2 What are your two best practices to implement VBHC?• Improvement by knowing outcomes• Getting right data• Meaningful Patient involvement

What are the challenges of VBHC Implementation?

27

Improvement by knowing outcomes

Results - A short overview:

Continuous learning loops result in improved outcome measures. On an Outcome Measure-level, ambitions can be defined. Outcomes can be measured and compared with the ambition.

You can’t manage what you don’t measure

Becoming the best Prostate Cancer center in Germany (Martini Klinik).

28

Getting right dataInformation Technology systems that allow for better cost allocations are evolving.

• Currently, specialized IT solutions that improve the efficiency of cost allocation processes are evolving.

• TDABC Pilots: map the processes that are supported by value added activities in the care delivery value chain. Time and costs can subsequently be ascribed.

29

Book recommendation: Atul Gawanda, a surgeon, writer, and public health researcher. He practices general and endocrine surgery at Brigham and Women’s Hospital. Professor at the Harvard School of Public Health and at Harvard Medical

School.

Meaningful Patient InvolvementOverview of aspects of patient perspective that can be taken into account: • Patient perceptions • Council membership • Apps / IT involvement • Communication towards patients • Ways of providing information

Try to involve the patient on every level. Surveys, keeping diaries, involvement in councils/teams. Input from patients is very valuable as patients are the customers of care.
