THEORY & CASE STUDIES

Presentations on various requested topicsSelected topics enhanced by group discussion of case studies

PRACTICAL EXERCISE

Group exercises based on single scenarioInvolves planning, implementation and audit of safety management systems

CONDUCT

Participants are encouraged to ask questions and discuss issues at any time on any subject

Although facilitators may steer the discussion back if we get too far off topic

OVERVIEW

WHAT IS IT ABOUT?

KESELAMATAN

Safety

The state in which the possibility of harm … is reduced to, and maintained at or below, an acceptable level…

ABOUT?

SAFETY OBJECTIVES

PERINGATAN

Warning

The following discussion is confusing even for native-English speakers…… at least it is to me!

ACCEPTABLE LEVEL OF SAFETY

The minimum degree of safety that must be assured by a system in actual practice

ICAO SMM

DEFINITIONS

Safety Measurement

Measurement of high-consequence events & high-level functions

Safety Performance Measurement

Measurement of low-consequence events & low-level

functions

MORE DEFINITIONS

Indicator – the aspect of safety being measured

Target – the objective of our safety actions

Value – the actual number assigned to the above

Paraphrased from ICAO SMM

ONE MORE DEFINITION

Goal – the expression of a safety indicator with a specific target value

ICAO tends to express safety indicators and targets separately each with their own values.

EXAMPLE – SAFETY GOAL

Maximum of 10 breakdowns of separation per 100,000 flight

hours

Grey = Indicator, Red = Target, Blue = Value

INITIAL ALoS

OutcomeMaximum of 5 runway excursions at certified aerodromes

FunctionMinimum of 85% of

aerodrome operator safety audits

completed within 12 months of certification

Selected goals relating to safety measurement

WHY STAGED?

MATURE ALoS

OutcomeMaximum of 25 Cat B

& C runway incursions

Selected goals relating to safety measurement plus

Selected goals relating to safety performance measurement

UNITED STATES

?

The UK CAA has established a range of performance

indicators for monitoring safety performance…to

ensure that the frequency of fatal (an, in some cases,

reportable) accidents does not increase in line with

forecast growthCAP 784 – UK SSP

Australia collects a range of aviation activity and safety information to assess safety

performance and trends, and to highlight areas of emerging risk. A key policy

objective for Australia is the maintenance of positive trends in safety performance across

the aviation sectors.

MAGIC NUMBERS

10-7

Although ICAO provides much guidance on the concept of an ALoS, there are still some doubts as to what constitutes an ALoS and how states should establish one.

PRACTICAL EXERCISE

OBJECTIVE

To perform the tasks of a safety action group tasked with

reviewing low visibility operating procedures and standards

NTSC PRELIMINARY REPORT

a. The National Transportation Safety Committee Director General of Civil Aviation recommend the review the procedures and requirements for usage of airport facilities such as runway lighting, especially during limited visibility, designed to improve safety of the flight.
