usually considered contaminated unless the clinical his-tory suggests otherwise.2 Correlation of the clinical his-tory with culture results is of paramount importance when practicing postmortem microbiology. In general, routine cultures in cases where infection is not clinically indicated may be a waste of time and resources. If infec-tion is in the clinical differential, however, positive cul-tures in two or more sites are of great significance. For this reason, in cases of possible or suspected infection, blood samples should be accompanied by solid organ cul-tures (eg, spleen, lung, liver) and biopsies of any lesions. See also chapter 29, Postmortem Microbiological Testing.

Obtaining the SpecimenSolid tissue specimens are obtained by sterilizing the surface of the organ with a red hot spatula and using sterile needles (for fluids) or blades (for tissues).1,2,5 Some researchers have advocated using iodophors or other an-timicrobial agents to sterilize the surface instead of the hot spatula.2 Sterile instruments should be used for each specimen. A sterilely excised, 1-cm3 tissue block is opti-mal for homogenation and most closely represents the in vivo infection status of the organ.4

Blood samples are usually taken from the sterilized surface of the right atrium or inferior vena cava and

transferred immediately to culture medium. Right versus left heart blood has been studied but to date is inconclu-sive as to preference.6 Before the autopsy, adequate blood samples may also be obtained from the subclavian area (after thorough sterilization of the skin).5 This can be done promptly even if the autopsy is delayed.5

Cerebrospinal fluid may be obtained by either a lum-bar puncture, cisterna magnum puncture (below the oc-ciput through the atlanto-occipital membrane into the basal cistern), or from the lateral ventricles of the brain after the cranial vault has been removed.7 The fluid can be Gram stained and cultured (Figures 28-1 through 28-3).

TransportSeparate aerobic and anaerobic media are used for the transport of blood. The aerobic bottle may be held for fungal cultures if indicated. Aerobic and anaerobic media are also available for tissue samples. Make sure that the tissue is pushed into the medium of the anaerobic trans-port in order to maintain an anaerobic environment. A special culture broth is used for mycobacterium.

Sterile sections of tissue or a sterile swab from a lesion or exudate may be submitted for viral culture. A trans-port medium must be obtained and then placed on ice for transport. DNA probes have been used on tissue sec-tions to look for viruses such as cytomegalovirus (CMV). Human immunodeficiency virus (HIV) antibodies, hep-atitis C antibodies, and hepatitis B surface antigen can be sought by submitting blood in a red or marbled-red top (serum separator) tube.

A “culturette” containing two sterile swabs may be used for swabbing lesions, abscess contents, effusions, or exudates (Figure 28-4). This particular medium will de-tect both aerobic bacteria and fungi. Also, swabs of the nasopharynx may be placed in viral transport medium for analysis.

Complementary StudiesThe following can be performed on tissue sections: (1) Gram stain/Brown-Brenn stain for bacteria (Figure 28-

5); (2) acid-fast stain and auramine-rhodamine fluores-cent stain for mycobacterium (Figure 28-6); (3) acid-fast stain for Nocardia; (4) periodic acid–Schiff (PAS) stain for fungus; and (5) Grocott-Gomori methenamine sil-ver nitrate (GMS) stain for fungus. Immunofluorescence staining can also be used on tissue sections to look for microorganisms such as Neisseria or Haemophilus spe-cies. Special Gen-Probe swabs may be used for detection of certain organisms, such as Chlamydia trachomatis and Neisseria gonorrhoeae.

HistochemistryIn additional to routine histologic sections with hema-toxylin-eosin (H&E) stain, “special stains” with or with-out polarization may be required on tissue sections ob-tained at autopsy (Figures 28-7 through 28-9). The vast majority of these stains can be performed on tissues fixed in 10% buffered formalin. Table 28-2 lists some common uses of histochemical special stains on autopsy material.

Immunohistochemistry and ImmunofluorescenceAlthough little has been written about postmortem im-munohistochemistry and immunofluorescence, anecdot-al evidence suggests that immunostains can play a useful role in characterizing poorly differentiated malignant neoplasms discovered at autopsy (Figures 28-18 through
