You will receive a text message and/or email (to your umb email only) reminding you to complete the survey in the morning. The email will include a link to the student My Health Beacon portal, where you can access the daily symptom survey by logging into your account.

Below is an example of the daily reminder that will receive.

“Your symptom screening survey is now available on the <a href="https://myhealthbeacon.umb.edu/"> MyHealthBeacon portal</a>. Please log in and complete it within the next 24 hours”.

You only need to complete the survey on the days that you come onto campus. If you live in the dorms, then you’ll have to complete the survey daily.

To access the survey, click on the link “Survey Forms” located on the left-hand column.

Next click on “Daily Covid-19 Screening (UMB)”.

Now click “Continue” to access the questions.

Answer all the questions and click "Continue".

The page above will appear after you click "Continue". The page will indicate if you've successfully completed and submitted the survey. Click on “Show Badge” to see your badge and the status of your Covid-19 health requirements.

If you have overdue items, such as submitting Covid-19 vaccination records or weekly testing, then your clearance status will register as overdue. Also, if you do not complete the daily Covid-19 survey your badge will register as overdue. You may be asked to show your badge to a public health official so please be sure to satisfy all your Covid-19 health requirements.

If you are not fully vaccinated then your badge will register as "Not Vaccinated" until 2 weeks after your 2nd dose in a 2-dose Covid-19 vaccine series or 2 weeks after a single-dose vaccine.

Remember that if you are not fully vaccinated you will be required to test weekly, and your badge will register as overdue if you are not tested at least once per week. If you test on Tuesday, then you’ll have to test no later than the following Tuesday to avoid an overdue lab requirement.

Students covid testing at UHS for the first time must complete the UHS registration form prior to scheduling an appointment. Failure to complete the registration form prior to scheduling an appointment will result in a much longer wait time. You only need to complete the registration form once.

Once you satisfy the lab testing and/or survey requirement(s) then your badge will indicate the next due date.

Not all students are required to test weekly and will not have the "Lab Test" field in their badge

Your Covid-19 vaccine status will register as "Vaccinated" once you are fully vaccinated. If you have an exemption to the Covid-19 vaccine requirement then your badge will register the exemption.

If you are not yet fully vaccinated your badge will register as "Not Vaccinated".
