1) Why do you wish to coach within the Brighton Bulldogs Football and Cheer program?

In completing and submitting this application, I understand and agree: that any misstatement of the facts will be sufficient reason for immediate withdrawal of this application or in the event of coaching, is cause for termination. That my previous programs members may be asked for information concerning my coaching, character, ability and experience. That no question on this application has beenanswered in such a manner as to disclose my sex, race, color, religion, or national origin. That if allowed to coach, I might be required to furnish proof of age by birth certificate. I agree to abide by all the rules and regulations set forth by Brighton Bulldogs Football and Cheer and Livingston County Area Junior Football League. I certify the above answers to be true and correct. I understand that any false or misleading answers to these questions will be sufficient reason for termination of my volunteered coaching duties, and basis for termination of coaching. I also understand that investigators will verify my answers and that these investigators have the authority to perform background investigations for any criminal records using my drivers license number. I agree and understand that if selected by Brighton Bulldog Football as a coach, it is strictly voluntary and that I will not be compensated in any way.
