dear patient, you have been diagnosed with heart failure, also known as congestive heart failure or CHF. heart failure
means that the heart is not pumping blood through the body as well as it should. while there is no cure
for heart failure, many people with this disease can improve the function of their heart and lead active lives
because they have learned to take good care of themselves.
you can take control of your heart failure by doing the following:
take all of your medicines, never skip doses
do not let your medications run out
keep all your healthcare appointments, even when you feel well
follow a low sodium (low salt) and low fat diet
remain or become physically active
stop smoking
weigh yourself everyday

GREEN – Goal Zone.Your symptoms are under control ● Able to do normal activities ● No shortness of breath or usual shortness of breath ● No weight gain or less than 2 pounds in 1 day ● No swelling or usual amount of swelling in your feet, ankles, legs or stomach ● No chest pain
YELLOW – Warning Zone. Caution
ACTION – call your healthcare provider or the Heart Failure Center
● Normal activities are harder due to symptoms ● Gain 2-3 pounds in 1 day or 5 pounds in 1 week ● More shortness of breath than usual ● More than usual swelling of feet, ankles, legs or stomach ● Dry hacky cough ● Dizziness ● Uneasy feeling; you know something is “not right” ● Hard time breathing when lying down (need to sleep in chair or use extra pillow) ● ________________________
RED – Emergency Zone.
ACTION – call 911 or go to Emergency Room
● Unable to do normal activities at all due to symptoms ● Unrelieved shortness of breath when sitting still ● Chest pain unrelieved by rest or nitroglycerine ● Confusion or unable to think clearly ● Faint or pass out ● Breathless or are unable to talk ● Continuous rapid, racing heartbeat
