.12.17 NON-RECURRING CHARGE WAIVER PROMOTION FOR WIN/WINBACK

A. DESCRIPTION AND AVAILABILITY

The Non-Recurring Charge Waiver Promotion for Win/Winback waives the charges for theinstallation of the access line, comprised of the Service Ordering Charge and Central OfficeConnection Line Charge when the customer subscribes to one or more Custom Calling Features orAdvanced Custom Calling Features on the access line. Eligible customers are those residentialcustomers who currently have local service with a competitive local exchange carrier operating inthe AT&T Nevada service area and now establish local service with the Company.

This promotion will be available from October 3, 2007 through December 31, 2007.
