References for Table A1 Methodology: Calculating outcome : Utilities associated with each outcome allow us to calculate quality of life. Quality-adjusted life years (QALYs) represent a cumulative measure of both quality and quantity of life. For example, if the subject has 5 years with a utility of 0.4, the product is 2 QALYs. If the next 5 years are associated with a utility of 0.3, the product is 1.5 QALYs, resulting in a total of 3.5 QALYs for the entire 10 year period. Our calculations ignore transient perioperative complications, whose effects over the 10 year period are negligible. Successful seizure control and perioperative complications are assumed to begin immediately after surgery. Late mortality and morbidity, as well as reoperations are assumed to occur midway through the follow-up period. If there are no perioperative complications and the subject has an Engel score of 1 postoperatively (the ideal surgical result), her utility is 0.754 (see Table 2). If there are no late complications and no need for further epilepsy surgery over 10 years, the subject will have a total of 7.54 QALYs. Any adverse events lower this number accordingly.
