Underground Injection Control Permit Application from PTP to EPA (Eric Byous of Ground Water Office), dated March 2004. Cover letter from PTP Project Manager Ernest Crowley dated April 6, 2004.Prepared by Lumos and Associates. Ernest Crowley's letter states, "We are hoping to start the new septic field this month."The application, dated 4/6/04, lists PTP Corporation as the owner and Ernest Crowley as the operator. It notes two existing underground injection wells and two proposed wells. The names of the field or project is: "Disposal Field A" and "Disposal Field B." The project is noted as being located on Indian Lands.Page 4 of the application lists palns for if one or two Nitrifast systems fail.Page 6 notes that there are no existing EPA permits.The disposal field calculations in Section B, also submitted by Lumos and Associates, lists a dosing interval of 33,400 gallons per day to the system (Disposal Field B).The Sewage Lift Station Calculations, in Section C, state that Ten State Standards are being applied. Area B2 was calculated for 86 lots, using 200 gallons per day per lot, for a total of 17,200 gallons per day.PTP applied for the UIC permit after the EPA required the permit, finding, "injected effluent from the wastewater treatment systems has the potential to contaminate Underground Sources of Drinking Water."
