• Le statut de la réponse à la greffe est primordial– Efficacité des protocole de sauvetage / rechute à améliorer
• L ’étude SFGM-TC suggère:– Un effet graft versus lymphoma dans cette histologie– Un bénéfice d’un greffon de CSP périphérique (RR de rechute 2.5 si
autre source ?)
• Mais la question de la sélection des patients meilleurs candidats reste posée:
– Pas d’indication évidente en première ligne (les échecs primaires restent chimio- réfractaires… )
– Rechutes précoces (< 12 mois) (cf résultats Coral)– Obtenir une réponse « stable » avant greffe = un challenge

• L’espérance de vie des patients atteints de lymphome folliculaire a sensiblement augmenté ces dernières années
• Comment sélectionner les patients ?
– Rechute après greffe autologue chez les patients en âge de recevoir ce traitement
– Caractéristiques de cette rechute (masse tumorale, transformation)
– S’assurer de la chimiosensibilité
• Problème de la chronicité de la maladie hématologique et du risque de GVHd
