4. Schedule a meeting with your entire dissertation committee* to complete and sign form.
5. Please have the appropriate Chair/GPD and Academic Dean sign the Evaluation Form6. Submit completed form to UGS no later than May 30, 2015. Incomplete forms will not be accepted.
The intent of the Annual Student Evaluation and Mentoring Plan is to assist PhD and EdD students and their dissertation committee with establishing goals and a plan of action towards degree completion. It is also designed to identify the steps necessary for professional growth within the student’s area of expertise as well as highlight his/her scholarly achievements. The Major Professor and other members of the dissertation committee are required to meet with the student to review and complete this evaluation on an annual basis.* Based on the meeting and completion of this plan, all parties should have a clear understanding of what goals and milestones are to be met within the next academic year.
5. Please take early action so that your dissertation committee has adequate time to meet with you andsubmit your plan to UGS by May 30, 2015. Failure to submit this form will result in a hold on your registration for next year.
