Evaluation and Assessment
Selecting one Eval/Assess page at a time allows multiple folks to work on document at same time.
Load pulls in any info alreadyentered or the first text boxes. Add gives you a new field - e.g.additional person testing or additional method of evaluation
Choose Method (RIOT)
Select your assess-ment method
Type in all info for this method & Save
Select “Add” for new method. Present levels and Recom-mendations should be loaded into report from first method
Test Listing and Banks
If you select T-Test, a list of assessments appears. If the one you use regularly is not listed, let Joane know.
BankedStatement
Banked Statements
Like web IEP banked state-ments over-ride text typed in box.
Select banked statements first and add typing next.
Current Health Status
Some fields require a checkmark for No, NA, None
Others require written response

Transition
Transition page is a working document. Add information regarding transition prior to exit – such as date completed.

The Final exit codes are not entered from the transition page, but the exit screen instead. They will be printed on the Transition Page.
Early Childhood Outcomes (ECO)
The ECO must be obtained from the Forms Page. Select “Early Childhood Outcomes” and “Add Form”
ECO (cont.)
At initials, only do the rating. All other documentation from IFSP templates is already recorded on Eval/Assess pages and will be transferred onto the ECO. Progress is required for annual reviews and upon exiting from Early ACCESS.
Prior Written Notice
If you complete other pages related to PWN data fields (e.g. eligibility, services), much of the PWN notice may be done. Verify and edit as needed.

Review and Implementation
At Keystone:
• Office Assistants/Sector Coordinators will review IFSPs
• Office Assistants will implement IFSPs
Pre Plan• Contact your office assistant prior to the periodic or annual
review so she can “Add Child.”She will enter the child’s name/birthdate and the child will show Service Coordinator and Service Provider list
• Once an IFSP has been implemented, you may begin pre-planning the next meeting at any time.
• Appropriate forms for the meeting type designated on meeting pages will be available
• This will give you access to add information to pages such as the Transition Plan, Service Logs, etc., as additions are indicated.
Exit
To complete the final exit for a child, select the Exit button on the IFSP overview screen. Be Careful!!!!
Exit WARNING!!!
Once child is exited, corrections cannot be made. It is essential for the exit code to be 100% accurate. If you aren’t sure, don’t save/submit.
