1.  The first thing you must do is go to Survey Monkey   http://www.surveymonkey.com/

2.  Once you have located the web site you should see a window pretty much familiar like this picture.

3.  On the right side of screen will see a box  that indicates “Get Started Today”

4.  For the purpose of showing the students on how to sign up for a free account, we will demo straight on how to sign up for a free Basic Plan.

5.  Click on the “Sign Up Free” button https://www.surveymonkey.com/MyAccount_Join.aspx

6.  Once you have clicked on the “Sign Up For Free” button,  the next screen you should be able to see will be the one where Survey Monkey will ask you question necessary to step ceate an account. https://www.surveymonkey.com/MyAccount_Join.aspx

(a)The First thing you would do is select or Pick a Username: for this training I have selected to input leoyanez

 (b)The second step would be is to select a or choose a Password (always make sure that you use a password that you only know and easy to remember

 (e)The fifth step would be is to leave the check box check or unchecked, this will depend on you for this demonstration I am going to leave it checked  I'd like to receive communications about SurveyMonkey services and survey tips.

 (f) The next and final step before creating you first account on Survey Monkey would be is to click on the Create Account

 Now that you have created your account , you should be able to view

this window, now lets create a survey by clicking Create Survey

  At this point you should view the above window, the first thing we will

do is name our survey , for this presentation we will name it School Lunch

do is select the Create a New Survey button, and then the second step is to name our survey is name our survey , for this presentation we will name it School Lunch The third step is to click on the continue button

  At this point you can use the current template or if want you can design

your own by selecting Create Custom Theme

  The next step is to Edit the Page by selecting the Edit Page Button

At this point you are taken to another page where you will Edit the Page and Body by inputting Enter In a short Title for this page and Enter a short Description of the purpose of this page now both are optional

At this point lets go ahead and save the changes by pressing on the Save Page Button

After pressing the Save Page Button this would be the result, the short title and a short description is in place. Now lets take another step by pressing the Add Question Button

Step One: After pressing the Add Question button the next screen it will take would be the Select a Type of Question

Step Two: We will then select the type of question from the choices it gives us, for the couple of questions we are going to select Multiple Choice (Only one Answer) and then press the Save Changes Button

Step Three: You are now ready to proceed, with the following selections since you already selected the type of question now is time to ask the Question Text and then Answer Choices and also to make sure the students answer the question make sure you check the check box for Requires Answer to Question Check box.

Continue to follow Steps One thru four to add more questions

Once you have completed adding all the questions you have an option to Edit Question, Add Question Logic, Move, Copy, Delete the question by press the appropriate buttons just mentioned

On this page you also have other options you can view such as Survey Options, Print Survey, Restore Questions for this demo we will skip those options for now. Instead we will proceed with Collect Response by pressing the Collect Response button

Once you have pressed the Collect Response button this is the next window you will come up to, here we will send out invitations to collect our data so our next step is to press the Lunch Special

 We are now going to add recipients by pressing the add recipients link indicated by the arrow

On this window we will do the following one we press the radio button  (Add Recipients Manually) and press the I agree with the terms of use button the next window will appear where you will enter the Recipients emails

I have enter the email account , at this point we will edit the message that we will be sending out to the following people, so lets go ahead and press the Edit message link indicated by the arrow

Now we will press the Create New Message by pressing the Create New Message button indicated by the arrow

Now we will select the New Unsent radio button and we will Save Selection & Continue Button to proceed with the message indicated by the arrows

On this next screen on the Subject of Email Message we will put Lunch Special and on the Body of Email we will leave it alone indicated by the arrows at this point we will scroll down the page

Once we scroll done will make sure that on the Reply Email Address to make sure we have your email indicated by the arrow and once we have made sure then we can proceed with the next step by pressing the Save & Preview button indicated by the arrow

This window show you the people who are in your email list, this also show you that are Recipients have been emailed and all have responded. Now our next step is to press the Analyze Results by press Analyze Results button indicated by the arrow

At this point you can view the results, on this survey everyone selected yes on all question. You can also create a Chart by pressing the Create Chart indicated by the arrow
