1 2019 Open Enrollment Instructions: Workday Welcome to 2019 Open Enrollment! Elections you make during Open Enrollment will become effective January 1, 2019. Please Note: If covering dependents, you will need their social security numbers as well as dates of birth to complete the enrollment process. Log into Workday directly through https://www.myworkday.com/convatec If signed on the VPN, select “I have a ConvaTec email address”. If you do not have a ConvaTec email address, Select “No CVT email or New Starter”. You will then use your credentials that have been provided to you. 2019 Open Enrollment Instructions: Workday

Welcome to 2019 Open Enrollment! Elections you make during Open Enrollment will

become effective January 1, 2019.

Please Note: If covering dependents, you will need their social security numbers as well

as dates of birth to complete the enrollment process.

Log into Workday directly through https://www.myworkday.com/convatec

If signed on the VPN, select “I have a ConvaTec email address”.

If you do not have a ConvaTec email address, Select “No CVT email or New Starter”. You

will then use your credentials that have been provided to you.

From the home screen, click on one of the “Inbox” icons to access the Change Benefits for Open

Enrollment.

Step 1 of 7

Enter your selections for the Medical/Dental/Vision plans. Select level of coverage: employee only/employee + child(ren)/employee + spouse/family. Select waive to opt-out of coverage.

The next step is to add your dependents. ***Please note: eligibility for dependent children is age 26 for Medical, Dental, and Vision regardless of student status.

Click on “Add My Dependent from Enrollment”.

If you will be using the same dependent as a beneficiary for life insurance benefits, select “yes”. If your dependent will not be listed as a beneficiary, select “no”. Click on Ok to proceed to the next screen.

Add dependent information: Relationship/First and Last Name/Date of Birth/SSN/Gender/Address. Click on Ok to proceed to the next screen.

***Repeat steps for adding dependents until all eligible dependents have been added to Workday.

After adding all dependents in Workday, you will need to add them to the applicable plans. Once your dependents have been added to your plans, click on Continue.

If enrolling in the Cigna OAP CDHP w/HSA and you would like to contribute to the H S A, you will need

to elect to participate AND enter the amount you would like to contribute. If you do not wish to

contribute to the H S A at this time, select “elect” and enter $0.00.

Please note: You can make changes to your HSA contribution at any time by logging into Workday and

initiating the H S A change event.

Click on continue at the bottom of the screen once you have completed your selections.

If you would like to contribute towards any of the Flexible Spending Accounts: Health Care

FSA/Dependent Care FSA/Limited Purpose FSA, you will need to elect to participate as well as enter

your contribution elections.

Please note: If you have enrolled in the CIGNA OAP CDHP with the HSA, you will NOT be permitted to

contribute to the Health Care FSA. You may, however, elect to contribute towards the Limited

Purpose FSA to be used for dental and vision expenses.

To participate in the Flexible Spending Accounts, you must Elect to participate AND enter the amount

you want to contribute.

Click on continue at the bottom of the screen once you have completed your selections.

ConvaTec provides Basic Life Insurance and Accidental Death and Dismemberment Coverage at 2x’s

your base salary. You may elect additional Optional Life Insurance, Spousal Life Insurance, and Child

Life Insurance. Select optional life insurance and coverage level. Click on continue at the bottom of

the screen once you have completed your selections.

***You must be enrolled in Optional Life Insurance to elect Spousal and/or Child Life Insurance.

*** Changes from your current benefit election may require completion of Evidence of Insurability (EOI)

You must enter your beneficiary designation for Basic Life Insurance, Accidental Death and

Dismemberment Coverage, and Optional Life Insurance. You may designate both primary and

contingent beneficiaries. Both primary and contingent designations must equal 100%.

Click on continue at the bottom of the screen once you have completed your selections.

election has been entered, please click on continue.

Step 7 of 7

Please review your enrollment. If you agree with all your selections, scroll down to the bottom of the page. You

must click on “Submit” at the bottom of the page.

Final Step: Review Your Elections and Print a Copy of your

Enrollment. Click on Done to Finalize.

Congratulations! You have now completed your

enrollment in the 2019 Benefit Plans!
