About HCAFHCAF is the premier state association serving state Licensed (Private Funded) and Medicare/Medicaid (Certified) Home Health Agencies and Nurse Registries in Florida. Affiliate members include hospices, homemaker/companion and sitter services, medical equipment supply vendors, computer hardware and software companies, consultants, insurance providers, accountants, data collection, benchmarking companies and many more!

About This ConferenceOur Annual Conference is the largest gathering of home health agency managers within the state of Florida. The conference is designed to offer administrators, supervisors, financial managers and clinicians an opportunity to learn more about new developments in home health care, effective management strategies, cutting costs, innovative programs, and new regulatory changes.

Happy 25th Anniversary, HCAF!At this year’s conference we’ll be celebrating HCAF’s 25th anniversary! We have planned educational sessions that will have an optimistic attitude and will focus on the silver linings and practical strategies to help home care agencies adapt, change and prepare for a new phase of healthcare in America. Anniversaries are a time for looking back and looking forward, a time to recognize the forces and trends affecting the future of home care and a time for recommitting to success. Come learn from industry experts what it takes to hit your own “silver anniversaries” and beyond, as we continue providing the best care possible to patients in their preferred setting: their home.

Home Care TodayHome care in the 21st century is bringing new challenges and enormous potential. Payers are expecting more and oftentimes paying less, while the demand for less costly forms of care are in higher demand than ever as the baby boomers age and wish to stay in their homes. Medicare denials are on the rise and new payment systems are rewarding good outcomes and financially punishing if outcomes are not met. The requirements for data driven results can be overwhelming. The expectation is not only that one has good outcomes, but that the outcomes are expressed through measurable data. Clinicians are being challenged to provide high quality care to more acutely ill patients. The care is expected to produce exceptional outcomes in shorter time frames. In order to survive in this new era, every patient encounter must be clinically meaningful and well documented. Today, healthcare is as much a business as it is a social imperative. Time is of the essence and this conference serves to help home care providers adapt and be armed with the tools and knowledge they need to flourish and prosper in the years ahead. Don’t miss out and register to attend today!

The Art of ICD-9 Coding (Intermediate Level)Sparkle Sparks,OASIS Answers, Inc.This workshop continues the quest for coding accuracy with an emphasis on understanding and integrating the chapter-

specific coding guidelines into your coding practice. Sparkle will include a discussion of the latest CMS guidance for M1012, M1024 and other coding criteria for OASIS-C1. Completion of coding exercises, as well as analysis of common home health scenarios facilitates the integration of these more complex coding concepts. Reminders of general coding guidelines are included but are not the focus of this class. It is highly advised that participants have a working knowledge of basic coding concepts prior to attempting this material. Emerald Diamond

The Art of ICD-10 CodingSparkle Sparks, OASIS Answers, Inc.This new 2-day workshop for 2014 is structured to develop basic ICD-10-CM coding skills and foster good

habits crucial to coding success. Regardless of the postponement this class contains crucial information that can be of great benefit to home health coders now. There are far more similarities than differences between the two code sets and the more detailed guidance provided by ICD-10 facilitates a deeper understanding of coding no matter which code set we are using. This comprehensive workshop highlights the aspects of ICD-10-CM that are relevant for home health & contains suggestions for how coders can truly become proficient prior to the implementation. While the delay is disappointing, this extra time can used to practice using the new codes & minimize the negative financial impact caused by decreased productivity when we do “go live”. Great Hall East (Sun) & Ireland C (Mon)

Blueprint for OASIS Data AccuracyLinda Krulish & Kim Corral, OASIS Answers, Inc.This two-day OASIS workshop is effective, up-to-the-minute education targeted for field data collectors, their supervisors and those preparing for the COS-C Exam,

including education on OASIS-C1 changes. Experience this comprehensive and nationally acclaimed workshop and learn to confidently teach, audit and collect OASIS items accurately. Through guided, expert instruction, and participation in problem-solving discussions and application scenarios, achieve mastery of the OASIS items, conventions, and the latest data collection rules. Rather than provide opinions, assumptions, or unfounded interpretations, the “Blueprint” presenters will model reliance on CMS guidance documents and provide and demonstrate strategies for how to find defendable answers to your OASIS questions. Referral Acceleration WorkshopWilliam Sloan, IMD MarketingThis marketing workshop will provide home care companies with a comprehensive strategic plan that will

result in growth of their company. Bill will provide workable tactics and tools, which will help organize your call schedules, develop your message, increase your sales activities and make recommendations on revenue growth. We will look back at our strategies from the past with a critical eye to keep the one’s that worked and to seek new strategies and tactics that prepare for the future. Silver linings will be discovered by companies who adapt to new marketing strategies and tools that promote growth in each segment of their business. By using data to schedule meetings with targeted clients, home care managers will be able to make decisions on sales call frequency. We will “look forward” to include an internet strategy to communicate with your client, physician, case manager and client’s family. We will review the conversational type presentations “of the past” to the future of using e-mails, videos, electronic pad illustrations and YouTube explanations of our service elements. Bill’s structured program was designed to provide up-to-the-minute statistics of sales call activity for each representative and provide analytics and recommendations to increase effectiveness and the bottom line. Participants will be armed for the future with a strategic marketing plan template and worksheets so that they can build their own plan within the workshop that can be implemented immediately after the conference. Home care is transitioning as all of health care is changing and we must develop a “Plan” to be able to compete in the future. Scotland A/B

The Audit Process Can be Fun? Yes, That’s What I Said.Glenda Burke, Alternatives: A Consulting & Education ServiceDo you believe the audit process can be fun? Maybe that is false advertising. A better promise might be that this session will be fun. Fun or not, the era of greater scrutiny for home care providers is here to stay and so effective, thorough audits

are a necessity for survival. Please come and learn all you wanted to know about the audit process, but were afraid to ask. Audits strike fear in the hearts of most of us. Learn methods to turn this around and make auditing work for us; not against us. We’ll identify steps to develop meaningful audit processes that will produce results that will actually be beneficial in the daily operations and preparation for external audits. Is there a difference between auditing and monitoring? What will I do with results? What if the results look bad? How can I make sure the results are accurate? These are just some questions we will answer in this informative program. England

People: How to Recruit, Hire, and Train WinnersBarry Banther, Banther Consulting CorporationBarry is the author of a #1 Best Seller Amazon, A Leader’s Gift - How to Earn the Right to Be

Followed. Join Barry as he describes best practices that make the difference in successful organizations. There is no winning in business, certainly not for the long haul, unless everyone involved truly wins. This means your business cannot possibly succeed unless the patient/clients that you serve feel like winners, and that is never going to happen unless you have winners touching and caring for those patients. Winning is contagious. You recruit, hire and train winners so they will pass that sense of winning on to every person that they encounter, and when that happens -- every one crosses the finish line in first place wearing a gold medal! England

Transitions Across the ContinuumJudy Fenton, Senior Home Care, An Affiliate of Kindred at Home & Mary Van de Kamp, Kindred at HomeHow can post acute care partners improve patient outcomes, positively impact the acute hospital’s value-based purchasing and reduce 30-day readmissions? Year-two of the Affordable Care Act’s inclusion of 30-day

mortality into the value-based purchasing for acute hospitals brought heightened awareness on the immediate post-acute patient care. Attend this session to learn how facilitating safe transitions can improve patient outcomes by reducing 30-day mortality rates and 30-day readmission rates. What role do you or your organization play in the continuum? England

Customer-Centric Care: The Cornerstone of High Performance Home HealthChristine Lang, National Research Corporation

Driven by changes in payment policy and emerging ideas about how to improve the delivery of care while controlling costs, healthcare silos are breaking down and providers are partnering and administering services in continually evolving ways. HHAs are in a potentially pivotal role: experts at holistically caring for patients in often the most cost-effective and preferred setting,

but hamstrung by legacy rules, an increasingly competitive market, and a general lack of awareness by many others across the continuum around the value that home health can provide. Two things in this uncertain environment are clear: 1) the patient experience can make or break an agency, and 2) objective data is a home health leader’s best friend. In this session, we will focus on these two concepts as leadership tools. We’ll discuss how effectively managing the patient experience and successfully using data can lead to improved cross-continuum partnerships and referral relationships, in addition to better outcomes and financial stability. England

Navigating the Medicare MazeTammy Tucci & Charles Canaan, Palmetto GBAThe maze of requests for claims and documentation information is a daily challenge for all Medicare providers. There are numerous contractors who can review claims and a myriad of ever-evolving Medicare regulations to which Providers

are required to adhere. Understandably, providers can become overwhelmed with the different types of errors identified during the review process. This free 4-hour session is designed for home health providers and their staff to equip them with the tools they need to successfully navigate the Medicare billing, coverage and documentation requirements. Come gain insight for your new, intermediate and advanced staff. Become familiar with the processes for addressing the various components of the benefit integrity process. Palmetto GBA’s ultimate goal is to have educated and astute providers who know how to accurately and skillfully navigate the Medicare Maze!

Specific topics that will be addressed in this session include: root cause analysis; statistics and data analysis; face-to-face documentation; symptoms, signs and ill-defined conditions; medical necessity; Recovery Audit Contractor (RAC); Zone Process Integrity Contractor (ZPIC); Comprehensive Error Rate Testing (CERT); Office of Inspector General (OIG); Strategic Health Solutions, the Supplemental Medical Review Contractor (SMRC); The Medicare Administrative Contractor (MAC); Additional Documentation Requests (ADRs) Process and Procedures; Comparative Billing Reports (CBRs); redeterminations – step by step; Medicare overpayment process; Medicare regulatory updates; web resources; as well as time for questions and answers. Thrive: How to Stop Wishing, Worrying and Whining about the FutureBarry Banther, Banther Consulting Corporation

None of these activities will insure your success or your survival in the imminent future that is already here. Now, was the future only seconds ago, so what are you doing in this present moment to insure your success in a future that soon will be the past? Time is of the essence and spending precious time on Wishing, Worrying, and Whining is wasteful and irresponsible. In our valuable time together, Barry will help place our focus upon what the present moment demands of us, what is needed to thrive in the now so that we experience the bright unfolding future that we all so desire. Bundled Payments: Opportunity, Threat or a Policy Fad?Jeannee Parker Martin, The Corridor GroupBundled payments are one of the most

immediately actionable and immediately rewarding efforts that home care providers can tackle as part of their clinical integration and physician alignment efforts. Bundled payments is an idea that is too bright and appealing to not be tried and on a grand scale. CMS, MedPAC and Congress will continue to push the concept. This session will help the attendee gain an understanding of what the BPCI initiatives are: the impact bundling has on home care providers and help identify some action steps to help your agency get involved in bundling. England

Home Health Sanctions: Avoidance Tips & TechniquesLisa Meadows, Accreditation Commission for Health CareThe new home health sanctions have placed

home health agencies under increased pressure to ensure their staff are adhering to the Medicare Conditions of Participation. Agencies will need to tighten their audit practices to ensure staff is providing care in accordance with the Medicare CoPs. Effective mid-year 2014, these types of deficiencies can create financial havoc on an agency. Learn how to avoid these deficiencies in order to avoid sanctions. Crown

Care Transition and Readmissions - Where are we now?Beth Kramer, FMQAIWhile improvements in care transitions have been made both statewide and nationally, CMS continues to focus on reducing

avoidable 30-day hospital re-admissions as a key goal of the national quality strategy. CMS continues to put a lot of their hopes and expectations in that basket, and no home health provider can afford to be anything less than excellent in the area of transitional care. Ms. Kramer will share the latest Florida

and national readmission trends, as well as the most frequently identified causes of readmission from the home care setting. Discussion will include new home care quality monitoring requirements, the impact of readmission rates for HHAs and opportunities to be a vital partner as health care models become more community-focused. Attracting ACOs II – Utilizing Telehealth Technology to Increase Your BusinessBrett Quas, Connected Healthcare Solutions

Winning ACO business is a competitive process that takes more than just investing in technology. This presentation incorporates updates on the ACO market, telehealth legislation, winning strategies home care agencies are using, and more. In addition to industry experts, our panel includes resources that work in the field every day. Together we will demonstrate ways to market, position and sell your organization’s technology and services. Learn what buyers are really looking for, and how to create a competitive advantage with your telehealth services. From providing the right services to achieving the goal’s “Triple Aim” attendees will learn directly from ACO resources. By better understanding what healthcare organizations are seeking, you will learn how to use technology and the vital data it provides to patient care plans. Ireland C

Home Health & Hospice Forecast: How Providers are Preparing for the FutureDavid Berman, Simione Healthcare Consultants, LLC

This session will briefly discuss what we project future reimbursement to look like in home health and hospice. We will take an in-depth look at how to prepare your agency for the next 3-5 years based on projected reimbursement changes. Models include both operational and financial, as well as strategies for earning buy-in from all staff. Lastly, we will take a look at trends in data that will demonstrate the impact of the changes we have already experienced. Knowing where we have been and the path we traveled to get here -- will surely help us navigate the treacherous road in front of us. Scotland A/B

Celebrating ICD-10: A New Tradition of CodesJennifer Warfield, PPS Plus SoftwareA new tradition is coming our way and the ICD-10 implementation will eventually arrive for all home health care agencies.

Will your agency be prepared to survive this crucial transition? The new coding system will reshape patient care. Join Jennifer as she discusses what your agency needs to know about the new coding system, how it differs from ICD-9 and how to successfully prepare for this transition. Ireland A/B

Partnering with Physicians: New Model StrategiesMichael Blackstone, Suture Health, Inc.Come discuss new model strategies for

partnering with physicians in order to win business and improve care coordination. Too often agency-physician relations deteriorate into burdensome exchanges that must be endured, rather than fruitful alliances to provide optimal patient care. Understand why this happens, what you can do to promote partnerships with your referral sources, and ensure you have a seat at the table. Ireland C

Ten Employment Law Issues That Should Keep You Awake at Night and What To Do about ThemMark Neuberger, Foley & Lardner LLP

Managing employees while staying in compliance with the multitude of laws which regulate the employment process can be a company’s biggest headache. Yet, employment law is an area where preventative medicine can easily lessen your potential for liability, and while at the same time improve your productivity. This program will identify the 10 biggest problem spots for employers in the home care industry and the practical strategies home care employers can and should be adopting. Included in this will be a discussion of the new U.S. Department of Labor Rule expanding the federal minimum wage and overtime law to home care workers. Scotland A/B

Medicare Rate Rebasing and the Face-to-Face Encounter Requirements: The Story Behind These Major Changes in

Medicare Home Health PolicyWilliam Dombi, National Association for Home Care & HospiceIt has been fours years since the Affordable Care Act became law. However, only now are home health agencies truly seeing the impact of the Medicare changes that brought on the rebasing of payment rates and the enforcement of the rules on the required physician face-to-face encounter. This session is a tour through the history of these dramatic changes. What lead Congress to include these game changers in the ACA? What were the options that were considered? Was it all politics or were there real policy goals? Is the current outcome consistent with the original expectations? Each of these questions and more on these changes will be answered in-depth from the genesis of the policies to their implementation by CMS and its contractors. The lessons learned from the experiences with the formation of these health policies and their execution are invaluable tools for influencing the future of home care in Medicare and beyond. England

Building a Strong Compliance Program: Avoiding the Top 10 Deficiencies!Barbara Muntz, Community Health Accreditation Program

We will look at the top ten deficiencies that are tripping up even the best providers, offering guidance based on examples from many successful organizations from across the nation. Excellence should be shamelessly copied. We will focus on how to implement real world strategies to ensure your home care organization is in compliance with Florida State Regulations, CMS’s CoPs and Accreditation standards. Effective July 1, 2014, CMS is scheduled to begin imposing civil monetary penalties on HHAs for either the number of days the agency is not in compliance with the CoPs, or for each instance the agency is not in compliance. Now, more than ever, it pays to focus on proven compliance strategies before the surveyors are standing at your front door! Crown

The Impact of Health Literacy & Improving Patient Outcomes of CareSue Blockberger-Miller, OCS by National Research Corporation

Health literacy has emerged as a key influencing component in improving patient outcomes of care. How could patients not heal quicker and manage their disease processes better when they make informed choices about their health, nutrition and general well being? Little can be accomplished without their help and no health gains can be sustained without greater literacy, which is the first step to greater commitment and improved health. Understanding the factors and implications of health literacy will help your home health agency identify realistic patient-centered clinical practices and achieve improved outcomes of patient care. themselves as the immediate “go to” person for investigating workplace situations caused by staff conflict, customer complaints and potential patient harm. This presentation will cover fundamental sales management principles to assist managers with limited or no sales management experience. This session offers information to enable the home health professional to understand the process of developing an effective compliance program and the necessary elements with emphasis on the audit/monitoring process. The fact is, the information before we even get ill will contribute to a healthcare system that is more self-aware and more efficient. In this session, we will explore the emerging landscape of patient engagement. We will delve into the data gathered and ACO drivers -- all developed in efforts to support the Triple Aim: population health, experience of care and how we can lower costs of care. Ireland C

Care Coordination for Medically Complex and Fragile ChildrenNancy Calvert, eQHealth Solutions, Inc.eQHealth will describe Florida’s program that cares for medically complex children by sharing some redacted stories and outcome data from when it was established until today. Effective Care Coordination is a collaborative effort between the parents, case managers and the child’s service providers. Home Care PAC Annual BreakfastThe Home Care Political Action Committee (PAC) represents the collective voice of the home health industry in advocating for important policies in Tallahassee that affect agencies, patients, and employees in Florida. The Home Care PAC isn’t Republican; it isn’t Democrat; it doesn’t buy votes. It exists to support legislators who stand with home care and understand the importance of the home heath services to all Floridians. Contributions to the Home Care PAC are voluntary and do not come from HCAF membership dues. Your contribution can accomplish so much. It is your voice speaking out about how government should be serving your needs and it gives a voice to the patients who need you to speak up for them! Participate in this breakfast and hear an update on the state of the health care industry in Florida. Check our conference website for specific confirmed speakers. The suggested contribution to attend this breakfast is $50 per person. All proceeds go towards the Home Care PAC’s 2014 campaign to elect home care-friendly lawmakers in the Florida Legislature. Therapist’s Role in Care Transitions and Reducing RehospitalizationsTheresa Gates, Beyond Home Health Care Services

While some home care agencies have educated their therapists on their role in reducing readmissions, for others it’s a huge paradigm shift. Do most therapists understand their role and responsibility in safely transitioning the patient from hospital to home? What expectation has the home health agency set for the therapist’s related to their role in care transitions? This session will offer practical strategies for home care therapists to collaborate with other care team members, use best practices to minimize hospital readmissions and improve the overall quality of the patient’s care. We will also discuss the importance of the hand-off from the discharging facility and how therapists should be utilizing their relationships with their inpatient colleagues to provide their home health patients with a seamless transition home. It’s important that your rehab team is included in adapting to home care’s significant changes, being part of the solution and not just another home care service. After all, patient-centered transitional care is about relationships. A team of health care providers working together to make a lasting impact on the patient’s well-being. Ireland A/B

In this session, we will look at how much change has occurred in the way that families and

professionals can monitor home care services. Twenty five years ago, fewer home care options were available, and very little could be done to optimize efficiency and ensure services were performed. Organizations’ leaders and clinical staff should be more mindful of the consequences of actions stemming from systems and processes. One method for becoming a mindful organization is to develop and implement a safety culture that supports the methods of high reliability. To do this, leadership must be committed to change its course and have a goal of high reliability, implement a culture where staff feels safe to implement and utilize new tools, and adopt a methodology based on factual data driven processes derived from a problem solving methodology.
