Collecting Data using Google Docs Forms

Have you ever wanted

to invite a group of people

to respond to questions via email?

Do you wish that all the replies could

automatically be

assembled into a spreadsheet format, so

you didn’t have to

copy and paste everything into a format

that was useful

to you? Through Google Docs, you can easily create a

questionnaire

or survey, and then invite others to

respond.

All responses are assembled automatically

into a Google Docs spreadsheet,

which you can save to your computer as

Excel spreadsheet.

LET’S SHOW YOU HOW THIS IS

DONE ! !

Logged in to your

Gmail account

Enter your “Email address”

Enter your “Password”

Click “Sign In”

Click “ ” icon

Click “Drive”

A new Tab will open

On the left side bar

Click “New”

Click “More”

Click “Google Forms”

A new Tab will open

Give your form a “Title”

Click here to change

Highlight it, click

Click the “Delete” key on your Keyboard

Enter your form “Title”

and your Form Description here

Add the “First Question”on the field

You can also add a “Help Text” here

Next, Click on the “Question Type”

And choose “Text” as Question Type

Check this if you wish to make

a required question

TO ADD ANOTHER ITEM OR QUESTION,

Click “Add Item”

Enter the “Second Question”

Add a “Help Text” here if you want

Click the “Question Type”,

and choose another “question type”

Next, Click on the “Question Type”

Click this if you want a required answer

CHOOSING ANOTHER

QUESTION TYPE

After following the same steps in adding

item/question

Click the “Question Type”,

Click the “Choose from the List”

Enter the “First Option” here

Enter the “Second Option” here

Click here if you want to Add more option

Click this if you want a required answer

Just repeat the steps above if you want

to

add more items or questions

You see three icons in theupper-right corner.

The pencil icon stands for EDIT,

The two papers icon stands for DUPLICATE

The trash icon means DELETE.

If you want to delete an Item or Question

Simply click the “Trash bin” Icon

If you want to duplicate an Item or Question

Simply click the “Two Paper” Icon

If you want to Edit an Item or Question

Simply click the “Pencil” Icon

If you’re done with your item or questions,

Click the “Done” button

VIEWING YOUR

LIVE FORM

Click the “View Live Form”

A new tab will open

CHANGING THETHEME OF YOUR

LIVE FORM

Click the “Change theme”

On the right side, choose your Form theme

Congratulations! You now have

Google Docs - Form

SHARING THE FORM

Click the “Send Form” button on the right side

This appears,

Click Google+, FB or Twitter

If you want to share form in this Social site

Enter names, email add, or groups

If you want to share via email

WHAT IF YOU WANT TO EMBED

THIS TO YOUR BLOG?

Click “Embed”

Customize the size of the Form

Click here to highlight the embed code

Once the code is highlighted, copy by

Pressing Cmd + C for mac Users

and CTRL + C for Window Users

Embed the code on your WordPress blog

Open your blog

Click “Post” button

Click “Add New”

Click on the “Text” Tab

And paste the embed code by pressing

Pressing Cmd + P for

Mac Users

and CTRL + P for

Window Users

Click on “Publish”

Congratulations! You have successfully

embedded your first form.
