FOR YOUR INFORMATION

Congratulations!

What's Ahead for Masonic Villages - Part 2by Cindy Phillips, executive director This article is the second in a series explaining the expected trends for each care level of our campus and the strategic plans we developed earlier this year. While it is very difficult to see too far into the future of our industry, we are very proactive in ensuring we can continue delivering our mission. We have been licensed to provide personal care services for almost 20 years. Many people believe it is the same as assisted living, but there are subtle differences. The most significant ones are in construction; presently our rooms would be too small to qualify for assisted living licensure. Also, the staffing requirements for assisted living are slightly higher than personal care. Both programs were developed to provide a level of care before skilled nursing and to save the government Medicaid (Medical Assistance) dollars. Currently in Pennsylvania, there are under 50 licensed assisted living residences compared to over 1,000 personal care homes. We currently are able to serve 135 residents, having recently added eight rooms including some larger suites, three with small kitchenettes. In that same renovation, we created a beautiful new restaurant and dedicated hair salon. We now have more amenities for our residents, and it has made us more competitive in the marketplace. These are the critical factors impacting us in personal care the most right now: Resident Needs Residents are moving into personal care, often later and with increasing needs. Interestingly, our average age in personal care is higher than in skilled nursing in the Masonic Health Care Center by over two years, and the average medications per resident are also typically higher. These extra care needs have warranted the addition of an RN to the staff in recent years. Occupancy Our occupancy continues to remain at 97 percent or higher on a consistent basis. You may ask, do we need to grow our personal care capacity? This is hard to answer as the concept of “aging in place” and the additional funding for home and community-based services may keep people at home even longer. As of now, we do not plan to expand the personal care building any further, but many things could cause this to change. Payer Mix Most staff do not realize that within personal care, the number of residents receiving Compassionate Care has increased steadily to near 60 percent. Again, this is part of our Mission; however, besides a resident's monthly income (including supplemental security income), we get no other government reimbursement for this care. In total, we provide almost $4 million in charitable (free) services per year to our residents in personal care. On a positive note, this year we began working with a Veterans Affair program to secure funding support for our veterans and their spouses, and we are hopeful this will help offset part of these Mission dollars. Additionally, Leading Age PA is advocating for Medicaid-like funding for assisted living and personal care residents who are income qualified, which would also help with our residents who can no longer pay for care. Here are some of the future considerations in personal care we are planning for: If home and community-based services like home care and home health grow as expected, many of our residents will remain in their apartments and cottages longer. That is better for the resident and better for us. It would also help ensure our 135 beds will adequately serve our campus and limit the need for expansion. Several times over the past 10 years, we've considered creating a memory care area for personal care. With the increase in dementia across our population, we believe there is a strong likelihood we will need this at some point, even allowing some of our higher functioning residents from the Lafayette neighborhoods to be able to move into it as well. As more services are being done as outpatient and no longer in hospitals, we expect fewer patients to come to our Transitional Care Unit and even our skilled nursing. There may be a need for patients or residents to either come directly from the hospital or from an outpatient procedure to personal care. For insurance providers, it's a lower cost alternative, and they will drive that type of change. These potential "respite" or even "rehab" stays may create a new demand for a short-term personal care offering. These are the most significant trends we see coming, but as cost reductions occur to Medicare and/or Medicaid, personal care may be even a more critical part of our continuum than it is today. We are ready to adapt and continue providing the best care and living environment for our residents.
