SafetyNET – online incident reporting
This presentation gives you some backgroundinformation about the riskNET incident onlinereporting system and takes you through the processof reporting incidents using the online tool
All incidents must be reported to Safety Servicesusing the online reporting system
What are the benefits?
• Quick and easy to use
• Anyone can report
• Paperless
• Same tool for reporting all incidents including fireand security
How do I access the system?
• Via the SafetyServices/SafetyNETwebpage

When using the system, remember to…
• Always use the buttons thatappear in the bottom right handside of the screen to navigatethrough the system – not theback/forward buttons on yourinternet browser
• Use Explorer as your browserand close down the browserwhen you have finished enteringdata (this logs you out of thesystem)
Making your report
• Click on the Report anIncident hyperlink on theSafetyNET homepage
• You won’t need to log in toreport an accident unlikethe other tools availableon SafetyNET
You will also be able to report via thelink that always appears on the lefthand side of the SafetyNET website
and also when you have logged into theSafetyNET tools
Step 1 – about the incident
You will be takenthrough the reporting in
a logical manner
This section captures basicinformation about the incident –
if you have logged into thesystem this will be pre-populated
with relevant information – butyou need check and change, or
enter, as required
Click the next button tomove to the next
screen
Note – all fields markedwith a red asterix* aremandatory – if you don’tknow the information at thetime, enter “not known” andcomplete when all theinformation is available
Step 2 – about thepeople affected
You will now need to provideinformation about anyone
affected by or involved in theincident – this includes
witnesses
If you click Yes, you willbe prompted to addfurther information
When you click the adddetails button this opens
a new screen

The first stage is to enterpersonal information about
the people involved.
If you have logged intoSafetyNET, you can find
UCL employees using thisbutton

If the person was injured orsuffered an illness, this is
where you can enter furtherdetail about the nature of
the injury/illness
You can add details ofmultiple injuries to the
same person if relevant

You can give detailsof any treatment
given at the scene ofthe incident or
elsewhere

You will then be asked toenter some specific
information if the affectedperson is a UCL
employee
When you click next, you willbe prompted to save the allthe information about the
person affected/involved thatyou have just entered

NB: You are only savinginformation about thepeople involved at thisstage, not any otherinformation you mayhave entered
Click the green savebutton once you have
completed all thedetails

Once you have entered andsaved the information, you
can then either edit thatinformation or add the
details of other individualsaffected by or involved in
the incident
When you have entered andsaved all the informationabout those affected or
involved, you are ready tomove on to Step 3.
Step 3 – Whathappened?
Now you need to describe whatactually happened – this is afree text box so you shouldprovide as much detail as
possible
If the incidenthappened on a UCLsite, you can pick the
location and area fromthe drop-down list
And you can givemore site specificinformation here
Step 4 – Where didthe accident occur?
A final set of questions thatrelate to the type of incident
being reported – in somecases, there may be no
further information requiredand you will skip straight to
the submit screen at theend of Step 4
Step 5– Otherrelevant information
Step 6 – Submittingthe report
You now be asked to confirmthe information in the report iscorrect before it is submitted -

Once you havechecked the
agreement boxyou can click the
submit button

You will now get a screen confirmingthat the incident has been reportedand a unique reference number.
If you want a hard copy of the reportnow, click to view the pdf, and you willthen be able to print the documentonce it has opened
You can also save the file too for yourown records
