We are submitting the enclosed documentation to renew our NRC Materials 'License Number 10-06772-02. We have chosen the alternative of submitting onlythe essential information for you 'to assess our current program and thisdocumento Lon' represents only changes that have occurred within our programsince our 4ast license renewal. We have reviewed all of the statements,

procedures, conditions, and representations contained in our license andlatters ' dated January 25, 1985; November 19, 1985; November 25,1985; and %

' March 14, - 1988.

The following changes are needed to our letter dated March 14, 1988:

1. The ZIP code 30344 should be changed to 30333.I ' ' {_

2. The ZIP Code 30344 should be changed to 30333.

3. The phrase, 24,000 curies of Cobalt-60, should be changed to 24,0001 10%curies of Cobalt-60.

' Note that Changes 1 and 2 are typographical errors which occurred when the[|' amendment was being typed. Change 3 represents a specification established by

the manufacturer, Atomic Energy of Canada Limited, before the gammacell wasshipped to CDC.

4. In Amendment No. 4 of our Material License No. 10-06772-02, Iendell|

White's name should be deleted from Item 12 because he does not presently' use the gammacell.

5. In Amendment No. 4 of the license, Linda S. Martin's name should bereplaced by Dr. Alison C. Mawle as the supervisor of the AECL Gammacell1000. Dr. Mawle is well qualified and her curriculum vitae is enclosed.
