Endometriosis is a chronic condition that affects a woman’s reproductive organs. It happens

when the lining cells of the uterus (called endometrium) grow outside of it. Those suffering from endometriosis typically have their endometrium on and around pelvic organs such as the ovaries.

This is a common condition and affects about 10 per cent of women.

Its symptoms include: • Painful menstruation • Pain during sexual intercourse • Chronic pelvic pain • Heavy or irregular menstruation • Pelvic bloating (from endometriotic

cysts or enlarged womb) • Subfertility • Bowel-related symptoms during

menstruation such as painful

bowel movements, diarrhoea and constipation

• Urinary-related symptoms during menstruation such as painful urination and bloody urination

TCM AND ENDOMETRIOSISAccording to the theories of traditional Chinese medicine (TCM), endometriosis is known as “menstrual pain” due to its symptoms. It is not categorised as a disease in TCM, and is described as a blood stasis syndrome with formation of abdominal lumps.

Blood stasis is a condition where small vessels are not capable of carrying blood flow normally. It is the cause of severe pain, especially in the lower abdomen during your menstruation, and is believed to be responsible for cases of excessive menstrual bleeding and infertility. Those with blood stasis have abnormal blood flow, which may trigger a negative response from the immune system due to the endometrial cells growing outside the uterus.

HOW CAN TCM HELP Blood stasis is said to be related to a stagnant qi (which restricts blood circulation), and kidney yang deficiency (which affects metabolism). Thus, TCM can ease endometriosis by using:• Herbs that vitalise blood circulation,

including danshen (red sage), chishao (red peony) and taoren (peach kernel)

• Acupuncture to relieve pain, regulate blood circulation and correct qi energy imbalances. Some acupuncture points that are targetted include baihui (top of the head), yintang (halfway point between the eyebrows) and guanyuan (lower abdomen).

TIPS FOR A LESS STRESSFUL SHOT VISIT

BEFORE GETTING SHOTSCome prepared! Take these steps before your child gets a shot to help make the immunisation visit less stressful:• Read vaccine materials given by your

doctor and write down any questions you may have

• Bring your child’s up-to-date personal immunisation records to your appointment

• Pack a favourite toy or book, and a blanket that your child uses regularly

Ways to soothe your baby:• Swaddling• Skin-to-skin contact• Breastfeeding• Offering a sweet beverage like juice

(when the child is older than 6 months)

For older children:• Be honest with your child. Explain that

immunisation shots can pinch or sting, however it won’t hurt for long

• Engage other family members, especially older siblings, to support your child

• Avoid telling scary stories or making threats about shots

AT THE PAEDIATRICIAN’S OFFICEWays to soothe your baby:• Distract and comfort your child by

cuddling, singing, or talking softly• Smile and make eye contact with

your child• Comfort your child with a favourite toy

or book• Hold your child firmly on your lap,

whenever possible

For older children:• Take deep breaths with your child to

help “blow out” the pain• Point out interesting things in the room

to help create distractions• Tell or read stories• Support your child if he or she cries.

Never scold a child for not “being brave”

Once your child has received all immunisation shots, be especially supportive. Hold,

cuddle, and, for infants, breastfeed or offer a bottle. A soothing voice, combined with praise and hugs will help reassure your child that

everything is ok.

AFTER THE SHOTSSometimes children experience mild reactions from vaccines, such as pain at the injection site, a rash or a fever. These reactions are normal and will soon go away. The following tips will help you identify and minimise mild side effects.

• Review any information your doctor gives you about the side effects of vaccination

• Use a cool, wet cloth to reduce redness, soreness, and swelling in the place where the shot was given

• Reduce any fever with a cool sponge bath. If your doctor approves, give paracetamol as a pain reliever

• Give your child lots of liquid. It’s normal for some children to eat less within 24 hours after getting vaccines

• Pay extra attention to your child for a few days

Four-year-old Ian was resting on a dentist’s chair, and it was not just his mouth that was wide open as he got his teeth cleaned.

His eyes, too, were wide open as he watched his favourite episode of

Thomas the Train on a widescreen TV on the ceiling.

At the new paediatric specialist unit of Thomson Specialist Dentistry, the little ones are entertained by books, toys and children’s TV programmes, both before and during their dental treatment.

A GENTLE TOUCH“The main difference between working with children and adults as a dentist is that you need to get children to relax and distract them long enough for you to examine them,” said resident dentist Dr Toh Siew Luan, who has been working with children for more than 10 years now.

For Dr Toh, patience is key. “When the children are nervous, I

will chat with them and explain what to expect, and then ask them to get on the chair when they’re ready,” she said. “Or sit them down with mummy or daddy to calm them. Sometimes it’s the parent who needs to be relaxed! Children often take their cue from their parents, so they won’t relax until their mummy or daddy does.”

TIPS FOR YOUR KID’S FIRST DENTAL VISIT • Play games with your children before they see a

dentist. For instance, role-play and take turns to be the patient and dentist as you do a fun checkup on their soft toys.

• If you are afraid of seeing the dentist yourself, don’t show it to your children. Avoid using words such as “painful” when describing your experience. Stay calm in the dental clinic because children can sense your anxiety and become tense themselves.

• See a paediatric dentist within six months of the eruption of your child’s first tooth, or around the age of one. Give your children’s complete medical history to the dentist.

general anaesthesia if necessary. To Ian’s mother, Mdm Deneuve Tan,

34, it was a relief that her children’s first visit to the dentist was a pleasant one.

“It was difficult to get Ian to brush his teeth, and before this, he had never met a dentist. When he was three, I noticed signs of decay in his front teeth, and he had bad breath. So we decided to take him to a paediatric dentist before he loses his teeth.”

Mdm Tan also brought her 16-month-old daughter Sophie along for a checkup, as her first few teeth had just erupted. “We thought it would be good to get her comfortable with a dentist now, and to start a dental regimen early,” explained Mdm Tan.

Dr Toh managed to keep Ian in the chair long enough to check and clean his teeth, and showed him how to brush it the correct way. As for Sophie, Dr Toh had to work quickly to check her teeth while Mdm Tan held her in her lap.

The visit went well. Ian got his teeth fixed and Sophie received a clean bill of health for her teeth, and both went

home with a little gift courtesy of Dr Toh – an instant photo of themselves as a proud souvenir of their visit to the dentist.

DENTAL HEALTH STARTS YOUNG“Many people have the misconception that baby teeth do not matter since they will be replaced by adult teeth anyway,” said Dr Toh. “That is not true. A decayed baby tooth does not drop out early by itself. If the decay reaches the nerves within, it will still cause intense pain, and extraction will be necessary for serious cases.”

She cited a local 2006 survey of more than 1,700 preschoolers, which revealed that tooth decay affects a staggering 40 per cent of preschoolers. “Most cases go untreated, and it is often too late by the time children see the dentist when they go to primary school.”

It is therefore important for children to visit the dentist within six months of

the eruption of their first tooth or by their first birthday.

“The pain and swelling associated with tooth decay are often obvious only in the advanced stages,” said Dr Toh. Since many young children are unable to express themselves, it is necessary to get their teeth checked and problems fixed at an early stage.”

Any gaps formed as a result of baby teeth lost could lead to uneven growth, which might require the use of braces later in life.

One trend that Dr Toh has noticed is how many local parents seem to be averse to a dental visit

themselves.“For many, it could be because

they were traumatised by their own experience with their dentists in primary school, and this fear has, in a way, influenced their own children,” she said. “So I try to make the experience as enjoyable as possible, especially for the first-timers.”

in Lorong Napiri, where she provides dental services to children with special needs. “Some kids, especially those with autism, require a routine in order to get used to anyone new; I go regularly so that they see the same dentist at every visit.”

To her, there is no greater reward than helping children overcome their fear of visiting a dentist.

“A little girl wrote me a thank-you card, and on it she drew a portrait of me, herself and all the things she loved about my clinic, including the TV and box of rewards and toys,” she said with a smile.

“It’s those little things that help our children take their first step to good oral health – one that will serve them well for the rest of their lives.”

TIP: As you get fitter,

you can hold light 1kg dumbbells to increase the intensity of the

exercise and calories burned.

1. Burpees are a good form of exercise when it is done properly, and at a rapid pace. Exercise the upper body by raising the arms at the end.

2. Recent studies have shown that a higher intensity

exercise, which uses all the major muscle groups of the body, is superior to traditional continuous aerobic-type exercise like jogging to build cardiovascular fitness

and decrease body fat. Burpees should be done, singly, or with other high intensity exercises, for 30 minutes three times a week to reap maximal benefits.

3. You should squat to a level where the thighs are parallel to the ground. Avoid squatting any lower as it puts unnecessary stress on the knees. The back should be kept straight at all times and

the arms raised overhead at the end.

4. Burpees have the advantage of being a simple and effective exercise that does not require special gear or equipment.

DR CHIAM SHARES WHY BURPEES ARE A WINNER:

Extreme athletes and elite military forces do it. Even footballers do it too. Although it is the exercise everybody loves to

hate, burpees can help you boost your strength and endurance. They are one of the most effective exercises because they work every muscle in the body, which means you can burn calories in a short amount of time. And there’s no excuse not to give it a try: this is one exercise that requires little space and no equipment so you can do it almost anywhere, anytime.

HOW TO DO BURPEESStand with your feet shoulder-width apart.Squat down with your back straight and place your hands

on the floor in front of your feet. Kick your legs backwards into a push-up position.Drop to a push-up – your chest should touch the floor.

You can also drop to your knees here if it makes the push-up easier.

Push your chest back up to the push-up position, then

thrust both feet forward so that you are back in the squat position.

Jump up into the air as high as you

can with your arms extended upward.

Land and immediately

lower your body back to a squat position. Repeat Step 2 onwards.

TAKE A RIDE ON THE WILD SIDEGet away from the concrete jungle and connect with nature at Coney Island Park. Here, 50 hectares of wildlife haven lie along a 2.4km trail, which is easily accessible via a rented bicycle from Punggol Settlement just 500m away. The trail is also short enough for a leisurely walk if you prefer. Relax on the rustic beaches or go birdwatching at the three specially built bird hides; you may even spot the free-roaming lone bull who is apparently a permanent resident of the island now. Note: There are no amenities except for a toilet at the eastern end of the island. Bring your own water as there are no shops or vending machines. Wear pants and covered shoes, and put on plenty of insect repellent to protect yourself from the sandflies in the area.

An exciting contest and new awards for Thomson Medical!

We are proud to announce that our Thomson Angels clinched 8 Gold and 77 Silver awards at this year’s Singapore Health Quality Service Award (SHQSA). SHQSA is organised by the SingHealth Duke-NUS Academic

Medical Centre and is Singapore’s first dedicated platform honouring healthcare professionals who have delivered

quality care and excellent service to patients.

To say Dr Tabitha Chng loves children is an understatement. In fact, she discovered her passion for working with kids at an early age. After graduating valedictorian of her class at the National University of Singapore, she pursued a diploma in Child Psychology, which, Dr Chng says, “cemented my lifelong desire to work with children.”

As a paediatric dentist, her main goal is to provide kids with the best level of care in a safe and friendly environment. Her bubbly personality helps in a big way to make children feel at ease

during treatments. She especially enjoys helping children get over their anxiety and look forward instead to their dental visits.

Thanks to a scholarship from the Ministry of Health, she furthered her training at the University of Toronto where she received the Richard C. Pugh Achievement Award, and was awarded the Dr Keith Titley Paediatric Dental Graduate Training Scholarship. Before joining Thomson Dental Centre, she was an Associate Consultant at National Dental Centre of Singapore and is

B.D.S. (Singapore)MSc Paediatric Dentistry (Canada)F.A.M (Singapore)FRCD (C), Diplomate, American Board of Paediatric Dentistry

currently the only paediatric dentist in Singapore to be certified both by the American Board of Paediatric Dentistry and the Royal College of Dentists of Canada.
