Working together to provide a caring, quality service
Haxby Group and Gale Farm GP practices have joined forces, following a four month local consultation.
The plan to bring together the two local GP practices was announced in August last year when patients and staff were asked to give their views.
Haxby Group, which already runs four GP surgeries in York and four GP surgeries in Hull, has now merged with Gale Farm, which has two York surgeries. It promises to maintain the traditional values of the existing local GP practice.

Two years-ago Haxby Group patient David Holliday was enjoying his well-earned retirement from local government. He had been a patient at Haxby York for ten years and always took a keen interest in his local surgery.
David, 76, is now part of a group of patients who feel just the same. He chairs Haxby Group York’s Patient Participation Group (PPG) and is urging others to join in.
Set up a few years ago, the group is growing from strength to strength with a new constitution, an elected patient Chair and a secretary. David is confident the PPG is making a real difference to the Practice. He knows that staff listen and appreciate the comments from members of the group.
David explained: “I sort of fell into the job because I’m passionate about giving patients a voice. The Group wasn’t very active, so we’ve made changes to strengthen it and give patients lots of ways to give feedback.
“We’re now looking at new initiatives such as Patient Champions, topical meetings and virtual groups. Anyone can make a contribution as long as they are over 16 and registered with Haxby Group - we’re confident that you can make a real difference to the way the practice operates.
In fact the PPG is currently helping to produce a leaflet to explain how to get the most out of your appointment.

Haxby Group patients will benefit from a new pilot scheme to improve their health and help them stay out of hospital.
Haxby Group is working with a number of health, social and voluntary sector organisations in York to help patients get better support at home. The aim is to prevent people needing to be admitted or readmitted to hospital.
The scheme, known as the “Integrated Care Pilot” is being run by the Vale of York Clinical
Commissioning Group (CCG) and also includes Priory Medical Group, MyHealth and Unity.
By providing extra support for these high risk patients, it is hoped that they will be able to stay at home and avoid going into hospital.
A special York Integrated Care Team has been set up to co-ordinate the care of these patients referred by their GPs. They will review patients’ hospital admissions and discharges, meeting every
day to discuss the care of each patient. A care co-ordinator will also contact patients and work with them to create a care plan and provide the support they need.
Dr Mike Holmes, GP partner at Haxby Group, said: “We’re pleased to be part of this pilot which gives our GPs direct access to a new team which will support our patients. We know from talking to our patients that they prefer to stay at home, but can only do this with extra help.”
