MESSAGE FROM THE PRESIDENT

This past year, it was again an honour to serve as President of Community Living Chatham–Kent. It was a pleasure to work with a very dedicated and diverse Volunteer Board of Directors. I extend thanks to the employees, volunteers, committee members, and community partners for their ongoing commitment to this exceptional agency. Organizational Improvements, Social Enterprise, Family Engagement, and Building Social Capital were the focal points of the agency. The Ministry of Community and Social Services conducted the annual Quality Assurance Measures (QAM) Compliance review in June /2015. The process included an assessment of Policies and Procedures, employee and volunteer records, documentation and records for people who received support, and visits to six locations. Community Living Chatham-Kent was commended for an outstanding review. The agency continued to work toward achieving compliance with the French Language Service Act and increase capacity to meet the needs of persons who speak French. Enhancements included English and French signage, key documents and forms translated to French, a menu available in French at the Parkfield Restaurant, and updates to the website. The utilization of Passport Funding to purchase supports from Community Living Chatham-Kent expanded over the last year. In an individualized and flexible manner, people developed new interests, acquired skills, and participated in numerous social, recreational, and educational activities within their communities. In conclusion, last year was very successful, and moving forward, the people who receive support will continue to be the top priority. Respectfully submitted, Stephen J. A key theme of the 2015-2016 year was change. That being the case the people who accessed services and received supports were of most importance. They were supported on various trips, in competitive employment, and to attend many events. I offer my sincerest thanks to the Board of Directors for its wisdom and strong leadership, to the staff, particularly Jodi Skipper, Linda Karnas, Brad Verfaillie, and Lisa Blonde, for their unwavering commitment to the people, and to the people and families for placing their trust in this organization. With respect to the 2015-2019 Strategic Plan, significant progress was made. Emphasis was on the areas of Organizational Improvements, Social Enterprise, Family Engagement, and Building Social Capital. The Diamond Dreams Gala celebrating Community Living Chatham-Kent’s 60th Anniversary was in May. There were over two hundred attendees including MPP Rick Nicholls, Wayne Hasson from MP Dave Van Kesteren’s Office, Councillor Brock McGregor, Mary Harper, Program Supervisor, MCSS, Chris Beesley, CEO of Community Living Ontario, and David Barber, OASIS President. Terri Bell was the recipient of the Joan Olive Risdon Memorial Award of Distinction, and Union Gas received the Joyce M. Carr Champion Award. The System Capacity Investment received from the Ministry of Community and Social Services was utilized to both meet pay equity obligations, and increase staff wages. In November an announcement was made that sheltered workshops would be closing. CLC-K recognized people as being the top priority, acknowledged the importance of the Day Program to the people who attend it, as well as their families, and committed to supports not being reduced. A Family Committee was established to provide insights, feedback, and ideas relevant to the transition of the Day Program. A spectacular ten bedroom home, totally accessible and meeting all fire regulations, was built on Llydican Extension. The house on Calais Court was sold and money reinvested into the new home. The many partnerships within the municipality, and province were vital to the organization. Ongoing collaboration occurred with MCSS, Community Living Ontario, Municipality of Chatham-Kent, St. Clair College, St. Clair Catholic District School Board, Lambton Kent District School Board, and businesses. The Community Relations department hosted a very successful 18th Annual Golf Tournament, Walkathon, and Community Christmas Concert. The 2016 Lottery Calendar was outstanding and a record profit was generated. Community Living Chatham-Kent welcomed Sandy Morrow, Quality Assurance & Planning Manager, Angela Corso, Manager of Community Relations, and Maurice Velle, Facilities Manager. The 2015-2016 year was extremely successful, and I envision a bright future.

In the fiscal year of 2015-2016 we sold a four bedroom home which needed significant renovations to meet fire regulations and the needs of the people living there, and rented a more accessible home in its place. As well, eight people living in a rented two-storey duplex, which needed major renovations to meet fire regulations, moved into a newly constructed, ten-bedroom duplex. The net proceeds on the sale of the four-bedroom home were applied to the construction costs, and a mortgage secured for the balance. Community Living Chatham-Kent was the recipient of additional funding during the 2015-16 year as noted below:

• $18,614 capital funds to replace fences at two residential locations • $41,667 funds to temporarily support two women needing assistance • $23,334 fiscally, and $76,000 annually to support a person in group living

The Fund Raising/Community Relations Program generated $182,250 through fund raising, donations, and appeals. The money raised by this department assists in covering the agency’s costs and securing agency viability. In the 2015-16 year $16,133 received through grants, was allocated to fund activities in the summer employment programs, a new deck for one of our 24 hour homes, state of the art audio visual equipment to be used for staff training at the Sass Road location, and children and adult respite programs. 6:31 p.m. Stephen then thanked Lois Bachynski and Diann Van Basselaere for volunteering their time to register the membership.

2.0 Congratulatory Messages Stephen introduced Keith Dee from Community Living Ontario, who attended on behalf of Chris

Beesley. Keith thanked Community Living Chatham-Kent for the invitation and opportunity to speak at the 60th AGM. Keith spoke about Community Living Ontario as an organization, their history and vision for the future. He congratulated Community Living Chatham-Kent and presented a certificate in honour of the 60th Anniversary.

Stephen Andari thanked Keith for his kind remarks and Community Living Ontario for continuing

with the excellent work they are doing.

Stephen Andari then introduced Rick Nicholls, MPP Chatham-Kent Essex. Rick Nicholls stepped

forward and spoke about the importance and value of CLC-K in the community. Rick shared that he had a wonderful time participating in this year’s CLC-K Annual Golf Tournament. Rick spoke highly about the individuals who have visited Queen’s Park and his office. Rick encouraged individuals to visit the Parkfield Restaurant and praised them for their hard work and warm welcoming atmosphere. Rick congratulated CLC-K on their 60th Anniversary and expressed his appreciation to all staff and volunteers for their meaningful worthwhile work in the community.

Stephen Andari thanked Rick Nicholls for his kind words and introduced Mary Harper, Program

Supervisor for the Ministry of Community and Social Services. Mary Harper was pleased to bring greetings on behalf of MCSS. Mary congratulated CLC-K on

another year of providing great service to the community. Mary commended CLC-K on the wonderful support provided to the people and families supported and expressed her gratitude of the partnership she has with the organization. She spoke of three memorable events she was privileged to attend including the Minister’s Visit, 60th Anniversary Gala and Lu-Ann’s retirement. Mary thanked the Board, Management and front line staff and all those who ensure people live in an inclusive community. In closing, Mary encouraged CLC-K to always share thoughts with her on how the transformation process is moving along, and congratulated CLC-K on 60 years.

Stephen Andari thanked Mary for attending and introduced Cynthia Swift from KPMG who will

come forward later in the meeting to present the Audited Financial Statements. Stephen then introduced Ron Coristine, Executive Director. Ron Coristine welcomed everyone and thanked the special guests for being present at the 60th

Annual General Meeting. 3.0 Introduction of Leaders Within Speaker Ron introduced Julie Tanton, who is a graduate of the Leaders Within Program. Julie Tanton came forward and gave an inspirational presentation about her life experiences and

successes. Julie spoke passionately about her family, interests, volunteer work and how Community Living Chatham-Kent has been there for both her and her husband. CLC-K has assisted her husband with finding a job and Julie’s worker has always been there for her to help with daily tasks and has also accompanied Julie on day and overnight trips. In closing, Julie expressed that thru her volunteering and with the help from Community Living she has been able to boost her confidence and self-esteem.

Ron Coristine, on behalf of Community Living Chatham-Kent thanked Julie for sharing her story and wished her and her husband a Happy 14th Anniversary.

4.0 Review of Annual Report Ron Coristine remained standing to review the Annual Report for 2014-2015. Ron reviewed the annual report, including the many successes experienced over the past year

including the opportunity to host the Minister’s visit and Plaque Presentation. During her visit, Dr. Helena Jaczek toured the Stirling Lane location, Park Ave Day Program and recited the words on the commemorative plaque that is now sitting at the location of the former Southwest Regional Centre. Also, this past January, implementation of the new 2015-2019 Strategic Plan began. The areas of focus are technology, social capital, social enterprise and family engagement. As an agency we acknowledge the importance of providing our services in French language. Ron reported that we have developed and will continue to develop in that area. Ron then thanked the staff of CLC-K, including the Senior Management Team, his Executive Assistant and the Board of Directors for all of their support over the past year and their commitment to the people supported. In closing, Ron acknowledged Lu-Ann Cowell and thanked her for all her years of commitment to the agency and wished her all the best in her retirement.

5.0 Acceptance of Minutes of 2014 AGM Stephen Andari asked if there were any errors or omissions in the minutes of the June 19, 2014, 59th

Annual General Meeting. There being none, he asked for a motion to approve the minutes as presented.

Linda Goldsmith (Jack Barei) moved that the minutes of the June 19, 2014 Annual General

Meeting of Community Living Chatham-Kent be approved as presented. Carried Cheryl Prince (Derek Jane) moved that the 2014-2015 Annual Report be approved as

presented. Carried Gord Crompton (Jack Barei) moved the actions of the Board of Directors for the fiscal year

2014-2015 be approved as enacted. Carried 6.0 Presentation of Audited Financial Statements Stephen Andari welcomed Cynthia Swift of KPMG and asked her to come forward to present the

Audited Financial Statements for the fiscal year 2014-2015.

Cynthia outlined that the statements were audited by KPMG and that there was sufficient

information to present the financial position fairly at March 31, 2015. The financial statements are in compliance with Canadian accounting standards for not-for-profit organizations. In the opinion of KPMG the financial statements present fairly, in all material respects the financial position of Community Living Chatham-Kent as at March 31, 2015.

Erin Ryan (Dorothy Cook) moved that the Audited Financial Statements for fiscal year 2014-

2015 be approved as presented. Carried Erin Ryan (Dorothy Cook) moved that the firm of KPMG be reappointed as Auditors for

Community Living Chatham-Kent for fiscal year 2015-2016. Carried 7.0 Special Presentations Ron Coristine announced that both Erin Ryan and Darlene Clarke would be retiring from the Board

of Directors. Darlene unfortunately could not be present at the AGM; however Ron spoke about both Darlene and Erin’s successes and commitment to Community Living Chatham-Kent and wished them all the best in their future endeavours. Ron asked Erin to come forward and presented her with a gift for her many years of service on the Board.

8.0 Introduction of Candidates and Election of Board Members

In Robert Birnie’s absence, Stephen Andari came forward to announce the recommendations of the Nominating Committee. Stephen announced that there was a full slate of officers to be presented to the membership this evening.

There are six people who still have a one year term remaining – Stephen Andari, Jack Barei, Dorothy Cook, Linda Goldsmith, Derek Jane and Cheryl Prince. There is one Board Member eligible for re-election, Derek Robertson. Derek Robertson has agreed to let his name stand for re-election. We also have three new proposed nominees for election to a two year term: Terri Bell, Robert Birnie and Zoli Horvath. This represents a full slate.

Stephen Andari then called three times for nominations from the floor. Hearing none Stephen then

asked for a motion to close the nominations. Erin Ryan (Jack Barei) moved that the nominations be closed and accepted as presented. Carried Cheryl Prince (Erin Ryan) moved that the recommendations of the Nominating Committee

be accepted. Carried

9.0 Installation of Board Members Stephen Andari asked the newly nominated members to come forward. Terri Bell completed the installation ceremony and was sworn in by Stephen Andari. *Not in attendance: Derek Robertson, Robert Birnie and Zoli Horvath who will all be sworn in at

the next Board meeting on September 15, 2015.* 10.0 Introduction of Special Guest Speaker Ron spoke about social capital and how it would fit into the new strategic plan. He mentioned that

Bill Labadie had approached him about doing a bowl-a-thon as a fundraiser for Community Living. In April, Bill, amongst many of his friends from the community who came out to help Bill, raised over $2200 which he then donated to CLC-K.

Ron Coristine then introduced Jeff Young, who is a dear friend of Bill Labadie, to share his

presentation on Social Capital. Bill Labadie joined Jeff for his presentation. Jeff came forward and spoke of his experiences of being a teacher and friend to his students and to

Bill. He shared his story, how he and Bill first met and how they worked very well together over the years and became great friends. Jeff praised Bill for all he had done over the years to help with school events, BBQ’s etc. and how Bill interacted so well with young children always encouraging them and making them feel included. Jeff encouraged everyone to take some time and spend it with a person who has a disability, take the person out into the community, demonstrate social capital and build friendships.

Jeff then introduced Bill Labadie and highlighted many of his accomplishments. Bill was very

appreciative of all the support that he receives through Community Living Chatham-Kent. Ron Coristine thanked Jeff and Bill for their presentation.

Stephen Andari expressed his deep appreciation to Bill and Jeff for reminding us about what we are doing and why we are doing it.

Stephen asked that the Board Members stay for a brief Organizational Meeting and guests were

invited to stay for refreshments. 11.0 Adjournment On a motion by Erin Ryan the meeting was adjourned at 7:37 p.m.
