CMCO Registration:This module explains the Process Flow of Patient Registration when the patient comes to the

CMCO. 1.The CMCO mithra has to register the patient. Mithra of the referred hospital needs to enter his User Name and Password and Press enter to login in the home page as shown in screen shot-1.

Screen Shot:23.Once you click on CMO RF Entry menu a window will be displayed as shown in Screen shot-2. The below window will be displayed as shown in screen shot-3.

4.Here you need to select the Card Type as 'CMO Reference' and the Registration Type as 'NEW'.select the Card Issue Date by clicking on the Calendar. Enter the socio Economic parametersof the Patient. After entering the entire details click on 'submit' button as shown in screen shot-4.

Screen Shot-45.Once you click on submit button, a message 'Patient Details Captured Successfully, withPatient NO" will Pop-up as shown in screen shot-5.

Screen Shot-56.Once you click on 'OK' as in Screen shot-6, a window will be opened to attach Patient Photo as shown in screen shot-6. Click on Patient Name to view the details and click on Attachment Icon toattach the Patient Photo as shown in Screen shot-6.

Screen Shot:1011.Once you click on Patient Name , a window will be displayed as shown in screen shot-11.Select the Type as ' Referred' and enter the complaint and Diagnosis. Select the Main and Subcategory and also the surgery/ Therapy as shown in Screen Shot-11. You need to select the Hospitalto which you want to refer the patient and finally click on 'submit' button as shown in screen shot-11.

Screen Shot:1112.Once you click on submit button as shown in screen shot-12, a message ' Provisionaldiagnosis completed and patient referred to Hospital-success' will Pop-up as shown in screenshot-13 and then click on 'ok' .

Screen Shot:1213.On clicking OK button the CMCO Referral Card with Card NO. Will be generated as shownin the screen shot-13. Click on print icon to generate a print of the referral card as shown in screenshot-13. The patient has to sign the card or thumb impression of the patient has to be obtained on thecard. Also the card will be attested by the doctor in CMCO. Once the card is signed by the CMCOdoctor, the patient will go to the hospital along with CMCO referral card.

Screen Shot:1314.Once you click on Patient Name as shown in screen shot-6, a window will be displayed asshown in screen shot-11. Select the Type as ' Non-Referred' and enter the complaint and Diagnosis.Select the Main and Sub category and also the surgery/ Therapy as shown in Screen Shot-11. Youneed to select the Hospital to which you want to refer the patient and finally click on 'submit' button as shown in screen shot-11.

Screen Shot:1415.Then click on submit button, then the patient will be converted as outpatient and the belowwindow displays as shown in screen shot-15 and an advice note is given.

Aarogyamithra of the referred hospitalneeds to enter his User Name and password and press enter to login in the home Page 17.Now Aarogya mithra needs to Login and click on 'Referred-CMCO Referred patients View'tab under the 'Operations--> worklist--> Initiate Cases' as shown in Screen shot-16.

Screen Shot:1618.Once you click on CMCO Referred Patients View as in Screen shot-18, a window will bedisplayed and here you need to enter the CMCO Card no/patient id which is issued by CM CampOffice as shown in Screen shot-17 and click on search Option.

Screen Shot:1921.Once you click on Update button, a message 'Patient Details Updated by Mithra' willPop-up as shown in screen shot-20 and then click on OK.

Screen Shot:2022.Once you click on OK button as in screen shot-20, an Vaidya mithra registration form willbe opened . Take the Print out and give the signed copy to the patient The vaidya mithra Registration form has to be printed out and given to the Patient along with thevaidya mithra signature. The network hospital has to supply the statement for this purpose.

Direct Registration :

This Module explains the process flow of patient Registration when the patient comes to theNetwork Hospital.

1.The Network Hospital Aarogyamithra has to register the Patient. Aarogya mithra of the referred hospital needs to enter his User Name and Password and press enter to login in thehomepage.2.After Aarogyamithra login, under patients tab click on " REGISTERED-Register Patient" toregister a patient as shown in screen shot-1.

Screen Shot:13.Now Aarogya mithra will see the Page as shown in screen shot-2. Enter the Health/WhiteCard No. And click on Retrieve Details to get the details of the Patient and all the details will be auto populated. Now you can select the source as 'Direct' which is shown in screen shot-2. Click on submitoption to register this case in that hospital as shown in screen shot-2.

4.On clicking the submit option in screen shot-3, the window will be displayed as shown inscreen shot-4 in which you can see the Patient NO. is generated.

Telephonic Registration

This Module explains about the registration of patient in Preauth Executive login in case ofemergency telephonic approval. Aarogya mithra calls the Preauth Executive and provides all thenecessary details and gets the approval for the surgery.As you are aware Aarogyasri scheme aims at providing treatment for life savingdiseases/procedures and emergency cases in addition to elective cases. In order to ensure that theAarogyasri beneficiary get timely treatment we are here by communicating the process of TelephonicIntimations.To facilitate emergency telephonic intimations Network Hospitals are required to follow theprocedure given below: The person calling from Network hospital can be RAMCO/Treating doctor/Duty doctor who can furnish minimum details of the patient and clinical findings. 4.Enter the 'Hospital Details', ' Approval Details and 'Approval Given By' details as shown inscreen shot-2 and click on submit button as shown in screen shot-4.

NOTE: The telephonic Intimation will be automatically cancelled if Aarogya mithra donot register the case within 72 hours from the Date and Time of Telephonic Intimation Initiated Status.

8.After you click on 'Telephonic Intimation' as in screen shot-7, you can find the cases whichare in "Telephonic Intimation Intitiated' and 'Telephonic Intimation Cancelled' status along with the Registration Date and Time and Telephonic Intimation ID as shown in screen shot-8.

Screen shot-89.Once you click on Telephonic Intimation ID as in screen shot-8 which is in 'TelephonicIntimation Intiated' status, the Registration Page will be displayed as shown in screen shot-9 fromwhich you can register the case and there is link up of Telephonic Intimation " View TelephonicApproval Details" the Intimation taken details available for the same.

10. Now you need to enter all the details in the mandatory fields and click on 'CommunicationAddress' check box if card and communication Address are same and select the source andthen click on Submit button as shown in screen shot-10.

Screen shot-1011.Once you click on submit button, then a message will be displayed as " Case Registeredsuccessfully, with Patient No" will Pop-up as shown in screen shot-11.

Screen shot-1112.Once you click on OK button as in screen shot-11, a Aarogya mithra registration form will beopened. Take the Printout and give the signed Copy to the patient as shown in screen shot-12.

Call Center Registration:

This Module explains the process flow of patient Registration when the patient comes to theHealth camps/PHC's.

1.The Executive CC has to login into the Aarogys sri portal as shown in the screen shot 1.
