Health care is one of the most pressing issues in the 2016 US presiden-tial election campaign. Regardless of the party or candidate you support, the outcome of this election will have a significant impact on our health care system. Since the passage of the Affordable

Care Act in 2010, companies, policy experts, health leaders and health care organizations have been working tirelessly to understand how best to imple-ment the changes that are mandated in the law, and make adjustments to unintended consequences and implications.

With passing of the political torch to a new president and Congress, disruption will occur. How much? Will there be a major overhaul of the ACA and Health Reform? What is the future of Medicare? Of HIEs? At this point, it is hard to pre-dict . . . but by the time this conference convenes, the political question will have been settled and we will have a better idea of the challenges to come.

As health care leaders, we know that we must manage both health care costs and the health of our population. The industry is transitioning from a fragmented volume-based fee-for-service system to one focused on deliver-ing value and patient-centered care. Regardless of the election outcome, a population health approach to health care will continue to be at the forefront of policy decisions and government oversight. The impact the ACA has had on healthcare for the past 6 years is wide-reaching, affecting consumers, busi-nesses, health care providers, insurers, and the overall economy.

This Special Edition of the Population Health Colloquium, led by the team from the nation’s first College of Population Health, will focus on health policy and strategy under the new administration. With keynote presentations from health policy experts, lawmakers and other thought leaders, we will examine how the new administration may influence the ACA, the impact of bundled payment and MACRA, the future of ACOs and new care delivery models. We will discuss how these new systems are improving value and quality of care, with real-world examples and insights from industry and health care leaders.

PRECONFERENCE SYMPOSIUM8:00 am – Noon

5 Key Elements to Implementing a Population Health Strategy

Join us for a special preconference session, reviewing 5 elements to implementing a Population Health Strategy. The key elements include: leadership communication skills to support culture development, patient engagement and activation, the role of health coaches, behavioral economics concepts and principles, and value-based payment programs.

Audience participation will be encouraged and this half day program will conclude with a panel discussion where the participants will try to “stump the faculty” with the chal-lenges they face in their own communities.

At the close of this session, participants will be able to:

• Apply leadership communication skills and practices that positively impact organizational culture and patient engagement;

• Describe and apply a shared model for improvement in patient engagement and activation that highlights specific responsibilities of both the caregiver and patients;

• Write a job description to successfully recruit for effective health coaches

• Understand why population health is easier to implement under value-based payment systems rather than fee for service models

• Apply behavioral economics principles to community-wide population health programs.
