    Introduction

    The Government of Madhya Pradesh has embarked on a unique project that

    seeks to redefine public service Lok Sewa Guarantee (LSG) . LSG is for the

    people and the Government vis a vis prompt delivery of services sought by public.

    Warehouse s application for online registration to set up warehouse under

    M.P. Warehousing and Logistics Policy 2012 is a web based solution to meet

    the requirement sought under LSG. Objective of the application is to make the

    process of applicants registration and selection on first come first serve basis

    transparently as well as promptly. This application is a computer based

    monitoring system which involves the processes of applicants registration,

    filing of application, deposit of payment , automated waiting list preparation and

    also online communication with the applicant for inspection, compliances and

    frequent intimations on e-mail. Applicant as well official can view application at

    all the stages and also communicate on e-mail.

    Key Objectives

    To provide facility for applying online for registration to setup warehouse under M.P.

    Warehousing and Logistics Policy -2012 and to avail benefits under the policy based on

    first come first serve basis,

    Deposit requisite application fees and obtain date time stamping.

    To Setup a mechanism in applications are automatically reached to concerned district and

    officer to save valuable time of applicant and officer in ascertaining and explaining the status

    of application.

    To provide inbuilt monitoring system for adhering to time limit for completing the process and

    also updating list of waiting.

    Key Benefits

    Facility to apply online from anywhere to be eligible under first come first serve basis.

    Applicant gets the information about receipt and disposal through e-mail.

    Payment of fees at any SBI branch and also online payment facility

    Very exhaustive and integrated computer based monitoring system (CBMS). All applications

    are automatically reached to the concerned district and office. Action taken can be viewed at

    any stage.

    Advance e mail alerts for over due time limit cases.

    E-mails to higher authority for over due time limit cases

    Accessing the Application

    You can easily access the application through web by simply typing the

    http://www.mpwlp2012.com in the address bar of your web browser. Whenever you open the above link in your web browser, you will have the below screen on

    your web browser.

    The above screen is the homepage of the web application. At the top row, you

    will find the current status of subsidy such as: -

    In the middle left of the homepage, you will find the different buttons as below: -

  • Logging into the Application

    **For logging into the application, applicant must be registered.

    Registration

    After clicking the registration link, you will be directed to the below screen. All

    the fields marked with * asterisk sign are mandatory. After submitting the

    correct details, an email will be forwarded to your mentioned Email id regarding

    the confirmation of successful registration.

    You must keep some key points in mind while registering onto the application

    against the following fields. 4

  • 1. Email id (username) Email Id must be active/live as the email regarding

    the confirmation of registration and further information will be send to it.

    2. Check Availability Link It checks whether the username exists or its

    available.

    3. Create Password Password must of at least 8 characters and must contain

    at least one special character like (@,%,$,#,~).

    4. Re-enter Password It must be the same as provided in the above textbox.

    5. Name of the investor/entity Application will be filed in this name/entity

    only. It must be correct because it cant be changed once saved.

    6. Type of Entity Here you will select the type of entity from the drop-down

    box.

    7. Mobile Number Please provide the correct mobile number to ensure

    further communication.

    8. Enter Image Value (Case Sensitive) Please enter the correct value as

    shown in Image.

    9. Submit Button It will submit all your filled details and a confirmation email

    will be sent to the email address provided by you.

    10. Cancel Button It will cancel all the details provided by you and it will

    redirect you to the home page.

    Every time you try to submit application with the incomplete information, you

    will be prompt for giving the complete information. So, make sure to submit the

    form after providing the complete and correct information only.

    Whenever you click the login link, you will be redirected to the below screen.

    You need to fill the following details to login:-

    1. User name Enter the email id which you provided at the time of

    registration.

    2. Password Enter the current password.

    Both the fields are mandatory to login.

    3. Enter Image Value Enter the value as displayed in the above image.

    4. Login Button This button is used to login with the filled details.

    5. Cancel Button Whenever user clicks this button, all the details will be

    discarded and user will be redirected to the home page.

    6. Forgot Password Link If a user forgets his/her password, he/she can use

    this link to reset password.

  • Using Application

    **There are two types of users who will use the application.

    Applicant

    Authorized officials

    **Whenever users login with their respective credentials, they will be login

    into their respective pages.

    Applicant

    Whenever applicant login with his credentials, he will find the below welcome

    screen on his browser. In the welcome screen, applicant will find the tabs

    containing:-

    Application Status

    Application Form

    Change Password

  • Application Status

    Applicant can view the current status of all the applications request send

    by him/her. After clicking the button applicant will have the report containing

    the details of all the applications filed by him/her only.

  • Application form

    Whenever applicant wants to file a new application, he/she can fill the form by

    simply clicking the tab Application Form. When he/she clicks the link, he will be

    introduced with the Terms and Conditions Agreement Window

    There he/she will find the two check boxes mentioning: -

    1. I agree If he/she agree with the conditions, he/she has to tick the

    option and click the ok button. Whenever he/she will submit he/she will

    get the application form to fill.

    2. I do not agree If he/she does not satisfied by the terms and conditions,

    he/she can tick the option and go back to the welcome screen.

  • Applicant will be redirected to the below form after the acceptance of the

    terms and conditions.

    **Every field marked with * sign is mandatory to be filled.

    **Instructions marked in Red must be followed for smooth processing.

    Part A

    1. Applicant Details

    In the Part A of the form, you will find Applicant Details as shown in the above

    image. Name of Investor/entity and Entity field is taken from the details

    mentioned in the registration form. Both the fields cant be edited too. PAN

    number is mandatory here as it is marked with * asterisk sign. Please make sure

    to fill the correct PAN number otherwise you will get the message box asking for

    the correction.

  • In the next section of Applicants Details, you will find the Address Details as

    shown in the above picture, here you will have to fill at least one address out of

    the three addresses such as Registered office, Corporate Office, Local Office i.e.,

    one address is mandatory. Here you will have to provide the details such as

    address, Telephone number, Fax Number and active Email Address.

    ** You must have to tick the at least one check box to mention the address

    that will be used for correspondence as it will be used for further

    communication.

    In the next section, you will find the Details of the

    PROPRIETOR/PARTNERS/PROMOTERS/DIRECTORS of the company. Here you

    have to fill the details of PROPRIETOR. You can also add more than one

    PARTNERS/PROMOTERS/DIRECTORS of the company.

    In the next section, you will have to fill the contact details which will be used for

    further communication.

    2. Project Details

    Here you will have to enter the land details. First you must select the district.

    On the basis of that, list of tehsil under that district got filled. You must select

    the tehsil for further process.

  • You must enter the proposed capacity (in MT). If you dont know the capacity,

    you can calculate it by using the capacity calculator in which you will have to

    provide the length, breadth and height.

    3. Project Cost & Means of Finance

    Further, you will have to fill the information related to the project you are

    proposing such as proposed investment and means of finance.

  • 4. Applicants Bank Details

    You need to fill yours bank details also in which you have to give information

    such as Bank Name, A/C Number, IFSC Code.

    PART B

    Application Fees will be automatically calculated. After filling the complete

    details you will found the four buttons save, cancel, submit and print.

    Save - You can save the application

    Cancel - By clicking the button, all the values will be refreshed and you

    will be redirected to the home page.

    Print - You can take printout of all the details you have filled.

    Submit - Your application will be submitted by clicking this button.

  • On successful submission of your form, you will find two more buttons as: -

    Online Payment only for SBI A/C Holders If you are a SBI A/C Holder and if

    you want to submit your fees, you can submit your fees by clicking this

    button. You will be redirected to SBIs website, if you click this button. There

    you can make payments for further processing.

    Generate Payment Challan If you are not a SBI A/C Holder and want to

    submit your fees by visiting the bank, you can take the printouts of the

    challan and can submit to any SBI Branch.

    ** User must agree to the terms and conditions before submitting the

    form.

    If a user wants to change password, he/she can change the password by simply clicking the

    link.

  • Authorized Officials

    We have provided the different homepage to the authorized officials i.e.,

    district officials. When authorized officials will login with their credentials, they

    will be redirected to their home page where they will find four tabs containing: -

    Here they can see the total number of applications filed under their district

    within the selected date range. They can also forward the application for

    further processing.

    Here they can see the application status directly by selecting any particular

    status from the drop-down list and clicking the show button.

    They can also view the status of any specific application by searching the

    application number as below:-

    District officials can upload data received from the Bank such as: -

    Online Payment or

    Challan Payment

    They can change their own password

    And reset password of any of the user as well.

    **A mail will be forwarded to the respective user once the password gets reset. 19

    If an official wants to save the data of applications filed in specified date range,

    he/she can download the data by visiting this link.
