Dear Friends and Well-wishers,

It is my pleasure presenting activity and financial report for the year 2014-2015.

The facility of providing primary health care services for the poor and needy people started through Anjali Hospital is now providing secondary health care services. With the increase in number of patients, the number of patients with chronic diseases / ailments has also increased.

The patient/ sick person is the main entity (center of focus) in providing health care. Doctors & para medical workers are primary essentiality to complete the team providing such care. Providing equipment/instruments and creating infrastructure is comparatively easy but we are finding it very difficult to get well qualifies & capable specialists and skilled para medical workers. At the same time it is perceived that the number, type and level of services when we started has also increased and changed. Though Anjali Hospital does not receive government grant, there is an association with government in some schemes like Rashtriya Swathya Bima Yojna (RSBY), National Revised Tuberculosis Control Programm (NRTCP), new born care and maternal care.

Under the banner of “ bachat mandal” , efforts are being made to connect women members with various aspects of their development. Women have started experiencing what are their family needs and those needs can be fulfilled with self-respect through “ Bachat mandal”.

As very rightly spelled out in this very popular verse of Gujarati poem by Shri Umashankar Joshi, the MAN has changed everything out there in the vast earth surrounding him. The atmosphere and climate change is being experienced. There is some solace that we are able to contribute to enrichment of environment through the tree plantation on large scale done every year.

In the current year also it is seen that the expenditure in providing health services is much more than the income. As our aim is to provide health services to the poor & needy, to include these poor families in developmental process through women's program and enrich the environment, the expenditure is bound to be more and such activities cannot be self-sustained.

All this is happening with warmth and assistance provided by many friends, well-wishers and individual & institutional donors. On behalf of the beneficiaries, Anjali and the trustees I extend heartfelt thanks to all.

Anjali was established in the year 1988. The activities of the organisation were thinitiated on 29 January 1989.

The founders after completing higher education, formed Anjali in order to utilise their skills & capabilities for the poor and the needy.

Anjali is a voluntary organisation based in village Ranasan with population of 5000. Ranasan is a village in Talod taluka of Sabarkantha district in north Gujarat in India.

Beginning was made with health activities, the poor and the needy being the target population. The health activities developed progressively and other activities were added as the needs were perceived.

At present the activities of Anjali are, a 90 bed Anjali Hospital, women's programm in 49 interior villages (on the platform of saving groups) and environment enrichment programm. Organisation is inclined toward development programmes for children, youth, and villages.

The soil in Ranasan & in its vicinity is rocky & sandy and the climate is dry, windy, hot during 7-8 months of the year and cold during the winter months. The rainfall is less and unpredictable. There are two rivers which run if there is ample of rainfall. There are few lakes & other sources of water. The principal vocation of the people is agriculture. There are few industries in the vicinity. The level of education is low. Most of the population is economically & socially weak.

The majority of the population of this area comes from the communities of Thakors, Dalits, Ravals, Vagharis, Banjaras, Vadi, Pagi, Rabari samaj and migrant adivasi labourers. Most of them earn their livelihood as agriculture or miscellaneous labour. They have not made much progress either economically or socially. This group of people along with other economically weak & distressed section of the population are the main beneficiaries of Anjali's activities.

(1) Anjali Hospital Anjali Hospital provides services like emergency care, child birth,

medicines, ambulance and qualified medical doctors for the poor and needy of this area 24 hours a day for 365 days.

Some facilities & arrangements started on small scale are becoming relevant & useful to these communities.

Some of the following facilities & arrangements are noteworthy: 1. Smt. Sumitra Desai Asthma Center: A bronchial asthma center was started long back with the assistance of

Jash Kanta Charitable Trust of Mumbai. The center provides 50 to 75 % concessions to such patients in daily requirement of medicines, investigations, hospitalization charges etc., the number of patients are increasing every year.

2. Chronic Disease Center: (CDC) Diseases like Diabetes, Insulin dependent diabetes, High BP,

paralysis, TB, epilepsy require life long medicines to prevent complications. To help such poor & needy patients, significantly subsidised Chronic Disease Center was started. The beginning of this center could be made with the assistance of Jewelex India Private Limited, Mumbai.

3. Other:

a) Migrated tribal laborers, their children and families are offered all the hospital services like OPD, indoor, emergency, laboratory, ambulance & medicines at 90 % concessions.

b) Significant concessions in Ante natal, intranatal and postnatal care are offered to pregnant mothers and infants till 1 year of age.

c) Provision of significant concession to those who do not fall under above categories but are poor & needy.

The poor & needy segment of the population has been able to seek the hospital services in significant numbers as a result of above arrangements. Consciously adding such services will make the poor & needy to seek the hospital services in time without any apprehensions, which is our objective. Such medical facilities & arrangements for the needy segment of the population will prevent poor becoming poorer, and keep children & mothers healthy. Efforts are being made to bring qualitative improvements in above arrangements by introducing costly investigations to improve diagnosis and support of counsellors.

2. Women's Programm:

Another year has passed satisfactorily in bringing women together and organizing them. All have understood the methods of financial transaction and accounts undertaken during the monthly meetings. Even then some members need to be reminded about the regular payments of monthly savings and the installment of the loan taken. Most of the saving groups started with monthly savings of Rs 30 were able to increase the amount to Rs 50 and reach level of Rs 100 per month. New saving groups now start with Rs 100 saving per month. Women are able to understand big benefits of small savings, especially preserving self-respect and increase in their self-confidence.

The theme of yearly monthly meeting held at Anjali was “Community Monitoring” at village level. Role play, storytelling, games, group discussions were the mediums through which the information was conveyed. Sources of Government income, various taxes, how we directly or indirectly pay taxes was explained. The school education, health services, anganwadi, food grains made available through ration shops etc. are provided from the taxes that we pay.

So it is responsibility of all of us to monitor various government programmes. Each group came together to decide which government program that group will monitor. In 2-3 initial months visits were made to the selected program but over a period, sincere visits that should have taken place did not happen.

Activities

They remember the time of yearly tour / outing very well and start making preparations. The demand this year was to visit a distant place so tours of Mt. Abu were organized. They take care of each other and feeling of oneness among them is perceived.

Kishori Programm: An effort is being made by visiting 233 adolescent girls in selected 8 villages

from the program villages, to make “Kishori Pravruti Mandal” (adolescent activity group). The women members are motivating their daughters and other adolescent girls to join these groups. Activities which, these girls like and are useful in life, like explaining changes in body at that age, menstrual cycle & hygiene, need of nutritious food/anemia, self-development & self-defense, making button eyelet, tailoring, various recipes, performing garba, make best things from waste material, play games, tour & excursions etc. are undertaken. School going as well as girls not going to school are guided to do these activities in villages.

Newly married couple meet is a regular yearly event. The list of participants is drawn with assistance from the group members. This meet is held during Diwali time. An effort is made to familiarise them with body physiology, family planning, addiction, nutrition, hygiene, life style, harmony in life especially with each other.

There are eight saving groups in Ranasan village. The nuisance of alcohol in villages is well known everywhere. Many young people have died because of this in this village. A group of friends of these young people, who died, approached us to help them do something. We narrated our experiences of two anti-alcohol programs that were conducted earlier. Many of the mothers of these young boys were members of saving groups. Considering their intense desire to do something it was decided to cooperate with them. All the saving group members of Ranasan joined in. A meeting of the whole village was called. A village rally was called. It was a well-attended & well spirited event which was successful in sending strong message, there was a minor clash with one of the families involved in production of alcohol. Hundreds of liters of prepared alcohol was drained from every nook & corner of the village and its surroundings. At the end of rally a Gram Sabha was held. The panchayat members also participated in the meeting and a committee was formed. A candle light Shradhanjali was held for those who died, at the center of the village.A memorandum was handed over to the district collector to take effective action. The outcome of the whole process was, the widely open activity of people drinking in public places reduced for some time. The district administration as well as the taluka administration could not take effective measures to control the nuisance which persists with the same intensity now.

Detail of purpose wise utilisation of loan during the year 2014-15

Every saving group is an individual entity with individual bank account. Women are seriously learning day to day management of the groups. Every group is self-help group in real terms, using their own funds and slowly making gradual additions to monthly savings. The total value of savings of all the groups put together amounts to Rs. 9504836 and amount of loans taken to Rs. 29360040. They continue taking loans, repaying it and keep their rupee in circulation.

3. Environment Enrichment Programm: 823 new trees with permanent tree guards were planted this year. With this

there are 1755 trees planted over last six years with survival rate of more than 95%. About 5% trees had to be replanted. The trees & recently planted saplings are being taken care of.

4. Activities for the staff: It is refreshing to break the routine and spend the day in a different way. All

those who participated enjoyed the one day program of tour to Patan's “Ranini Vav”, Tirupati amusement park in Visnagar and darshan of “Unja mataji mandir”.

Occassions like Swami Vivekanand Jayanti & Janamashtmi and national th th

events like 15 August & 26 January were celebrated. To encourage reading habit in staff members and their children, an exhibition of books of the library of Anjali was kept with offer of few books free of cost especially for children.

A sports club was started for the members of staff. “Balpanni Shodhyatra” was organized all over Gujarat by “Shaishav”, a

child rights organization based in Bhavnagar. The children of some of the staff members were taken to adjoining “ Vishwa Mangalam” organization in Anera village when the yatra reached there. The yatra was to high light and provide information about Child Rights & needs of children through games, songs and group discussions.

ACCOUNTING POLICIES:

The Accounts are maintained as per generally accepted accounting principle and are in line with the relevant guideline statements, standards issued by the Institute of Chartered Accountants of India.

METHOD OF ACCOUNTING:

Books of accounts are maintained on historical cost convention on accrual basis.

FIXED ASSETS:

Fixed assets are stated At Cost less depreciation.

DEPRECIATION:

Depreciation is provided on assets on WDV method as under:

(Equipments, Furniture-Fixtures, Vehicles etc.)

In The Current year the assets are shown their respective Cost value and Correspondingly the depreciation fund is created to make the value of assets at its cost.

INVESTMENTS:

Investments mainly FD’S are stated at Cost.

INVENTORY OF STOCK OF MEDICINE:

The Closing stock for medicines as on 31/03/2015 are taken physically & certified by trust, are valued “At Cost”.

Medicine and other Supplies are used exclusively only for regular Hospital indoor & outdoor Patients

Physical checks of the inventory are regularly & periodically done by management.

RECOGNITION OF INCOME & EXPENDITURE:

1. Items of income & expenditure are generally recorded on accrual basis.

2. During the year donation received are as under:

a. General & 35AC Donation Rs.8464772.84(includes Foreign donation Rs.500000/-)

b. Corpus Donation. Rs.6900256/- (includes Foreign donation Rs.167862/-)

c. Hospital Sustenance Rs.419419/-

OTHER NOTES

1. The Trust has paid gross remunerations of Rs.274566.00 P.A. to Managing Trustee Smt. Anitaben Shah as a full time administrator cum Director of community based Development Action (CBDA) Programme

2. The Trust has provided Free Residential accommodation in a campus to Smt Anitaben Shah as a full time administrator cum Director of Community Based Development Action (CBDA) Programme.

3. Rs.180000.00 P.A. is paid to Trustee Dr. Lalitbhai Shah as a Medical Officer In Charge of Anjali Hospital.

According to our professional judgment the above remuneration (as per note 1 & 3) & the said perquisites (as per note 2)are most reasonable looking to their qualification and long experience as compared to any other person having such qualification and experience in other similar organisations. Members of the Board of Trustees other than Smt. Anitaben Shah and Dr. Lalitbhai Shah strongly opined that the services rendered by them to the Trust are very crucial and indispensable to the Trust.

• Expenditure of Anjali Hospital during the year 2014-2015 was Rs. 1.13 Crore more than the income. This Rs. 1.13 Crore of deficit was met from donation income and corpus fund.

• For Bronchial Asthma Centre and Chronic Disease Centre we appeal for yearly contributions.

• Large endowment fund (corpus) is essential to meet the huge yearly deficit. We seek assistance of donors in this.
