The Use: Scintillation scanning ofthe lungsforinformation on pulmonary vasculature is a valuablecomplement to other diagnostic procedures for thedetection (early) of pulmonaryemboli, lungmalignancy, and other pulmonary disorders.

The Procedure: Briefly, after blocking the thyroid withan iodine preparation, macroaggregated albuminI 131is injected intravenouslyand the lung scannedshortly thereafter. (For actual use information, consultthe detailed package insert and applicable literature.)

The Mechanism of Action: The blood stream rapidlydelivers the macroaggregates to the lung whereverpulmonary blood flow is unimpaired. Mechanicalentrapment of the aggregates in the lung capillarybeds permits visualization of normal pulmonaryvasculature. Subsequent splitting of the largeaggregates yields particles sufficiently small to leavethe capillary bed and enter the blood stream. Liver andspleen reticuloendothelial cells then remove thesesmaller aggregates, and proteolytic enzymes digestthe albumin returning free and protein bound I 131 tothegeneralcirculation.

The Advantages of the Method: This isa simple, safe,fast method which provides the information obtainedby pulmonaryarteriography but without the needforradiopaque material or cardiac catheterization. Allevidence to date suggests freedom fromcardiovascular, immunologic, and radiation hazard.(Nevertheless, see comments immediately below.)

The Risks of the Method: The thyroid is subject tounnecessaryradiation exposure unless blocked withan appropriate iodine preparation. Althoughmacroaggregatedalbumin I 131appearsto be free ofantigenic properties, the possibility of this exists andthe usual precautions should be exercised. Althoughnot clinically observed, some investigators havepostulated the possibility of untoward hemodynamiceffects. (See package insert for further details.)

The Necessary Cautions: As with allradiopharmaceuticals, MAA I 131 should not beadministered to pregnant or lactating women, or topersons under 18 years unless the circumstancesspecifically justify the risk. Radiopharmaceuticalsshould be used only by physicians familiar with theprocedures, precautions, and equipment.

Macroscan-131 is aseptically prepared and non-pyrogenic.It is ready to use and should not be heated prior to use.

INDICATIONS: For scintillation scanning of the lungs toevaluate total, unilateral, and regional arterial perfusionof the lungs.

WARNINGS: Radio-pharmaceutical agents should not beadministered to pregnant or lactating women, or to personsless than 18 years old, unless the information to be gainedoutweighs the hazards. There is a theoretical hazard inacute cor pulmonale,becauseof the temporary small additional mechanical impedimentto pulmonaryblood flow. The

possibility of an immunological responseto albumin shouldbe kept in mind when serial scans are performed. If bloodis withdrawn into a syringe containing the drug, the injection should be made without delay to avoid possible clotformation.

PRECAUTIONS, ADVERSE REACTIONS: Care should be takento administer the minimum dose consistent with safety andvalidity of data. The thyroid gland should be protected byprophylactic administration of concentratediodide solution. Urticaria and acute cor pul- @.monale have been possibly related to the drug.

Radiopharmaceuticals should be used only by physicians who are qualified by specific training in the useand safe handling of radioisotopes and whose experienceandtraininghavebeenapprovedbyan individualagencyor institutionalreadylicensedin the useofradioisotopes.

Note: The Tesuloid Kit is not radioactive. However,after the eluted 99mTcis added, adequate shielding of theresulting preparation should be maintained.Precautions:As in theuseof anyotherradioactivematerial, care should be taken to insure minimum radiation exposure to the patient as well as to all personneldirectly or indirectly involved with the patient.

Note: The Tesuloid Kit was designed to be used withthe sodium pertechnetate eluate obtained from aTechnetope II (Technetium 99m) Sterile Generator. Thelow concentration of polyvalent cations In the Technetope II eluate results in a 99mTc@Scolloid which issuitable for liver-spleen scanning. Use of other sourcesof sodiumpertechnetatehavinga higherconcentrationof polyvalent catiorts may produce an unsuitable99mTc@Spreparation which is not a colloid; this is evidenced by the formation of a flocculent precipitate. Ifsuch a precipitate occurs, the preparation should not beused. It is, therefore, recommended that only Technetope II be used as the source of sodium pertechnetatewith Tesuloidunlessthe userhasdemonstratedthatother sources of 99mTcare consistently compatible andmeet the standards of Technetope II.

TECHNETOPEII (TECHNETIUM99m)STERILEGENERATORprovides a means of obtaining a sterile, nonpyrogenic supply of Technetium 99m (99mTc),a versatilescanning agent that can be administered intravenouslyor orally. 99mTc,the short-lived daughter (T'/2 6 hours)of Molybdenum 99 (99Mo,T'/2 67 hours), is obtainedfrom the generator by periodic elution. The amount (inmillicuries) of99mlc obtained in the initial elution willdependon theoriginalpotencyof thegenerator,whilethe activityobtainedfromsubsequentelutionswill depend on the time interval between elutions.Warning:Properradiationsafetyprecautionsshouldbemaintained at all times. The column containing 99Moneednot be removedfromthe leadshieldat anytime.The radiation field surrounding an unshielded column isquite high. Solutions of 99mTcwithdrawn from the generator should always be adequately shielded. The earlyelutions from the generator are highly radioactive. Forradiation protection, a lead shield for the collecting vialis included with Technetope II.Important: Since material obtained from the generatormaybe intendedfor intravenousadministration,aseptictechnique must be strictly observed in all handling. Thestoppers of the eluent bottle, the elution tube, the evacuatedcollectingvial,andbothrubberclosuresin thegenerator column should be swabbed with a suitablegermicide before entry. All entries into the generatorcolumn must be made aseptically. Only the eluent provided should be used to elute the generator. Use a freshmilking tube and collecting vial for each elution; sufficientequipment is provided for this purpose. All equipmentused to collect or administer the 99mTcmust be sterile.
