supporting healthy lifestyles, emotional wellbeing and more, we arm you with tools to take an active role in your wellness.

Getting healthy has its rewards like feeling better, having more energy or experiencing less pain. As an added benefit, you can receive up to $325* per year for completing certain activities and programs.

Rest assured any health information discussed during our wellness programs is not shared with your employer and is kept completely confidential.

The road to good health is not a destination, it’s a journey. And we’ll be there to motivate, inspire and keep you on track.

COMPLETE YOUR PERSONAL HEALTH ASSESSMENT (PHA) Next, the nurse will enter your screening results into your Personal Health Assessment. To complete your PHA, go to rochester.edu/well-u and answer the questions about your health and lifestyle. Based on your PHA score, a personalized health report will be created.

Making healthy choices is easier when you know where you stand. Knowing a few key stats can help you set goals, track progress, and reach new milestones.

Cholesterol Management 3 sessions Work with a dietitian to keep your cholesterol in check and decrease your risk of heart disease, heart attack and stroke through nutrition and activity changes.

Blood Pressure Management 3 sessionsKeeping a healthy blood pressure can lower your risk of heart disease, kidney disease and stroke. Work with a dietitian to lower your blood pressure through nutrition and activity changes.

Tobacco Cessation6 sessionsA tobacco dependence counselor will help you build a plan to manage withdrawal symptoms, prevent and manage relapses, and improve overall health.

Weight Loss 24 weeks | 17 group & 7 individual sessions Receive tools, support, and encouragement to make and maintain lifestyle changes. Individuals must be eligible to participate; eligibility will be determined at enrollment.

Physically Active You8 weeks | 6 group & 2 individual sessions An exercise physiologist will help improve your cardiovascular and muscular fitness, increase strength and flexibility, and prevent injuries.

Weight Loss for Life 12 weeks | 12 group sessions Get help staying on track after completing the Weight Loss program. Receive continued nutrition, physical activity, and behavior support for weight maintenance.

Basics for Building a Healthy Lifestyle4 weeks | 4 group sessions Want to improve your health but not sure where to start? Learn how to prioritize health goals and monitor diet and exercise.

Stress Reduction 4 weeks | 4 group sessions

Learn how stress impacts physical and mental health, monitor diet and physical activity, and assess your work-life balance.

Mindfulness-Based Stress Reduction6-8 weeks | 6-8 group sessions Explore the practice of living mindfully and using meditation to achieve balance, stability and personal satisfaction. May include a day-long silent retreat.

When it comes to improving your health, even small changes can have a lasting impact. Our physician, team of dietitians, and lifestyle counselors will equip you with tools to make healthy changes for life.

If you are living with a chronic health condition, you’ll have one-on-one access to a certified wellness coach. Together, you will create a custom-tailored wellness plan.

CONDITION MANAGEMENT

Asthma Learn how to avoid harmful triggers and properly use medications to breathe easier and feel better. Participants will receive a prescription drug discount on eligible medications.

Atrial Fibrillation Learn how to avoid triggers for uncontrolled A-fib, and take necessary safety precautions to prevent complications.

Congestive Heart Failure Learn how to eat well and move more to reduce strain on your heart. Recognize signs to seek medical attention. Participants will receive a prescription drug discount on eligible medications.

Chronic Obstructive Pulmonary Disorder (COPD) Learn to recognize the warning signs for flare-ups and when to use your inhaler. Simplify daily activities to avoid breathlessness and poor air quality to reduce lung damage.

Coronary Artery Disease Know your target blood pressure and cholesterol levels, and develop a plan to reach them. Create a diet and exercise plan to achieve and maintain a healthy weight. Participants will receive a prescription drug discount on eligible medications.

Diabetes Keep blood sugar, blood pressure and cholesterol in check through proper diet and exercise. Learn to identify symptoms of hypoglycemia and hyperglycemia, and what to do when they occur. Prescription drug discount on eligible medications.

High Blood Pressure Achieve and maintain a healthy blood pressure by developing a diet and exercise plan that will work for you. Learn ways to reduce sodium intake and manage stress. Participants will receive a prescription drug discount on eligible medications.

High Cholesterol Achieve and maintain healthy lipid levels through specific dietary changes, weight control, and exercise. Learn about the harmful effects of tobacco on cholesterol.

Lower Back Pain Learn how to control your low back pain through simple lifestyle changes such as best body mechanics, helpful exercises, and proper footwear.

Stroke Learn self-management techniques and lifestyle changes to deal with the consequences of a stroke. Avoid future strokes by learning to control blood pressure and lipid levels through diet, exercise and proper medication management.
