I, the undersigned parent or guardian, hereby give my consent for the above-named child to attend and

participate in the Cheer Clinic. I certify that my child is in good health and is capable of participating

physically in the program. I understand the risk involved in this type of activity and I waive any responsibility

on the behalf of the school or any individual for personal injuries, which may result during participation.

Bulldog Cheer Clinic September 24th

and 25th! Parents are responsible for transporting students to and from the High School for the clinic.

Transportation is not provided by the schools.

The Clyde High School cheerleaders will sponsor the 2012 Cheer Clinic on Monday and Tuesday,

September 24th

and 25th.

The clinic will take place from 4:00 – 5:15 pm at the Clyde High School gymnasium. Enrollment is

open to students ages four years through sixth grade.

Tuition is $25 per student and payment must accompany the registration form. Please make checks

payable to Clyde High School.

Participants will receive their shirts and certificates when they meet us on the track at 6:30,

September 28th

, for the Clyde vs. Breckenridge Football game.

Please pick your child up from the south end of the track after the performance Friday night.

REGISTRATION IS DUE BY 3:30 pm on WEDNESDAY, SEPTEMBER 19TH

We must have the registration in at this time in order to place our t-shirt order.

Late registrants will still be $25, but your child will not receive a shirt.

Registration forms will be available beginning Wednesday, September 5th

at any campus. You may return

forms to either campus or drop them off in the high school office.

Each girl participating will receive a certificate and a t-shirt. Participants will perform on Friday, September

28th

at the Varsity Clyde Bulldogs vs. Breckenridge Buckaroos game at Clyde Bulldog Stadium at about 6:45

pm. YOU MUST HAVE YOUR CHILD AT THE STADIUM BY 6:30 PM. EACH PARTICIPATING

CHILD WILL BE ADMITTED TO THE GAME FOR FREE, BUT PARENTS, FAMILY AND

FRIENDS WILL HAVE TO PAY GENERAL ADMISSION.
