the strongest determinants. The patients in the United King-dom had significantly lower SF-36 physical functioning scoresafter adjusting for these significant covariates, and there was asignificant difference between the centers within each country.Female patients were not significantly worse than male pa-tients with respect to SF-36 physical functioning (p = 0.17).
Preoperative Predictors of Postoperative WOMAC Pain and Functional Status The final models for the one and two-year outcomes that con-trol for age and gender in all instances are presented in the Ap-pendix. The preoperative WOMAC pain and SF-36 mentalhealth scores were the strongest determinants of the postoper-ative WOMAC pain score at one year and two years. Althoughno significant difference among the countries was found withrespect to pain at one year, the patients in the United King-dom were significantly worse than the other patients at twoyears (Fig. 1). With a large cohort of patients, we can demon-strate a significant difference, but it is important to note whenthe differences in the WOMAC scores are clinically important.Differences in the WOMAC scores of 9 to 12 points on a 100-point scale have been shown to be perceptible to patients andare clinically meaningful19,20.
The preoperative WOMAC function score was noticeablythe strongest determinant of the WOMAC function score at oneyear and two years after the operation. Despite the marked dif-ferences between male and female patients at the preoperativeassessment, no significant difference in the WOMAC functionscores was found between men and women at one year and twoyears. After adjusting for the other significant predictors, wefound that the patients in the United Kingdom were signifi-cantly worse than those in the United States and Australia in
terms of the WOMAC function score at one year; this differencewas also evident at two years (Fig. 1).
To demonstrate the strong influence of the preoperativeWOMAC function score on postoperative outcomes at oneyear and two years, we analyzed the total patient group ac-cording to the preoperative WOMAC function score quartiles(<34, 34 to 45, 46 to 57, and >57). The mean WOMAC func-tion scores for each group at the preoperative, one-year, andtwo-year assessments are shown in Figure 2. At one year andtwo years, the patients with a preoperative WOMAC functionscore in the lowest quartile (<34) had mean scores of 62.1 and59.8, respectively, which indicates that these patients had, onthe average, considerable functional disability after total kneearthroplasty. The patients with scores in the lowest quartilegroup had the greatest improvement in their scores comparedwith the other groups. We found that patients with a preoper-ative WOMAC function score in the lowest quartile were overfour times (odds ratio, 4.12; 95% confidence interval, 2.86,6.25) more likely to have a score of ≤60 at two years followingsurgery than were patients who had a preoperative WOMACfunction score of >35.
