Practice Edition will enable your practice to offer a new channel of care for your existing population via a HIPAA complaint web application.
LiveHealth Online Practice Edition Present a customized ‘online practice’ privately for current patients and/or to the public. Online
care becomes a valued channel by physicians , the care management team, and your patients

Services: after hours coverage, increased access, management of chronic, enhanced discharge management, proactive outreach to patients

Business Scenarios
Triage
Allow provider to staff their LiveHealth Online practice (instead of “call service” after hours or
“front desk staff “during business hours) with a nurse or other staff personnel who can triage
patients to appropriate setting, schedule appointments (online or in the office for during business
hours), or ‘connect with a doctor ’ a practice MD on call if needed. This function can be used to
support practice scheduling and demand management as it allows channeling patients to
appropriate point of care (office or online)
After Hours Physician Coverage (Activate Retail LiveHealth Online)
Provider to activate/turn on LiveHealth Online “retail” physicians or other designated practices to cover office after hours (instead of call service). A summary of information from the LiveHealth Online visit is available to provider the next morning.
Online Care for Chronic Conditions (Conduct On-going Chronic Condition Check Points)
Following an in-office visit, provider schedules and establishes on-going, required follow-up online check points for patients with various chronic conditions via LiveHealth Online.
Discharge Coordination (Conduct Post Discharge Follow-up)
The care team is aware of a patient discharge via reports from Anthem and contacts patient for a discharge follow-up using LiveHealth Online
Specialty Consultations – (Coming in future version)
Incorporate specialist physician availability and 3-way video dialog, and specialists to the practice edition model
