HOW WE CAN PROVIDE QUALITY AND TAILORED SERVICES TO YOU

We will work closely with you to understand your situation and provide the quality support that you need. This can be provided through a home care package, disability package or can be individually funded.

If you have reached your limit or are not eligible for government funded services, or wish to pay for additional support tailored to your needs, we have a range of services to assist you. You do not need a referral and there is no waiting list.

We offer coordinated and quality services, tailored to meet your specific care needs.

These services are ideal for

• Busy families in need of extra support for a parent or loved one living with dementia.

• People who may not be eligible for government funded services or who wish to ‘top up’ the services they are already receiving.

• People who require one-off service or support on an ongoing basis.

• People who need extra support to enable then to remain at home for longer.

SERVICES FOR YOU

How we help you and your loved ones

Home and community respite

We provide a range of respite and ‘time out’ options for families and carers of people with dementia. In-home respite is provided in the person with dementia’s home. Our social support is a one on one service providing respite in your chosen places in your local community. We can also provide services like overnight support or companionship once a person has transitioned into residential care.

Our services focus on providing the person living dementia with meaningful and enjoyable activities, while providing the carer with time to themselves.

Day Centres

Based around a household model, our community wellbeing centres in Perth, Albany and Mandurah provide a welcoming, relaxed but therapeutic environment. They offer a wide range of meaningful activities based on a person’s likes and interests. The centres provide meaningful engagement, enjoyable activities and friendship opportunities in specially designed dementia friendly locations.

Friendship Clubs and Social Support

Groups provide a safe and fun environment for people living with dementia to socialise, remain active in the community, make friends and maintain relationships. We offer these groups throughout the metropolitan area and in some regional centres.

Members have input into club activities to ensure they have a sense of belonging and club ownership. Our team will talk to you about the different groups to fit your specific needs.

Carer Support Groups

Our Carer Support groups provide a specialised support service for people who are caring for a loved one living at home and in the community. These groups meet monthly and provide an opportunity to meet others in a similar situation. In these groups, carers can discuss issues they might be facing, receive information, emotional support and share tips with each other. These enable carers to build a support network and link in with our team who can direct them to the support they need.

SERVICES FOR YOU

Counselling

Our counselling service provides short term dementia-specific counselling and support across the state to people with a diagnosis and their carers. This service can be offered in one-to-one and group counselling sessions delivered by counsellors with experience in dementia. Due to the transitional nature of dementia you can reconnect to the specialised knowledge and understanding we provide as needed if new challenges arise.

Education

We offer a range of workshops and courses that will enable couples, individuals, friends and family members to better understand dementia, and to support and communicate with the person with a diagnosis. Workshops cover a wide range of topics including meaningful engagement, communication, adapting your home, introducing creative arts, grief and loss, caring for the carer, legalities, services and more.

Younger Onset Dementia (YOD)Key Worker Programme

Our programme provides individualised services and support for people living with younger onset dementia (under 65) and their carers. Key Workers act as a primary point of contact for individuals and carers living with YOD.

Enabling Environments

A well-designed home environment can maintain abilities and provide independence for a person living with dementia at home. The project team can provide a review of your home with recommendations on changes and adaptations to enable a more dementia friendly environment.

SPECIAL PROGAMMES

We work with people living with dementia to provide meaningful, enjoyable pastimes that have both proven therapeutic value and more importantly value to the person with dementia.

Men’s Shed

Being a part of a Men’s Shed brings opportunities to participate actively in a community with other men from diverse backgrounds and make new friends. Sheds are a common shared space for social interaction and help maintain strength, coordination, thinking skills and emotional well-being.

Choir

It has been proven that when used as therapy, music has positive effects on our physiology, affecting our heart rate, blood pressure and nervous system. Our ‘Friends in Harmony’ choir is comprised of people living with dementia and their carers.

This specialist programme enables people to connect into communities whilst also keeping physically, mentally and socially active. It allows people to feel valued doing things they love, while making a real difference.
