MESSAGE FROM THE CEO I’m pleased to present Hospice of Central PA’s Annual Report for 2017. This report gives us the opportunity to highlight those milestones that our hard-working team has reached. Our most significant work is always the day to day provision of the highest quality of emotional, physical and spiritual support with compassion and respect. The team’s other meaningful achievements are: • HCP reached Level Four status with the We Honor Veterans program! This is a program developed in partnership with the National Hospice and Palliative Care Organization (NHPCO) and the Department of Veteran’s Affairs. Level Four is the highest level designation and I want to recognize and congratulate the HCP Veterans’ Committee. • In 2017 our long-standing relationship with AndrewsPatel Hematology/Oncology was further enhanced to allow our staffs to better serve patients and their families through compliantly sharing information via electronic medical records and meetings. Additionally, Dr. Wenner and Dr. Olender began providing palliative consultative services to patients in both AndrewsPatel office locations. • Staying connected with our community supporters is so important. To that end, we began holding board alumni luncheons this year. Each fall and spring, current and past board members are invited to the office for lunch and an update of what is new and happening at Hospice of Central PA. I’m pleased to present Hospice of Central PA’s Annual Report for 2017. This report gives us the opportunity to highlight those milestones that our hard-working team has reached. Our most significant work is always the day to day provision of the highest quality of emotional, physical and spiritual support with compassion and respect. The team’s other meaningful achievements are:

• HCP reached Level Four status with the We Honor Veterans program! As chair of Hospice of Central PA’s Board of Directors, I am proud to be sharing

the 2017 Annual Report with you. HCP’s loyal staff and volunteers serve

tirelessly to build a community that embraces specialized care at the end of life.

I am very grateful to be joined by a group of board members who are all truly

devoted to our mission. This report is more than an annual summary of giving.

Included in these pages are descriptions of programs that have assisted many

families through difficult times.

The changing environment in healthcare is a daily reminder of the challenges

facing HCP. It is more important than ever for hospice providers to

demonstrate quality care and to add value to our referring partners. We think

our varied programs do that and you can refer to recent accomplishments with

our partners throughout this report.

We are also very proud of HCP’s commitment to mission fulfillment. Please

note the many generous members of the community who made this good work

BOARD OF DIRECTORS

Our mission is clear; to provide the highest quality of emotional, physical

and spiritual support with compassion and respect. Quality is always on our

minds. In keeping with regulations and best practices, HCP measures the

quality of our programs through third-party administered caregiver surveys.

The agency we have engaged for this purpose is called Deyta Analytics

HEALTHCAREfirst (HCF).

Given that two symptoms—pain and dyspnea (shortness of breath)—are

the most common for patients at the end of life, HCP closely monitors

our effectiveness at screening and treating these symptoms in our patient

Memorial and Tribute Gifts

Memorial and tribute gifts offer a way to remember or

honor a loved one, while also supporting Hospice of

Central PA. Families are notified that you have made a gift

when you provide a name and address for this purpose.

Restricted GiftsHCP is able to honor donor-restricted gifts

for the following programs:

Carolyn’s HouseHCP welcomes gifts restricted to Carolyn’s House which provides

24-hour hospice care in a comfortable home-like setting in

Harrisburg. HCP offers this residential hospice service when

personal choices or medical needs make other options difficult.

Camp DragonflyCamp Dragonfly is a free weekend camp for children who are

grieving. It provides a safe and fun environment in the great

outdoors to work on coping skills and healing activities with peers.3rd Party EventsA 3rd party event is when a business, organization or family

plans a special event to benefit HCP. Hospice of Central PA

is the fortunate recipient of the net proceeds of many 3rd

party events throughout the year, such as Angel’s Cookie

Sales in memory of Joanne “Angel” Palmer. For guidelines and

suggestions regarding effective 3rd party events, please reach

out to the HCP development department.

Special EventsA fun way to get more involved with HCP and fundraising

is through special events. Every year HCP offers many

opportunities to support end-of-life and bereavement care.

Thanks to the deliberate acts of

generosity from you and others, HCP

patients and families receive the

highest quality of care. Your gifts

make this possible and every donation,

regardless of size, brings comfort.

At the age of 81, Sherlyn “Sherry” Robinson decided to make a life-

changing gift to HCP. After witnessing HCP’s care for a friend and

neighbor, and learning more about Carolyn’s House, Sherry decided to

include a bequest in her will. She called Chief Executive Officer, Gil Brown,

to notify him of the gift in September 2017 and gave HCP permission to

share her story below.

Sherry spent her early years in Lancaster and has lived ever since in

Harrisburg. She worked diligently throughout her life, including more

than 30 years for the state of Pennsylvania. When asked why she made

plans for such a powerful gift, she said, “I connect Hospice of Central

PA with dignity.” Sherry’s mother took in and cared for Veteran patients

discharged from a local hospital. She worked hard to maintain their dignity

and this, in part, inspired Sherry.

Like most of us, Sherry hopes to pass away quickly (like her mom) with

minimal end-of-life care. But, she feels comfort knowing HCP is there if

needed. “I know there is a great call for Carolyn’s House. I would like to

do what I can for people who, for whatever reason, need a safe place to

transition.”

Sherry talks about her friend, Betty, who chose HCP for her hospice care.

She told Sherry that “I don’t feel like a burden. I feel like my life matters.”

In addition to a close network of supportive friends, Sherry values her

family and a friendly black cat named Sheba. She encouraged her two

“adopted” sons to make their way in life when she helped them to

purchase and manage their own business. She also treasures a beloved

brother, Rob, who lives in Montana.

Sherry has said more than once, “I worked hard and want to feel that my

money will go to HCP to do good.”

The energetic team behind Angel’s Cookie Sales in memory of Joanne “Angel” Palmer

The Legacy Society HCP’s Legacy Society was established to recognize

and honor all who name Hospice of Central PA as a

beneficiary of a will, trust, retirement plan, life insurance

policy, life income gift, or other type of planned giving

arrangement. Everyone is welcome to join the Society

regardless of the size of the bequest or planned gift –

simply notify HCP of your intention.

The Journey Program

Recent Accomplishments

HCP’s new mobile-friendly website went live in December. The site features a page dedicated to our

quality scores and links to many affiliations in the community. The website also provides answers to

frequently asked questions regarding hospice care, palliative care and Carolyn’s House. Please visit

HCP has a new video library available on our website. Hear directly from HCP volunteers about why they

volunteer. Listen to families share their HCP story and talk about how they are coping with help from The

Journey Program. The video library gives caregivers the opportunity to “meet” HCP providers before they

step into their home.

Carolyn’s House provides 24-hour hospice care to individuals who are unable to remain at the place

they call home, and offers residential hospice services when personal choices or medical needs make

other care options difficult. Thanks to the Dauphin County Gaming Advisory Board and Dauphin County

Board of Commissioners, significant improvements to the residence have been made to ensure the

residence’s accessibility, safety and comfort. A portion of these improvements was made possible by a

$47,500 gift from the Dauphin County Local Share Municipal Grant.

HCP was honored to receive funding from the Partnership

for Better Health to pilot a new community program called

Inspiring Community Conversations, on April 4 at UPMC

Pinnacle Carlisle. The program was designed to inspire

conversations about advance care planning. Watch for a
