Hoops4Health aim is to better the lives of its students. Our goal is to make a positiveimpact on youth by providing them a top notch training in the sport of basketball as wellas teaching them how to live a healthy lifestyle off the court as well as teaching them how

to live a healthy lifestyle off the court. We cater to kids of all ages and skill levels. Our fourkeys are mind, body, nutrition and discipline. We offer a wide range of programs includingbasketball camps/clinics, AAU travel basketball, personal and small group training, videoshooting analysis, nutrition classes, Tai-chi, Kung-Fu, and more. F I RST , S ECOND AND TH I RD GRADE

YOUTH BASKETBALL HOUSE LEAGUEIf you have a rst through third grade boy or girl interested in playing basketball against other area players without the hassle oftraveling to a different location for every game, this program is for them. Participants will learn fundamental skills through weeklypractice and games. Adult volunteers coach all teams. First grade games will have youth size basketballs and eight foot basketball rimsSecond grade will have youth size basketballs and nine foot basketball rims. Third grade games will have adult size basketballs, 10 footbasketball rims and score will be kept. All games will have staff referees and timekeepers. All participants will receive a team t-shirtand commemorative basketball.1. Teams will be established according to school (Crow Island, Greeley, Hubbard Woods, Sacred Heart, Faith Hope, North Shore

Country Day and more), there will be no evaluation. When lling out your registration form please remember to insert your child/childrens school. Otherwise placement on a particular school team cannot be guaranteed. If there are more than 10 children from aschool, multiple teams will be formed. If there too few children from any one school, children will be placed on a team according tovillage of residence. Due to the number of children in this program requests will not be taken.

2. There will be a one hour practice during the week. Time, assigned at a later date, will be sometime between Monday and Friday,5:008:00 p.m. There will be two weeks of practice before the rst game.

3. Games will be held on Saturdays. Time, assigned at a later date, will be sometime between 10:00 a.m. and 6:00 p.m. Games will begiafter two weeks of practice.

Are you a basketball enthusiast who would like to pass on your skills and knowledge of the game? The Community House is looking foadult volunteers to coach in the Youth Basketball Program for rstthird graders. There will be a mandatory coaches meeting on Mon-

M e m b e r s h i p I n f o r m a t i o nEarly registration: Members are eligible to register for all classes one week

prior to the start of seasonal registration. Early registration can be done

via the web at winnetkacommunityhouse.org, fax or in person at Winnetka

Community House. Please note: early registration does not guarantee a

spot in a class.

Who is eligible for benets? All family members in your household are

eligible for membership benets. When registering for membership, just list

your family members, and they will be included for the program registra-

tion benets automatically when registering for classes.

Membership renewal: Members receive a notice about one month prior

to the expiration, so the membership can be renewed for the next year.

Refunds for memberships will not be available.

tion form. Then check the box and add the membership fee in withyour registration.

Prior to any class, registration is reviewed to determine if the enrollmentis sufcient to hold the class. Register early! Do not wait until the day ofthe rst class to register. You may nd the class has been cancelled due to

low enrollment.

R e f u n d P o l i c yFull refunds will be issued for courses that are cancelled by WinnetkaCommunity House due to insufcient registration. Anyone affected will be offeredthe option of attending a session of the same program on a different date as amake-up, or receiving a prorated refund of the series fee for the date missed. Winnetka Community House is not responsible for the supervision

of children on site prior to or remaining on site after the establishedprogram times.

Winnetka Community House is handicapped accessible. If you or any member of your family has special needs, please see our

Executive Director. Winnetka Community House receives no tax dollars. We are supported by

our programs, fund raising events, our annual campaign, and donations. Winnetka Community House assumes no responsibility and carries no

It must be noted that the absence of health insurance coverage does not make Winnetka Community House automresponsible for payment of medical expenses. Due to the difficulty and high cost of obtaining liability insurance, the agency providing liability coverage for Winnetka Community House requires the executhe following Waiver and Release.
