REACH Launches Patient Engagement for Care Teams Portal
REACH, the Regional Extension and Assistance Center for Health Information Technology (HIT)in Minnesota and North Dakota, has launched a web portal to help care teams effectivelyengage patients. This portal has resources, tools, templates, videos, blogs, and links to bestpractices in patient engagement. It is available to all providers, not only REACH clinics andhospitals.
“Engaged patients are essential for quality outcomes. When patients are not engaged, they areless likely to take responsibility for their own health. And, they are less likely to express theiropinions to providers or to share their own priorities. If a patient’s plan of care does not meettheir own priorities, it is likely to fail. An engaged patient is a team member in their own careand consequently an active participant in improving their health. When they are an engaged,they understand the plan and are much more likely to execute the agreed upon care plan. Thistoolkit will help providers understand how they can engage patients in their own care which willlead to better outcomes.” - Paul Kleeberg, MD, Chief Medical Informatics Officer, REACH andStratis Health.
The importance of engaging patients is further underscored by the “Engage Patients andFamilies” core objective of Meaningful Use Stage 1 to provide clinical summaries for patients foreach office visit. Patient engagement is the responsibility of those who interact with the patientand can happen at every step of patient interaction. The toolkit includes tools, workflows,methods and other resources, associated with various points in the patient encounter includingcheck-out and post-visit.
The Patient Engagement for Care Teams Portal can be found at: http://www.khareach.org/patient-
engagement-portal. It was designed by REACH staff, clients, volunteers and consultants. It wasdesigned by REACH staff, clients, volunteers and consultants. MN Community Measurement andthe Robert Wood Johnson AF4Q initiative provided financial support to the University ofMinnesota School of Nursing for undertaking a literature review which resulted in an annotatedbibliography.

Volume 5, Issue 4 NDHIN Update
NDHIN conducted user acceptance testing of the stage environment on Sept. 5 and the production
environment of the HIE solution/Clinical Portal has now been fully installed and tested. This marks
completion of the second major deliverable from Orion and was completed ahead of schedule. Direct
continues to be successfully used and planning is underway for Direct Secure Messaging v2.0, PDMP
Integration, Imaging Solution, Advance Directive/Patient Portal, and Eligibility Data.
North Dakota has been accepted into NATE (the National Association of Trusted Exchanges) as the seventh
member. This provides ND with an opportunity to be involved in the development of a scalable, sustainable
Trust and Policy framework that supports and advances intra-state and interstate HIE. Information

NDHIN has been accepted as a member of Direct Trust. Direct Trust is a group collaborating to build the
security and trust framework for directed exchange. NDHIN also has been notified they are approved to join
Healtheway (formerly NwHIN). The next steps for testing are being planned with Orion Health.

Successes
There are now 100 participation agreements signed and 463 authorized users
enrolled in NDHIN Direct. If you wish to use Direct with another organization
(hospital, ambulatory care, long term care, pharmacy, public health, etc.) that
has not yet joined NDHIN, please encourage them to do so. The NDHIN

Outreach & Training
There will be NDHIN Provider Education Packets along with a link to a video
that will be mailed out to all facilities within the next two weeks. This packet is
provided as an educational tool to assist providers and those that will have direct
contact with their patients or consumers. The information includes an overview
of what the NDHIN is, benefits, and other key information. We will be providing
educational webinars for anyone who may have questions. Please watch our

North Dakota Department of Health
The North Dakota Department of Health Division of Disease Control
has signed up for Direct and is now accepting reportable conditions
and syndromic surveillance messages through this method. Below
are the two direct addresses for reporting to the Division of Disease
Control.
Reportable Conditions ([email protected])
The Disease Reporting direct email address can be used as a
fax replacement to send reportable conditions to the Division
of Disease Control. Reportable conditions can be found on
the Division of Disease Controls website at http://
www.ndhealth.gov/Disease/.
Syndromic ([email protected])
The Syndromic Surveillance direct email address can be used
for sending batched HL7 messages. If your facility is
interested in transmitting syndromic surveillance messages
through this system, please visit http://www.ndhealth.gov/
disease/mu for specifications regarding syndromic
surveillance message requirements. The Division of Disease
Control will be accepting syndromic surveillance messages
for both eligible hospitals and eligible providers.
If your facility is interested in transmitting Electronic Lab Reporting
messages through this system, please visit http://www.ndhealth.gov/
disease/mu for specifications regarding ELR message requirements.
Direct could also be utilized for sending a batched HL7 message for
low incident facilities.
ND Quits
National Jewish Health, contractor for ND Quits, is now able to
accept tobacco cessation referrals via Direct. The referrals can be
scanned and sent as an attachment in place of a fax to the ND Quits
Direct address at [email protected].

Reminders
The NDHIN should be notified within 24 hours to inactivate an Authorized
User’s account when the Authorized User has been removed by a Participant
for reason of termination of employment or otherwise.

The 2013 North Dakota Health Information Advisory Committee Connectivity Grant
Program for HIN is designed to:
1. Assist health care providers who are potentially eligible for Medicare and
Medicaid incentives for meaningful use of an EHR in exchanging health
information with other health care providers,
2. Increase the number of North Dakota pharmacies capable of accepting
electronic prescriptions and
3. Facilitate the transition of care for North Dakota patients.

Dept. of Health, Public Health Reporting connection – connection complete, analyzing test syndromic messages, preparing for ELR to be sent from first participants.
