TENDER NOTICE – FOR LAB INVESTIGATIONS & IMAGING INVESTIGATIONS
FOR ONE YEAR FROM APRIL 2018
The Medical Superintendent, ESIC Model & Super specialty Hospital, Asramam, Kollam invites
Sealed tenders from reputed laboratories and investigation centers for lab investigations and Imaging
investigations for the period from April 2018 to March 2019
GENERAL CONDITIONS
1. Agencies/Establishments must have at least three years of standing in their respective line of
activity. Evidence to this effect should be reflected in the tender document.
2. Each tenderer must remit earnest money deposit by means of a Demand Draft drawn in favor
of ESIC FUND, Account No. II, of Rs. 5000/-(Rupees Five Thousand Only) and the D.D should
accompany the tender.
3. The contract will normally be valid up to March 2019.. The Medical Superintendent, ESIC
Model & Super specialty Hospital Asramam, Kollam reserves the right to terminate the
contract at any time, before the expiry of one year, by giving one months notice. The contract
can be extended up to a maximum period of one year beyond this period, on the same terms
and conditions.
4. The selected tenderer should deposit an amount equal to 5 % of value of the estimated tender
value for one year as Security Deposit within 7 days. It shall be refunded after satisfactory
completion of contract without any interest. The tenderer should have valid PAN Number and
copy of the same should be attached with the tender document. Income Tax will be deducted
at the rates applicable.
PROCEDURE FOR SUBMISSION OF TENDERS / BIDS:
I. The tender should be submitted in ‘TWO BID’ SYSTEM:-
1. TECHNICAL BIDS: a) TECHNICAL BIDS in one separate envelope, sealed, super scribed with the wordings
“TECHNICAL BID”. b) The original copy of tender duly completed and signed, should be submitted/returned
back, enclosed along with the “TECHNICAL BID”. c) The “Technical Bid” shall contain Earnest Money, in the form of demand drafts as
prescribed herein before, and all the technical details & documents. Each page of the

tender document should be numbered and signed. d) Prices / Costs should not be indicated anywhere in the Technical Bid. This should be
followed meticulously failing which the tender is liable to be cancelled.
2. PRICE (FINANCE) BID:
PRICE (FINANCE) BID is to be submitted in SEPARATE sealed envelopes, super scribed with the wordings “PRICE (FINANCIAL) BID”. This should be followed meticulously failing which the tender is liable to be cancelled.
II. Both the Technical Bid envelope and the Price (Financial) Bid envelope, prepared as above, are to be kept in a larger single envelope super-scribed with “TENDER (TWO BID SYSTEM) FOR LAB INVESTIGATIONS AND IMAGING INVESTIGATIONS FOR ESIC MODEL & SUPER SPECIALTY HOSPITAL, ASRAMAM, KOLLAM FOR ONE YEAR APRIL 2018” so as to give a double cover protection.
III. The outer cover should also be sealed and addressed to the Medical Superintendent, ESIC Model & Super Speciality hospital, Asramam, Kollam, Kerala - 691002.
IV. Tenders submitted without following the 'Two Bid' System procedure will be rejected.
Special conditions for lab investigation / Imaging investigation
1 The lab should have been recognized/approved by the competent authority and should have all in-
house facilities for testing the samples.
2 In case of a tie up arrangement, the lab should have been an authorized franchise of reputed
laboratory for not less than a period of three years.
3 The Investigation Center will provide all the investigations on cashless basis to the ESI beneficiaries
referred for tests/investigations by this hospital. No registration fee or' other fee whatsoever will
be charged by the center from these beneficiaries.
4 Result will be issued to ESI beneficiaries at the earliest In case of specialized investigation the
report should be supplied within three days. In case of emergency the report should be given on
the same day.
5 The lab shall be subject to inspection at the time of award of initial contract and subject to repeat
inspections as desired by the Hospital authorities.
6 Lab should have all the requisite equipment and man power required for testing as per statutory
provision.
7 The lab shall post minimum one lab technician in the hospital in order to draw the sample, label,
collect and transport the sample to lab.
8 The Investigation Center should provide ambulance service for patients who require CT & MRI
scans.
9 Criteria of counting the firm as reputed lab is the sole right of Medical Superintendent, ESIC Model
& Super specialty Hospital Asramam.
Last date of receipt of tender is 22.03.2018 upto 1 p.m
Tender will be opened on the same day at 2 P.M. The tenderers may remain present for the
opening of the tender if they desire.
The Medical Superintendent, ESIC Model & Super Specialty Hospital, Asramam, Kol1am reserves
the right to accept or reject the tender without assigning reason. Selected tenderer should sign an

awarded work.
