Public Benefit Assistance Programs: Social Service and Outreach – Title III B – Funded by AgeOptions
Metropolitan Asian Family Services is one of the leading agen-cy in providing social services to the low-income and under-served population. The demand for these services are increas-ing due to the economic downturn and slow recovery of our economy. MAFS staff assists clients in accessing public benefits available to fit their individual needs. Our staff is helpful in filling out applications for these services and escorts them to the appropriate public benefit offices such as Department of Human Services or Social Security Administration, Immigration and Naturalization to submit the applications. We also trans-late official forms in Hindi, Urdu, Gujarati, Spanish and Bosnian languages to facilitate the process of applying for benefits. Our staff acts as liaisons between the client and public officials to help our community members get the help and support they may need. This year we provided assistance to about 900 peo-ple in Cook, Suburban Cook. In DuPage and Lake County we assisted over 50 people.
Nutrition Programs
MAFS is in its 15th year of serving hot lunches to the senior citizen population. These lunches are nutritionally balanced but more importantly they provide an opportunity for seniors to make friends and socialize with each other. These meals are freshly prepared by trained cooks under strict hygienic condi-tions. We provided 33,905 hot meals community this year from our Chicago, Niles and Roselle facilities.
Educational Program
Adult Literacy Grant program, funded by Jesse White, Secre-tary of States
Metropolitan Asian Family Services has accomplished a huge success in providing community-wide resources for South Asians population through Adult Literacy Grant program. The Grant has brought positive impact on the lives of recipients in the areas - citizenship, community participation, family and work life. The grant has also helped our elderly to become more educated and self-confident. After successfully com-pleting 2013-2014 grant cycle, we have been awarded once again for 2014-15. Classes are offered at MAFS Chicago and Niles locations. For more information, please contact - 773-465-3105/ 847-824-9414.
Immigration
Since its establishment, MAFS has and continues to provide help with the immigration and naturalization process for com-munity members. MAFS also helps applicable members of the community in acquiring their OCI card. This year we have helped 300 members apply for citizenship and petition for OCI cards. This service is provided without any outside funding, yet we continue this service to benefit immigrant families.
Energy Assistance Program (LIHEAP)
The Light Heat Energy Assistance Program is designed to help low-income families pay their heat and light bills. This program is offered multiple times throughout the year which brings hundreds of people from all nationalities to the MAFS’s Chica-go and Niles offices. This program is funded by CEDA. MAFS’s staff help the clients in processing their application in order for them to receive the funds from the government. This year we helped 1400 eligible families through this program.
Pink Pashmina Project
A research project for breast & cervical cancer in South Asian Women Metropolitan Asian Family Services (MAFS) is a part-ner to the Asian Health Coalition’s Pink Pashmina Project. This is a community-based program funded by the Susan G. Komen Foundation of Chicago to provide culturally tailored health education to increase breast and cervical cancer awareness. They offer free screenings for limited English-speaking, eco-nomically disadvantaged immigrants and refugee women from India, Pakistan and Nepal.
The main objectives of this project are - 1) To enhance commu-nity awareness of breast and cervical cancer in South Asian women through culturally and linguistically appropriate in-formative education. 2) To promote early detection through routine breast and cervical screening. 3) To identify and ad-dress barriers to screening and care.
Implemented in June 2012 the program has successfully in-creased the awareness among South Asian Muslim women and reduced cancer mortality affecting Muslims and South Asian Women in Cook County. Collaborating with Asian Health Coali-tion has provided MAFS the opportunity to reach more com-munity members.

UMAS Community Care Program funded by the Depart-ment of Aging
Homecare
Homecare program is the cornerstone of UMAS. Our Commu-nity Care Program is exclusively for elderly population. This program signifies our vision of helping the homebound elderly with their daily chores and making a positive impact in their lives. This year our Homecare aides served 2,650 elderly cli-ents, which is a 1.4% increase since last year.
Impact of the Program
This program has an amazing impact on the well-being on the elderly and their families. The benefit of this program is that elderly can stay in their own homes with their families instead of nursing home placements which could be a tremendous financial burden on the State and Federal government. Most of the immigrant seniors are lonely, depressed and suffering with aging complications and are culturally isolated while their fam-ily members are at work. This program offers a unique method of interaction where the seniors feel safe and integrated with society. UMAS employment and training services which have created an army of multilingual caregivers that provide excel-lent HCA services to the elderly population. Programs like this offer jobs to those that may face difficulty in acquiring work due to language barriers. Thereby, this program positively affects the economy by decreasing the poverty level and ena-bling capable persons to work.
Adult Day Care Services
This is a special program provided by Illinois Department on Aging. Adult Day Service (ADS) program was initiated in Illinois for South Asian seniors by UMAS in 1999. ADS provides an op-portunity for the elderly to get involved in structured and fun-filled activities. The purpose is to provide the elderly with a safe, secure and home-like environment outside of their home. This program provides breakfast, hot lunches and snacks to the participants, on-site medical monitoring by trained nurses, yoga and fitness sessions, health education and screening seminars, recreational activities and round trip transportation services from their home to the centers by ex-perienced drivers. This program is currently available at our Chicago, Vernon Hills, Niles, Carol Stream, Naperville and Or-land Park centers, and opening soon in Schaumburg.
Impact of the Program
This promotes a healthy lifestyle and allows the seniors to so-cialize with same-age seniors in a home-like environment. In 2014 we served 252 individuals through our multiple locations, which is an increase of 5.4% from the previous year. This pro-gram also offers opportunities for physical and mental health wellbeing. It promotes new friendships and reduces depres-
sion and social isolation in many cases. These centers also pro-vide an open door policy for seniors to ‘DROP-IN’ to take part in the various services offered at the adult day services center for those who are not approved by the state. Adult Day Service programs keep seniors happy and healthy as well as mentally, physically and socially stimulated.
Future Plans
This will be a period of significant changes for the agency as we are moving toward streamlining and integrating with various Health and Social Services organizations.
Last year we have been awarded new grants such as – Healthy Lifestyle grant funded by Community Memorial Foundation; Adult Literacy grant from Illinois Secretary of State; Human Service grant from DuPage Community Foundation and Cardio-pulmonary Resuscitation grant from Medtronic Foundation of Heart Rescue. We have accomplished a great success in execu-tion of these grants through collaborating with Metro Family Clinic and Universal Metro Asian Services. With the success of
the South Asian Healthy Lifestyle Initiative (SAHELI) project, we are once again partnering with Northwestern School of Medi-cine in community-engaged research project for fiscal year 2014-2015.
Our future plan is to reach out to the targeted ethnic commu-nity in new area of the Western and South suburbs.
Our next goal is a good governance by strategic planning and by streamlining our operation through automation system, we will ensure that all significant proposals/grants are executed expeditiously and efficiently. We are planning to start corpo-rate office in Schaumburg by this year. In addition, forthcom-ing years we will see that our recently opened centers in far suburban areas will continue our legacy of serving and caring with love and compassion.
We will increase our base of resource to undertake new pro-grams and develop them with sustainable funding resources to support our operational costs in the future.
As always, my sincere thanks goes to the Board members, staff, friends and supporters for their hard work and dedica-tion and for their continued commitment in following our prin-ciples and mission.
Sincerely, Mrs. Santosh Kumar

HEALTHY LIFESTYLE
The Healthy Lifestyle Grant funded by The
Community Memorial Foundation (CMF) was
set in motion by Metropolitan Asian Family
Services (MAFS) with the support of Metro-
politan Family Clinic (MFC) and Universal
Metropolitan Asian Services (UMAS). The
goal of this joint venture was to implement a
positive change in the lifestyle of the South
Asian community by increasing awareness
and ultimately lowering high blood pressure,
controlling diabetes mellitus type II
and eradicating obesity. The target popula-
tion was South Asians and Eastern Europeans
who either work or reside within the South-
western and Western Suburbs of Chicago set
forth in the grant provided by CMF.
The project began in January 2014, with the
motto New Year, Healthier You. Followed
by a series of workshops developed by MAFS
along with the expertise of Dr. Manohar
Jethani, Dr. Manjusri Majumdar, Dr. Humera
Khan and Dr. Paula Patel from Metropolitan
Family Clinic (MFC). Through these sessions
the MAFS/MFC/UMAS team emphasized
and increasing awareness about hyperten-
sion (high blood pressure), diabetes mellitus
type II and obesity. Including how the DASH
(Dietary Approaches to Stop Hypertension)
diet can lower blood pressure, glucose and
decrease weight.
The study compromised screening tests for
blood pressure, glucose, HbA1c and BMI.
These sessions also included teaching materi-
al, presentations and contests to facilitate
group learning and interactive fun during
their transition into a healthier lifestyle. On
June 14, 2014, MAFS along with MFC hosted
its final session at 1032 E. Ogden in Naper-
ville, Illinois.
This workshop‘re-screened’ each participant
to collect data which is to be analyzed to de-
tect changes in blood pressure, sugar/HbA1c
levels, and BMI. Participants were elated to
see their levels change due to simple healthi-
er lifestyle adjustments like alternative
eating and increasing activity levels through
routine exercise. There were many partici-
pants who personally thanked Mrs. Santosh
Kumar for providing such a great program
and teaching them how to live a healthier
lifestyle.
MAFS, MFC and UMAS are really impressed
with the participation of our people and we
look forward to working with them, in future
projects.
MAFS, MFC and UMAS would like to person-
ally thank The Community Memorial Founda-
tion for providing MAFS this amazing oppor-
tunity to really help the community. We
would also like to thank all of the employees
who volunteered their time and energy espe-
cially the UMAS staff of the Naperville office
and Karla Guajardo from MFC, Naperville.
GRANT
Starting from July 2013 Metropolitan Asian Fami-
ly Services received Adult Literacy Grant program
from the office of the Secretary of State, Jesse
White. We have accomplished success in provid-
ing community-wide resources for the South
Asian population. By providing one-on-one and
small group literacy tutoring, we have improved
the reading, writing, and English language skills of
all learners who have participated. This Grant has
brought a positive impact on the lives of recipi-
ents in the areas of citizenship, community par-
ticipation, family and work life. It has also helped
our elderly become more comfortable in their
communication skills and commute via public
transportation to different places. Thus, it has
boosted their self-esteem and self-confidence.
After successfully completing the 2013-2014
grant cycle, we have been awarded this grant
once again for the 2014-15 year.

‘Universal Metro Services’ is like … Shri Krishna lifted up ‘Goverdhan’ mountain. This center has brought tremendous joy in the lives elderly from various plac-es. Oh, my God it is overwhelming! We play different games like children, we walk on a treadmill and a bicy-cle in the center to keep ourselves healthy, we do Yog, sing devotional songs and tell jokes which in turn, beautifies our personality. We go to the temples, we go on a picnic and we go to a casino, what a life! We find a doctor and a nurse next to us when we have health problems. We are informed about the knowledge of the world as we to use computers. Oh! What is this! Joy beyond words. Joy….Joy….Joy.

gone but not forgotten
Jayram Bansali October 1, 1941 - March 21, 2014
Mr. Jayram Bansali was a man of strong will power even though he had many health ailments. He was our ADS and Homecare participant. We will miss the echo of his harmonium which he loved to play and his habit of telling jokes. We always found him in a good spirit even until the last breath of his life. Our ADS seniors still always remember him a great deal and pray for his soul.
Shantaben Bodiwala April 5, 1928 - August 14, 2014
Mrs. Shantaben Patel believed in simple living and high thinking. Her moral values of honesty, sincerity and ethics were great for her family and friends. She has been a mentor for her four generations keeping the family together. Umas
took pride in taking care for this special lady for the last fifteen years which added happiness and a peace of mind to the Bodiwala family. According to her this service was a God sent for her. We will miss a great soul.
Mariyam Awal February 14, 2014 - January 24, 2014
Ms. Mariyam Awal was our Homecare and ADS participant more than fifteen years. She was lively, active, and woman of great knowledge of English language and Bible studies. Her fam-ily is thankful to UMAS for providing such a
great service in her acute stage of Dementia. She always played a big role by arranging Carol group and arranging for a priest during Christ-mas time. Our seniors will miss her dearly.
