ELA Process Monitoring of Goal
How will you monitor
change in adult
practice? How will you
know if your staff is at
full implementation?
Measure you will use/develop to monitor changes in adult practices:
Weekly Guided Reading Lesson Template (weekly date, group members, reading levels, reading skill(s), and book title.)
Pre/Post Writing Tests for Writing Units (four times during school year)
Process you will use to monitor changes in teacher practices:
Walk-through data – done by leadership, ELA focus team and administration; lesson plan documentation to be reviewed as part of
administrative TKES observations; observations; formative assessment data

What student
achievement measure
will you use to monitor
the impact of this
process?
How will you know if
this process goal is
impacting student
learning (formative &
summative)?
Measure you will use to monitor impact on student achievement:
MAP Testing
Pre/Post from writing units (County/Calkins)
Conference Log
Student Writing Folder
Pre/Post Writing Tests for Writing Units (four times during school year)

MATH Process Goal
MATH Process Goal (Identify S.M.A.R.T. goal here): In 2017 – 2018, teachers will establish and implement weekly classroom practices for students to set goals and
develop action plans and actively monitor their weekly progress toward mastery of math learning target goals through consistent structured support and feedback.
Strategic Action PD Component Person Responsible Resources Needed Date PD will occur:
The teacher will facilitate the unpacking of learning targets
with students at the beginning of each unit so students will
understand their performance level based on the pre-
assessment and use this knowledge to set action plans.
